IT-Assisted Process Management in Healthcare

Clinical processes need to be well understood before a new health IT tool can be introduced. Observations, interviews, surveys, or documentation analysis are carried out to systematically collect information to better understand a clinical process. To aggregate and visualize the collected information about a clinical process, use case diagrams can build a basis. Formal process models such as process chain diagrams or BPMN diagrams are well suited to model the process in detail. The objective of this chapter is to discuss these methods for analyzing and modeling clinical processes, as this is an important precondition for systematic process management in health care.

Innovation in IT solutions introduces new usages in ehealth that allows the patient to be more active in his (her) health care and well-being. Strong expectations on electronic exchanges or shared medical documents from healthcare professionals and patients are pushing decision makers and stakeholders to develop quickly efficient and structured data enabled health IT infrastructures. One of the challenges of this paradigm shift is to provide at the point of care interoperable solutions in full compliance with regulations, ethics, organizational and technical states of the art. This paper describes this approach developed as a four steps process and defines the basic concepts for designing an interoperability infrastructure. For each of the steps, examples will be provided and discussed accordingly with the different interoperability layers (legal, policy, process, information and technical) developed in the Refined eHealth European Interoperability Framework (ReEIF) (eHealth Network, 2015).

Care’ is a shifting, plural word when used in the context of discussions of health. It suggests attention and compassion when articulated as a verb, but has overtures of regulation and control when used as a noun – to be ‘in care’ is usually not unproblematic. Two chapters in this section – those by Sarah Atkinson and Lucy Burke – speak specifically to the complexities of this idea. As Atkinson makes clear in her chapter, care invokes questions of resource just as much as it outlines interpersonal relationships; it presents what she terms ‘dilemmas, paradoxes and challenges’ when conceived of as a totality and, especially in global contexts, suggests entangled modes of time and space.

This paper examines the rationality of the concepts underlying evidence—based medicineand health technology assessment (HTA), which are part of a new current aimed at promoting the use of the results of scientific studies for decision making in health care. It describes the different approaches and purposes of this worldwide movement, in relation to clinical decision making, through a summarized set of specific HTA case studies from Catalonia, Spain. The examples illustrate how the systematic process of HTA can help in several types of uncertainties related to clinical decision making.

O processo de adoecimento dos homens vem sendo determinado pelo seu comportamento na sociedade e como expressam suas crenças de masculinidade. O objetivo deste trabalho foi identificar as ações realizadas pelos enfermeiros da Atenção Primária à Saúde (APS) direcionadas à promoção da saúde do homem. Trata-se de uma pesquisa descritiva, exploratória e de abordagem quantitativa. A pesquisa foi realizada com 29 enfermeiros de serviços da APS do município de São Carlos-SP. Os dados foram coletados por meio de uma entrevista, utilizando um instrumento validado. Os dados foram analisados por meio da estatística descritiva. A pesquisa foi aprovada pelo Comitê de Ética em Pesquisa. Os resultados mostram que 65,5% (19) dos enfermeiros referiram não ter recebido capacitação sobre a saúde do homem. Quanto aos fatores facilitadores do acesso dos homens nos serviços de saúde, destacam-se que 19% (11) das respostas incluíram o vínculo estabelecido entre os profissionais e usuários; e, como agentes dificultadores, em 35,1% (19) das respostas foi ressaltada a cultura do homem. Para uma maior efetividade da assistência à saúde do homem, torna-se necessário a sensibilização de profissionais e gestores de saúde na busca de estratégias para facilitar o acesso da população masculina aos serviços de saúde.Descritores: Saúde do Homem, Atenção Primária à Saúde, Enfermagem. Fragilities and strengths of man's health care in primary health careAbstract: The illness process of men has been determined by their behavior in society and how they express their beliefs about masculinity. The objective of this study was to identify the actions performed by nurses of Primary Health Care (PHC) directed to the promotion of men's health. It is descriptive, exploratory, and quantitative approach research. The research was conducted with 29 nurses from the PHC services of the city of São Carlos-SP. Data were collected through an interview using an instrument validated. Data were analyzed using descriptive statistics. Still, data collection began after approval by the Research Ethics Committee. The results show that 65.5% (19) of the nurses reported not having received training on men's health. Regarding factors that facilitate men's access to health services, it is noteworthy that 19% (11) of the answers included the bond established between professionals and users; and, as hindering agents, 35.1% (19) of the answers highlighted men's culture. En cuanto a los factores que facilitan el acceso de los hombres a los servicios de salud, cabe destacar que el 19% (11) de las respuestas incluyeron el vínculo establecido entre profesionales y usuarios; y, como agentes obstaculizadores, el 35,1% (19) de las respuestas destacaron la cultura de los hombres. Por lo tanto, para una mayor efectividad de la atención médica de los hombres, los profesionales de la salud y los gerentes deben ser sensibilizados en la búsqueda de estrategias para facilitar El acceso de la población masculina a los servicios de salud.Descriptores: Salud del Hombre, Atención Primaria de Salud, Nursing.

This article considers approaches to tracking changes in the productivity of a health care system over time. Economists generally believe that measured productivity growth in the sector is biased downward because of difficulties in measuring medical care output accurately and also that measurement errors are pervasive in some of the inputs. This article discusses the most essential tasks of getting the data right in all productivity measurement. It also discusses some of the economic implications of the fact that medical care, though it is demanded to improve health, does not, by itself, produce health. It considers an alternative concept for medical care output and necessity to separate trends in medical care expenditures into trends in quantities of treatments and prices or costs of treatments. Improving the database for the analysis of medical care productivity deserves high priority because vital questions of health care policy demand exactly the same data.
