BackgroundAn international six-centre randomised controlled trial comparing individual placement and support (IPS) with usual vocational rehabilitation for people with serious mental illness found IPS to be more effective for all vocational outcomes.AimsTo determine which patients with severe mental illness do well in vocational services and which process and service factors are associated with better outcomes.MethodPatient characteristics and early process variables were tested as predictors of employment outcomes. Service characteristics were explored as predictors of the effectiveness of IPS.ResultsPatients with previous work history, fewer met social needs and better relationships with their vocational workers were more likely to obtain employment and work for longer. Remission and swifter service uptake were associated with working more. Having an IPS service closer to the original IPS model was the only service characteristic associated with greater effectiveness.ConclusionsThe IPS service was found to be more effective for all vocational outcomes. In addition, maintaining high IPS fidelity and targeting relational skills would be a valuable focus for all vocational interventions, leading to improved employment outcomes. Motivation to find work may be decreased by satisfaction with current life circumstances.

Research article introductions (RAIs) play a significant role in gaining publication, and therefore have been studied by many applied linguists. Research into RAIs published in Indonesia has begun to be developed (Adnan, 2009; Mirahayuni, 2001; Safnil, 2000), and generally conclude that Indonesian Humanities RAIs were structured differently from English RAIs. However, as these are early studies, their findings still awaits scrutiny, and little information on Indonesian RAIs especially in Education has been published. Several models describing discourse structure of research article introductions have been proposed, but they have been problematic when applied to analyse RAIs. This paper reports an examination of the applicability of two important models, the CARS (Swales, 1990) and the PJP model claimed to be an Indonesian model of Humanities RAIs (Safnil, 2000), using a selection of 21 Indonesian research article Introductions (RAIs) written by Indonesian academics in Education. It concentrates on the following questions: To what extent do these models fit the data and why? The examination found that none of the RAIs fit the CARS, and only less than half fit the PJP model for various reasons. Therefore a new model is proposed. This model fits most of the data. The paper claims that apart from national concerns, discourse patterns of RAIs are also affected by writing guides provided by the discipline.

Introduction Assessment of the effectiveness of individual placement and support in forensic mental health settings is a relatively new field of research despite evidence demonstrating its effectiveness in generic mental health settings. Method IPS was implemented into a community forensic mental health setting within a large National Health Service trust in the United Kingdom over 6 months. Using the Consolidated Framework for Implementation Research this paper describes the lessons learned from implementing individual placement and support into such settings. Results Our findings suggest that implementation of individual placement and support in forensic mental health settings is complex and requires robust planning and collaboration with internal and external agencies. Barriers to implementation included staff attitudes, difficulty engaging employers and lack of employment related performance indicators, and facilitators included the support of service managers and outside groups. Adaptations to the IPS model were made to address challenges encountered, including difficulty starting rapid job searches, concerns about stigma, lack of confidence, uncertainty around employment opportunities, offence restrictions and lack of interest from potential employers. Conclusion This paper adds to the limited literature in the field. Findings are relevant to practitioners and service providers who wish to implement individual placement and support services for people with mental disorder and offending histories.

Abstract KGF (FGF-7), an epithelial cell growth factor, can lessen the injurious effects of post-BMT lung injury (IPS) and GVHD when given prior to conditioning of the recipient. We reported that mice treated with KGF pre-conditioning had elevated systemic levels of Th2-type cytokines at the time of BM infusion which was associated with less GVHD. In non-conditioned SCID mice that developed lethal GVHD from allogeneic T cells, KGF pretreatment also increased Th2 cytokines implying that KGF alone was responsible for the systemic Th2 milieu and not a response that was dependent on conditioning. In the post-BMT lung, KGF pretreatment did not alter the cellular composition of the inflammatory infiltrate but resulted in decreased levels of immune activation markers compared to non-treated control IPS mice. The mechanism of this immunomodulatory effect is unknown. Since Th2 responses are dependent on the STAT-6 transcription factor, we studied the effects of KGF in our established murine IPS model using lethally-conditioned STAT-6−/− mice (B6 background) as recipients of allogeneic B10.BR BM and T cells and compared to WT hosts. STAT-6−/− mice pretreated with KGF (5 mg/kg s.c. for 3 days) had an accelerated mortality rate and poorer survival compared to WT recipients. Examination of GVHD target organs revealed that KGF pretreatment ameliorated liver GVHD in WT and STAT-6−/− recipients. In contrast, the benefit of KGF for GVHD of the colon was only seen in the WT but not in STAT-6−/− recipients, consistent with post-BMT body weights and survival. Whole body plethysmograph was done to evaluate lung function at 30 cm water pressure on day 7 post-BMT, a time when acute IPS is manifested in this model. WT but not STAT-6−/− mice pretreated with KGF had improved lung compliance compared to their non-treated counterparts (p<0.05). KGF-pretreated STAT-6−/− recipients also had increased lung weights compared to control- or KGF-treated WT mice with IPS (p<0.05). Histological analysis of lung cryosections taken on day 7 post-BMT showed that all recipients of allogeneic BM and T cells had increased inflammatory cells in their lungs, consistent with acute IPS. However, STAT-6−/− hosts with IPS had elevated numbers of CD4+ T cells in their lungs compared to WT hosts with IPS (18.3%±1.5 vs 11.6%±2.7, p=0.01). Other cell phenotypes did not differ. However, KGF-pretreated STAT-6−/− mice had significantly elevated levels (10-fold) of IL-17, CCL2 and CCL8 in their bronchoalveolar lavage fluid on day 7 post-BMT compared to their WT KGF-treated counterparts implying a skewing to a Th17 inflammatory response in STAT-6−/− mice. These data indicate that the STAT-6 pathway plays a role in the cytoprotective function of KGF and this STAT-6-dependency is tissue-specific. To achieve the full benefits of KGF and other cytoprotective agents, it will be important to take into account the immune modulatory mechanisms operative in individual GVHD target organs.

The measurement of the tensor-to-scalar ratio r shows a very powerful constraint to theoretical inflation models through the detection of B-mode. In this paper, we propose a single inflation model with infinity power series potential called the Infinity Power Series (IPS) inflation model, which is well consistent with latest observations from Planck and BICEP2. Furthermore, it is found that in the IPS model, the absolute value running of the spectral index increases while the tensor-to-scalar ratio becomes large, namely both large r ≈0.20 and [Formula: see text] are realized in the IPS model. In the meanwhile, the number of e-folds is also big enough N≈50–60 to solve the problems in Big Bang cosmology.

Purpose The individual placement and support (IPS) model is an evidence-based approach to employment support for people with severe mental illness that functions by co-locating an employment consultant from the local disability employment service within a community mental health team to assist service users to find work. This paper aims to examine the unintended impacts of implementing IPS on occupational therapy practice and offer some suggestions. Design/methodology/approach The authors performed a narrative literature review on the IPS model, employment and occupational therapy. Authors then analysed and discussed impact on occupational therapy practice and concluded by making suggestions based on current evidence and practice. Findings The authors concluded that implementation of IPS has resulted in some unintended changes of practice in mental health with occupational therapists taking a less active role in enabling employment outcomes than previously. This paper concludes by calling upon occupational therapists to re-establish their role of enabling employment. Originality/value This paper offers an original viewpoint on employment and occupational therapy based on current evidence and authors’ expertise.
