A shipboard fire/explosion may be occurred due to various causal factors such as actions, omissions, events or conditions. In this study, it is aimed to carry out an analysis focused on shipboard fire/explosion casualties. With this aim, the data on 127 shipboard fire/explosion casualties occurred in the Turkish Search and Rescue area, which were reported to the Main Search and Rescue Coordination Center between 2006-2015, have been analyzed. As a result of the statistical analysis (descriptive statistics), it has been observed that majority of the shipboard fire/explosion casualties were occurred on-board the Turkish flagged ships, on-board small passenger vessels/recreational vessels/private-commercial yachts among the classified ship types, in the regions of İstanbul, İzmir and Çanakkale, in the summer season, during the night-time, and in machinery spaces of the ships by described locations. It has been also observed that main events caused shipboard fires/explosion casualties were electricity contact/leakage, gas accumulation/leakage, cargo ignition, welding/hot works and other undescribed factors. Additionally, by examining the existing shipboard fire & explosion accident investigation reports of Transportation Safety Investigation Center between 2014-2020, it has been observed that the main possible causal/contributing factors for the shipboard fire/explosion casualties were related with the violations of the ISM-Safety Management System (SMS) requirements. Many previous studies in the relevant literature point out to the ISM/SMS-related causal/contributing factors as well. In conclusion, special attention should be paid to the effective implementation and continuous improvement of the ISM/SMS procedures related with shipboard fire safety for the prevention of shipboard fire/explosion casualties as well.

Railway transportation safety management must adhere to the approach of safety and prevention first. The transport of dangerous goods should be paid more attention as a particularly important part. This paper made use of fault tree analysis to research and analyze the risk factors of dangerous goods LPG which may explode in the rail transport. The analysis was made from two aspects of chemistry and physics, then the minimal cut sets and structure importance of the specific factors which may cause explosion accidents are solved, and the importance of each factor is analyzed and sorted on the calculated results. Based on the above method, measures of LPG rail transport explosion prevention were proposed.

The article considers the role of integration processes of the quality management system in the field of civil aviation. The process of effective implementation of IMS in the corporate environment is taken into consideration and substantiated. The analysis of ICAO-9859 Document is carried out and the need for the implementation of an integrated corporate management system in organizations operating in civil aviation is substantiated. The advantages of using integrated systems are substantiated. General and specific principles of the Safety Management System (SMS) and Quality Management System (QMS) are highlighted. The created system for the effective implementation of the IMS is considered using a specific example of applying the requirements of the “Safety Management Manual”.

The exploitation of Arctic resources is becoming now one of the most important directions of Russia’s strategic development. The coordination center for this project is St. Petersburg. The article assesses the potential of this region which forms an essential prerequisite for the effective implementation of the given work from the standpoint of the state and prospects of industrial clusters formed in its territory. The subjects of the cluster environment of St. Petersburg relevant programs of state support are divided into three categories: 2 innovative territorial clusters, 3 territorial clusters, 9 territorial clusters administered by the Center for cluster development. Specific recommendations for them are proposed on the basis of analysis of their strengths and weaknesses, as well as assessment of opportunities and threats to their development.

BACKGROUND: Abnormal excessive daytime sleepiness (EDS) has been reported worldwide, but too little is known about EDS and its determinants in Search and Rescue (SAR) populations. We aimed to determine the prevalence of abnormal EDS and contributing factors among Royal Norwegian Air Force (RNoAF) SAR helicopter personnel.METHODS: In this cross-sectional study, a total of N = 175 RNoAF SAR personnel completed an electronic survey including socio-demographic and lifestyle questions. The Epworth Sleepiness Scale (ESS) was used as both a continuous and categorical outcome variable to measure EDS.RESULTS: Abnormal EDS defined by ESS was found in 41% of the participants in this study. We observed no associations between socio-demographic and lifestyle factors and abnormal EDS in this study. DISCUSSION: There is a high prevalence of abnormal EDS in the current RNoAF SAR population. Despite this elevated level of fatigue, we did not find that the socio-demographic and lifestyle factors assessed in this study were associated with abnormal EDS in RNoAF SAR helicopter personnel. Also unusually, the study cohort did not demonstrate higher scores in factors found to change ESS scores in similar study populations (e.g., caffeine use, tobacco use, exercise level). In this paper, we proposed a method of monitoring human driver by reconstructing trajectories which transportation vehicle followed. For safety and management of logistic transportation, it is important to monitor the states of driving behavior through the whole course of path. Since many accidents occur due to the reckless driving of the driver every year, continuous monitoring of the status of commercial vehicles is needed for safety through the entire path from start point to the destination. To monitor the reckless driving, we tried to monitor the trajectory of the vehicle by using vehicle's lateral acceleration signal. Using the correlation between steering angle and lateral acceleration, we could find the relationship between steering angle and acceleration, and finally it is possible to estimate the global direction of vehicle maneuvering. We conducted experiments to find the history of vehicle position on the curved road using Kalman Filter, and classified steering wheel condition (over-steering, and under-steering). The method is applied to central safety management system for safety control of vehicles transporting toxic gases.

Safety of employees is one of the important concerns of the manufacturing organizations and needs to be addressed carefully. It was once, not in ambit of focus but the realization of making safety management as an integral part of the organization is gaining pace. The research article attempts to study the influence of safety perception of the employees on their job satisfaction level in selected small and medium manufacturing enterprises of Uttarakhand in India. A self-developed 29 items scale was used to determine the safety level of employees and job satisfaction inventory by Paul and Spector was used for assessing the job satisfaction level of employees. Linear regression analysis was used to find the degree of influence of safety on job satisfaction level of employees. Stepwise multiple regressions were used to identify the safety factor which contributes the most for employee’s job satisfaction. The research highlights positive influence of safety perception of employees on their job satisfaction level and draws attention towards safety as predictor of job satisfaction. Training and supervision and work procedures were identified as the most contributing factors to enhance job satisfaction of the employees. This study could be helpful for organizations to identify most contributing safety factor which lead to improving the level of job satisfaction among employees.

In Nepal‘s context, concepts like 'development' and 'prosperity' are two age-old (suppressive Rana oligarchy, uni-Panchayat regime and its aftermath) mirages chased by rulers for the ruled ones almost seem to be unattainable. Nevertheless, the mirages of 'development' and 'prosperity' can be materialized when one readily changes his/her antediluvian attitude of fatalism (Bista 1991) and low work ethics. In a recent political paradigm shift from unitary monarchism-oligarchy to multi-party federalism, those two mirages have again been reiterated as Nepalese people's dream and discourse (claimed as Oli-vision) in political arena. In our recent practice, federalism is in its infancy and as a form of government may not matter much first about separating or devolution of political power more effectively, avoiding the power concentration and bringing government responsibilities closer to the citizens. In fact, what acutally matters is its action-oriented effective implementation for separating 'development' from 'fatalism' before it turns frequently into a vicious nightmare. Thus, this paper aims to explore some pertinent aspects or preconditions/hindrances of implementing federalism effictively for achieving the aforesaid goal of 'development' and 'prosperity' within our prevalent general socio-political and bureaucratic trend of 'Balaram syndrome' (a metaphor of ethical degradation of mankind), 'bholi' and 'chiya' syndrome, 'sida-bida' syndrome, nepotism, cronyism, medieval mindset, malpractices of blind capitalism and absence of good governance. In this paper, my main point of departure in methodological tool as such is a closer observation of socio-political trends in Nepalese society and its relevant literature.

As an important raw material of chemical industry, logistics and transportation quantity of sodium cyanide is sharply increased. However, the accidents of sodium cyanide ISO-tank transportation are frequently happened, which caused huge negative impacts on the society and the environment because of its high toxicity. In order to ensure smooth transportation of the hazardous chemicals, we must strive to control the dangerous sources of transportation to eliminate and reduce accidents for the safety and property of the people. On the basis of risk analysis of ISO-tank transport of sodium cyanide, this paper studies the reasons leading to accidents on the point of people, vehicles, ISO-tanks, roads and managements in detail. At last, it focuses on the countermeasures for safety management of sodium cyanide ISO-tank transport on the view of safety assessment in order to provide some guidance for the healthy and sustainable development of this industry. Many publications have reported about 37% of major accidents in petroleum, chemical, nuclear, aviation, and in the other process industries that occurred worldwide were due to human error. One of the keys contributing factors that could prevent these accidents is to provide appropriate training to the plant personnel. However, accidents still can happen if training is poorly managed and improperly trained personnel handling the operation of the plant. One of the established industrial standards to manage the training is Training element of Process Safety Management (PSM) 29 CFR 1910.119(g). This paper presents a system to manage training for safe operation following 29 CFR 1910.119(g) namely Operational Training Management System (OPTRAMS). It provides strategies to manage information and documentation related to training. OPTRAMS was implemented at the CO2-Hydrocarbon Absorption System (CHAS) pilot plant at Universiti Teknologi PETRONAS as a case study. The study showed that all operational training can be managed efficiently with OPTRAMS and also assists end users to identify the gaps that hinder training of PSM compliance. The implementation of this technique could help end users to prevent and minimize catastrophic accidents and comply with training of PSM standard.
