Introdução: Inserido no desafio de possibilitar a interação de profissionais da saúde com prontuários eletrônicos baseados na Norma ISO 13606 permitindo a personalização de interface ao mesmo tempo que se preserva a estrutura e padronização dos Sistemas de Registros Eletrônicos de Saúde (S-RES), o presente recorte objetiva caracterizar propriedades essenciais de S-RES - flexibilidade, padronização e estrutura e facilidade de interação - analisando-se três destes sistemas. Método: Utilizou-se o método de Design Science Research que busca gerar conhecimento teórico a partir da resolução de um problema prático (identificado no contexto da investigação mais ampla). Mais especificamente, no objetivo explicitado neste recorte, efetivou-se uma revisão de literatura sobre os requisitos existentes e dificuldades dos usuários dos sistemas S-RES, seguida de um levantamento e caracterização das propriedades essenciais dos S-RES e do estabelecimento de critérios de análise. O método de inspeção completa a proposta metodológica, visto que as questões (que indicam os critérios) foram apresentadas a dois especialistas para a análise dos Sistemas. Resultados: No recorte proposto, as propriedades levantadas como essenciais dos sistemas foram caracterizadas e analisadas em três S-RES. Confirmou-se a importância das propriedades essenciais destes tipos de sistemas, ainda que verifique a dificuldade de serem atendidas em um mesmo sistema. Conclusão: Os desafios postos aos S-RES não são pontuais ou mesmo de um contexto de uso. Como trabalho futuro propõe-se a criação de um modelo de interface extensível de interface para sistemas de Registro Eletrônico de Saúde baseados na ISO 13606.

The cityEHR Electronic Health Records system is a pure XML application for managing patient health records, using open standards. The structure of the health record follows the definition in the ISO 13606 standard, which is used in cityEHR as a basis for clinicians to develop specific information models for the patient data they gather for clinical and research purposes. In cityEHR these models are represented as OWL/XML ontologies. The most widely adopted approach to modelling patient data in accordance with ISO 13606 is openEHR, which uses its own Archetype Definition Language to specify the information models used in compliant health records systems. This paper describes a translator for the Archetype Definition Language, implemented using XSLT and XML pipeline processing, which generates OWL/XML suitable for use in cityEHR.

Reuse of EHRs requires data extraction and transformation processes are based on homogeneous and formalized operations in order to make them understandable, reproducible and auditable. This work aims to define a common framework of data operations for obtaining EHR-derived datasets for secondary use. Thus, 21 operations were identified from different data-driven projects of a 1,300-beds tertiary Hospital. Then, ISO 13606 standard was used to formalize them. This work is the starting point to homogenize ETL processes for the reuse of EHRs, applicable to any condition and organization. In future studies, defined data operations will be implemented and validated in projects of different purposes.

SummaryBackground: To organize an efficient interaction between a doctor and an EHR the data has to be presented in the most convenient way. Medical data presentation methods and models must be flexible in order to cover the needs of the users with different backgrounds and requirements. Most visualization methods are doctor oriented, however, there are indications that the involvement of patients can optimize healthcare.Objectives: The research aims at specifying the state of the art of medical data visualization. The paper analyzes a number of projects and defines requirements for a generic ISO 13606 based data visualization method. In order to do so it starts with a systematic search for studies on EHR user interfaces.Methods: In order to identify best practices visualization methods were evaluated according to the following criteria: limits of application, customizability, re-usability. The visualization methods were compared by using specified criteria.Results: The review showed that the analyzed projects can contribute knowledge to the development of a generic visualization method. However, none of them proposed a model that meets all the necessary criteria for a re-usable standard based visualization method. The shortcomings were mostly related to the structure of current medical concept specifications.Conclusion: The analysis showed that medical data visualization methods use hard-coded GUI, which gives little flexibility. So medical data visualization has to turn from a hardcoded user interface to generic methods. This requires a great effort because current standards are not suitable for organizing the management of visualization data. This contradiction between a generic method and a flexible and user-friendly data layout has to be overcome.
