The article discusses important features of trends in the transformation of the wholesale and retail level of the Russian medicinal market in the context of innovative factors in the development of the pharmaceutical industry in Russia. The paper gives a general assessment of the impact of state regulation of the maximum selling prices for medicines from the list of vital and essential medicines on the profitability of the market. The author presents a simplified institutional scheme of interaction between distributors and other market participants. The study discusses important aspects and tactical steps of the distribution and pharmacy level of the Russian pharmaceutical market. The article gives the ratings of the leading distribution pharmaceutical companies in Russia and analyses the peculiarities of their structural changes under the influence of innovative development factors. The paper gives ratings of leading pharmacy chains and analyses the features of new structural formations – associations. The author proposes the term of “polarization” for wholesale and retail companies.

One of the established priorities for the development of the retail pharmaceutical market is the digital transformation of classical market mechanisms, in particular, the recent trend is the expanded expansion of ecosystem platforms in retail pharmaceutical activities. The relevance of studies devoted to assessing the applicability of the ecosystem approach for the territorial organizations of productive forces and the market distribution system of the pharmaceutical market is dictated both by the natural processes of the development of the pharmaceutical market and the regulatory specifics of the pharmaceutical sector. The aim of the study was to conceptualize the ecosystem approach to the organization of the pharmaceutical industry at the regional and interregional levels to form the internal potential for the development of the pharmaceutical industry and the distribution network of the pharmaceutical market. The object of the study was a complex of subjects of the pharmaceutical market, for which the architecture of drug distribution was simulated, as well as functional modeling of the processes of pharmaceutical distribution logistics. To assess the integrative and cooperative relationships of the logistics subsystem of a promising territorial pharmaceutical ecosystem, the results of the semantic tree of connections of regional players in the pharmaceutical market were used, as well as the content analysis of shipping documents in the drug supply chains. To assess the applicability of the ecosystem approach in the studied market segment, we have proposed an adaptation of the approach of specialists from Accenture and Kuzmina T.I. and Mandrik N.V., which consists in the industry specialization of the 5p-complex elements for the pharmaceutical market. When testing the proposed methodology, a semantic model of the logistics subsystem of the territorial pharmaceutical ecosystem was obtained, the typology of integrated structures in pharmaceutical logistics of the Ural Federal District (Ural Federal District) was substantiated, 3pl, 4pl, 5pl-operators of the distribution network of the logistics subsystem were determined. The applicability of the ecosystem approach to the modification of the existing structure of logistics channels in the pharmaceutical logistics of the Ural Federal District in the transition from the model of cluster integration in the industrial production of drugs to the ecosystem patient-oriented platform mechanism for the implementation of drug supply has been substantiated.

One of the most important problems in the development of the pharmaceutical market is the problem of medicines’ pricing, which, in addition to the economic aspect associated with fluctuations in the supply and demand curve, is associated with the level of accessibility of medical care to the people. In the unfavorable epidemiological situation, there is a serious problem of unjustified increase of the prices on antiviral drugs. Objective: to study the system of state regulation of pricing for medicines.

The aim: The aim of the study was to study drug consumption in pharmacotherapy of arterial hypertension in a hospital setting. Materials and methods: In the course of work medical cards of patients of the Kyiv regional Cardiac Dispensary in the conditions of inpatient treatment were used. Methods such as: questionnaire, pharmacoeconomic, expert assessments were used. Results: One of the main issues of drug supply for the population, in particular for patients with arterial hypertension, is the study of the demand and consumption of drugs. The drugs used to treat hypertension belong to different pharmacotherapeutic groups and are used in the treatment of a number of other diseases. Recent years of development of the domestic pharmaceutical market are characterized by an increase in the number of these drugs. Conclusions: One of the main issues of drug supply for the population, in particular for patients with arterial hypertension, is the study of the demand and consumption of drugs. As a result of the study, an analysis of the range of drugs for the treatment of hypertension, a comparative study of the market of offers and prices for the treatment of hypertension was carried out.

The study is aimed at determining the characteristics of the activities of representatives of the pharmaceutical industry in the context of compliance with the requirements of international standards in order to provide the population with high-quality, affordable and effective drugs in the context of the COVID-19 coronavirus pandemic. It is substantiated that the marketing activities of pharmaceutical companies and pharmacy chains should comply with the principles of socially responsible marketing and be subject to state regulation in terms of protecting and ensuring public health. The use of individual tools of the marketing complex is not able to solve the problem of reforming relations in the system of providing pharmaceutical care as a whole, since it is necessary to take into account the social orientation of the activities of pharmaceutical companies and the restrictions that exist in the system of informing consumers about the consumer properties of medicines. The theoretical foundations of state regulation of the pharmaceutical market in the direction of creating a legal basis for the functioning of the subjects of the pharmaceutical system by establishing legal and effective relationships between manufacturers, suppliers and consumers of medicines on the principles of implementing socially responsible marketing, compliance with legislative and ethical norms in terms of ensuring the health of citizens of Ukraine have been developed. The social orientation of the activities of representatives of the pharmaceutical sector is focused on providing the population with affordable and effective medicines by improving the pricing system for medicines, producing safe medicines, providing complete and reliable information about the consumer properties of medicines. With the help of generalization and system analysis methods, theoretical and methodological recommendations have been formed for improving the directions of state regulation of the pharmaceutical market in terms of adhering to the principles of socially responsible marketing, the implementation of which will allow government bodies to develop an effective mechanism for state regulation of the pharmaceutical market.

The original version of "the theory of economy management", developed in the 1920s by Russian economists-emigrants who called themselves "Eurasians" (N. Trubetskoy, P. Savitskiy, etc.) is analyzed in the article. They considered this theory to be the basis of the original Russia's way of economic development. The Eurasian theory of economy management focuses on two sides of enterprise activity: managerial as well as social and moral. The Eurasians accepted the Soviet economy with the large share of state regulation as the initial step of development. On the other hand they paid much attention to the private sector activity. Eurasians developed a theoretical model of the mixed economy which can be attributed as the Russian economic school.

Implications of the modern Marxist theory create the opportunity to show the inevitability, the reasons and the main features of the first world crisis of the XXI century. It has been generated by deregulation of economy, which caused the ‘classical’ crisis of overproduction, and by the new contradictions of late capitalism, in particular, by persistent over-accumulation of capital and by the excessive development of the transactional sector, of the fictitious financial capital and its isolation from the real sector. Marxist analysis of social interests and contradictions shows that anti-crisis measures require not only increasing of state regulation, but also determining on behalf of whom and in the interests of what social groups this regulation will be realized. The authors propose to do this on behalf of the financial capital and in the interests of citizens, but also formulate the neoconservative scenario of post-crisis development.
