Fracture Resistance of Endodontically Treated Teeth Restored With Bulk Fill, Bulk Fill Flowable, Fiber-reinforced, and Conventional Resin Composite

SUMMARY The aim of this in vitro study was to evaluate the fracture resistance of endodontically treated teeth restored with different types of restorative resins. Methods and Materials: Seventy-two sound maxillary premolar teeth were randomly divided into six groups (n=12). The teeth in the first group were left intact and tested as unprepared negative control (group I) specimens. The teeth in the remaining five groups were prepared with MOD cavities and endodontically treated. The teeth in one of the five groups (positive control group II) were unrestored. The rest of the prepared cavities were restored as follows: group III: bulk fill resin composite/Filtek Bulk Fill (3M ESPE); group IV: bulk fill flowable resin composite + nanohybrid/SureFil SDR Flow + Ceram.X Mono (Dentsply); group V: fiber-reinforced composite + posterior resin composite/GC everX posterior + G-aenial posterior (GC Corp.); and group VI: nanohybrid resin composite/Tetric N-Ceram (Ivoclar/Vivadent). Each restorative material was used with its respective adhesive system. The restored teeth were stored in distilled water for 24 hours at 37°C and were then thermocycled (5-55°C, 1000×). Specimens were subjected to a compressive load until fracture at a crosshead speed of 0.5 mm/min. The data were analyzed using one-way analysis of variance followed by the post hoc Tukey honestly significantly different test (p<0.05). Results: Sound premolar teeth (group I negative control) showed significantly higher fracture resistance than did the other tested groups (p<0.05). No statistically significant differences were found in the fracture resistance values of the restored groups (groups III, IV, V, and VI) (p>0.05). The lowest values were obtained in the positive control group (group II); these values were significantly lower than those of the other groups (p<0.05). Conclusion: The fracture resistance values of endodontically treated teeth restored with either bulk fill/bulk fill flowable or fiber-reinforced composite were not different from those restored with conventional nanohybrid resin composite.

Objective: Endodontically obturated teeth have lower fracture resistance depending on the obturating material and technique. The purpose of this study was therefore to evaluate the influence of ProRoot MTA (Dentsply Sirona, Tulsa Division) and OrthoMTA III (BioMTA, Daejeon, Korea) as an obturating material on the fracture resistance of endodontically treated teeth. Material and Methods: Thirty extracted human maxillary central incisors were decoronated and instrumented using Protaper instruments (size F5). Irrigation was performed with 2.5% sodium hypochlorite between each instrument change followed by 7% maleic acid for one minute. Finally, canals were flushed with 5 ml of PBS solution for one minute. Samples were then divided into three groups. Group I- positive control (no root canal filling); Group II- obturation with ProRoot MTA; Group III- obturation with OrthoMTA III. Ten teeth were randomly selected as a negative control in which no treatment was performed. All the specimens were then subjected to fracture strength testing using universal testing machine. For evaluation of biomineralization, six maxillary central incisors were divided into two groups. Group I obturated with ProRoot MTA and group II obturated with OrthoMTA III. These samples were subjected to SEM analysis. Results: Positive control group demonstrated the least fracture resistance, while OrthoMTA III group showed the highest fracture resistance. There was no significant difference between negative control group and ProRoot MTA groups (p=0.821). OrthoMTA III group showed better tubular biomineralization when compared to ProRoot MTA. Conclusions: Root canals obturated with OrthoMTA III had better fracture resistance and increased tubular biomineralization compared to ProRoot MTA. Since root canals obturated with OrthoMTA III had better fracture resistance, it can be used as a promising obturating material.KeywordsBiomineralization; Fracture Resistance; OrthoMTA III; ProRoot MTA; Root Canal; Tubular.

Clinical Relevance The fracture resistance of excessively flared endodontically-treated teeth (ETT) without ferrule preparation is not acceptable. Adhesively luted FRC posts with 2 mm ferrule are recommended.

The aim of this study was to compare the vertical fracture resistance of roots obturated with different root canal filling materials and sealers. Crowns of 55 extracted mandibular premolar teeth were removed to provide root lengths of 13 mm. Five roots were saved as negative control group (canals unprepared and unfilled). Fifty root canals were instrumented and then five roots were saved as positive control group (canals prepared but unfilled). The remaining 45 roots were randomly divided into three experimental groups (n=15root/group) and obturated with the following procedures: in group 1, glass ionomer-based sealer and cone (ActiV GP obturation system); in group 2, bioceramic sealer and cone (EndoSequence BC obturation system); and in group 3, roots were filled with bioceramic sealer and cone (Smartpaste bio obturation system). All specimens were tested in a universal testing machine for measuring fracture resistance. For each root, the force at the time of fracture was recorded in Newtons. The statistical analysis was performed by using Kruskal-Wallis and post hoc test. There were no significant differences between the three experimental groups. The fracture values of three experimental and negative control groups were significantly higher than the positive control group. Within the limitations of this study, all materials increased the fracture resistance of instrumented roots.

AbstractFracture resistance of endodontically treated teeth restored with post. Aims: This study aims to compare the fracture resistance of endodontically treated teeth restored with resin fiber and stainless steel post. Commercially available prefabricated resin fiber post(Dentsply Maillefer Easy Post), prefabricated stainless steel post(Coltene/Whaledent Parapost) were used. Methods and Material: Forty five maxillary central incisors were obturated and divided into 3 groups: Control Group (Group I) without any post (n = 15), Resin Fiber Post Group (Group II) (n = 15) and Stainless Steel Post Group (Group III) (n = 15). In all Groups except control group, post space was prepared; a post was cemented, and a core build-up was provided. All the specimens were subjected to compressive force under a universal testing machine until fracture. Statistical analysis used: The results were analyzed using the variable analysis test (ANOVA). Results: One-way analysis of variance revealed significant difference among test groups. The control group demonstrated highest fracture resistance (925.2183 N), followed by the resin fiber post group (486.7265 N) and stainless steel post group (423.539N). Conclusions: Teeth restored with resin fiber post showed higher fracture resistance values than prefabricated stainless steel post.

The restoration of a root canal treated or, endodontically treated teeth, is often can be achieved with post and core because of the significant loss of tooth structure. Until the mid-1980s, the safest way to restore an endodontically treated tooth was considered to be the cast-metal post, made indirectly by a dental technician. Also prefabricated metal posts in combination with different core materials under artificial crowns were used. The development and use of fiber reinforced composite (FRC) root canal posts over metallic posts make possible of the attachment & reinforcement of the crown with maximum esthetics.   The niacin in energy drinks has metabolic product that cause oxidative stress and liver damage, while the liver damage can be prevented by hepatoprotective agents. Scoparone in Artemisia vulgaris L. can act as a hepatoprotector by its antioxidant effect. This study aimed to investigate the effectivity of Artemisia vulgaris L. extract as a hepatoprotector in wistar hepatocytes induced by niacin. This study used 25 male rats which were divided into 5 groups: normal, the negative control, the positive control, the treatment group I, and II. Treatment was conducted for 28 days. The samples were terminated and the hepatocyte were prepared for histological examination. Histological appearance was catagorized as mild, moderate, and severe damage with or without inflamatory cells activity. The data analysis by Kruskal Wallis showed significant difference (p<0,001). Further analysis by Mann Whitney revealed significantly difference (p<0,05) between normal group and all groups, negative control group and positive control group, and positive control group and treatment group I, but not significantly difference between negative control group and treatment group I, negative control group and treatment group II, positive control group and treatment group II, and between treatment groups. The study concluded that the effectivity of Artemisia vulgaris L. extract has not been proven as a hepatoprotector but further study is needed to draw a definite conclusion.   Keywords: energy drink, niacin, Artemisia vulgaris L., hepatoprotector
