INTRODUCTION: Orthodontic patients frequently present composite resin restorations, however there are few studies that evaluate the best way for orthodontic bonding in this situation. OBJECTIVE: The objective of this work was to evaluate the bond strength of orthodontic brackets in resin restorations with surface treatment. METHODS: Fifty one bovine lower incisors were randomly divided into three groups. On the control group (CG) the brackets were bonded to dental enamel; on experimental groups, brackets were bonded to resin restoration with diamond drill treatment (EGT) and with no treatment (EGN). The teeth were placed in PVC tubes with autopolymerized acrylic resin. The shear test was performed in EMIC universal testing machine. The groups were submitted to ANOVA analysis of variance with Tukey post test to verify the statistical difference between groups (α = 0.05). RESULTS: CG (6.62 MPa) and EGT (6.82 MPa) groups presented similar results, while EGN (5.07 MPa) obtained statistically lower results (p < 0.05). CONCLUSION: Therefore, it is concluded that the best technique for bonding of orthodontic brackets on composite resin restorations is the performance of surface detritions.

Objectives: This in-vitro study aimed to evaluate the effect of cavity disinfection with chlorhexidine (CHX) on marginal gaps of Class V composite resin restorations bonded with a universal adhesive using self-etch and etch-and-rinse bonding strategy. Materials and Methods: Sixty sound human premolars were randomly assigned to two groups (n=30): group 1 (CHX) and group 2 (no CHX). Each group was divided into two subgroups (n=15) according to the bonding strategy of the universal adhesive (self-etch or etch-and-rinse). Class V cavities were prepared on the buccal surfaces of the teeth. The occlusal and gingival margins of the cavities were placed in enamel and dentin, respectively. In the first and second subgroups of both groups, the All-Bond Universal adhesive was applied with self-etch and etch-and-rinse bonding strategy, respectively. After restoration and thermocycling, the samples were sectioned, and marginal gaps at the gingival margins were measured in micrometer (µm) under a stereomicroscope. Two-way analysis of variance (ANOVA) was used to compare marginal gaps between the groups and the subgroups. Results: The mean marginal gap size was significantly affected by cavity disinfection (P=0.001) and bonding strategy (P=0.002). However, the interaction effect of these two factors on the mean marginal gap size was not significant (P=0.79). Conclusion: The use of CHX resulted in larger marginal gaps at the gingival margins of Class V composite resin restorations. Irrespective of disinfection, the self-etch bonding strategy resulted in larger marginal gaps compared to the etch-and-rinse bonding strategy.

Clinical Relevance In the current study, contemporary adhesives are assessed mechanically and ultra-morphologically to predict clinical effectiveness. Adhesives with simplified application procedures (in particular, one-step self-etch adhesives) still underperform as compared to conventional “gold-standard” multi-step adhesives. “Mild” two-step self-etch adhesives that provide additional chemical bonding appear to most optimally combine bonding effectiveness with a simplified application protocol.

This article reviews the literature regarding factors that affect proximal contact tightness (PCT) when restoring Class II cavities with direct composite resin. The PCT between two adjacent teeth is an important factor in maintaining oral health by ensuring that tooth positions remain stable, food is deflected away during mastication, dental papillae are maintained and to facilitate hygienic cleaning. If open contacts are created, there is a greater chance that the patient will experience periodontal problems or caries. Using sectional matrix systems with separation rings, and using materials with less polymerization shrinkage are effective methods to increase PCT and create significantly tighter contacts compared with using circumferential matrix systems. CPD/Clinical Relevance: Understating the clinical factors that affect proximal contact tightness between adjacent teeth is important to achieve optimal contacts in direct composite resin restorations to maintain oral health.

Clinical Relevance All Bond 3 and All Bond Self-Etch are versatile systems, capable of being used either in the full or simplified version. Both showed adequate microtensile bond strength results in enamel and dentin after 24 hours of water storage.
