Introduction. Capnography is an essential part and standard monitoring tool during the perioperative period, which can be invaluable in detecting anaesthetic equipment malfunction. Case report. The atypical, ??tails-up?? capnographic waveform was noticed during routine surgical operation. Comprehensive physico-mathematical and graphical explanation of this complex capnographic pattern has been given, together with in-depth analysis of possible differential diagnosis and clinical significance for routine clinical practice. Conclusion. ?Tails-up?? capnographic trace gives early clue to diagnosing and fixing the problem of cracks in sampling line, before leading to an inadequate course of action. The understanding of the physics and physiology behind capnography is of vital importance for the analysis of capnographic waveforms, for early detection of anaesthetic equipment malfunction and for safe clinical practice.

Sarcoidosis is a chameleon disease of unknown etiology, characterized by the growth of non-necrotizing and non-caseating granulomas and manifesting with clinical pictures that vary on the basis of the organs that are mainly affected. Lungs and intrathoracic lymph nodes are the sites that are most often involved, but virtually no organ is spared from this disease. Histopathology is distinctive but not pathognomonic, since the findings can be found also in other granulomatous disorders. The knowledge of these findings is important because it could be helpful to differentiate sarcoidosis from the other granulomatous-related diseases. This review aims at illustrating the main clinical and histopathological findings that could help clinicians in their routine clinical practice.

Methaemoglobin is a form of haemoglobin with oxidised ferric (+3) iron rather than ferrous (+2) iron, which causes a leftward shift in the oxyhaemoglobin dissociation curve and prevents oxygen delivery. Anaesthesiologists need to be familiar with this differential diagnosis for hypoxia given the use of drugs in the perioperative setting known to induce methaemoglobinaemia, including benzocaine and lidocaine, antibiotics such as dapsone and anaesthetic gases, including nitric oxide. This case report details an interesting case of symptomatic methaemoglobinaemia in the perioperative period in the setting of dapsone use and an erector spinae block performed with ropivacaine.

Ayurved is the science of good long health and vitality. All possible vyadhis (disease) and their treatments classified under vata, pitta and kapha (tridosha) types. There have been mentioned 80 types of vata vyadhis (type of diseases) as per classical texts of ayurved. Uruthambha vyadhi (disease) is the type of vata vyadhi explained by acharya charak separately in Charak samhita  other than mentioned 80 types. Urusthambha is caused due to avrodh (obstruction) of vata (type of dosha) by aama (indigested food) and meda dhatu (fat). It obstructs the natural karya (function) of vata in urupradesh (femoral region) due to which the patient has restricted or no movements of thighs. Hence it’s the avran (covering) of aama & meda dhatu to prakruta vata (normal vata)which produces difficulty in walking along with symptom as sthambha (stiffness) in uru Pradesh (thigh region) and hence known as urusthambha. Urustambha cases are very uncommonly seen and diagnosed in routine clinical practice and hence we thought to present the case which diagnosed & successfully treated with only ayurveda. A male patient aged 39 yrs was reported for OPD by us at Shree gajanan ayurvedic chikitsalay and panchakarma centre, panvel, navi mumbai, Maharashtra (Registration no- 2166) with complaints of difficulty in walking with extreme pain and stiffness in uru pradesh (thigh region).We diagnosed and treated for urusthambha with classical treatment mentioned as per charak samhita (ayurvedic text) and was recovered completely after the regular treatment. Patient showed progressive improvement with complete cure in the period of 1 year & 2 months with abhyantara (internal) and bahya chikitsa (external treatment).

Differential diagnosis between pre- and postcapillary pulmonary hypertension (PH) in patients with diastolic heart failure (DHF) is a challenge in clinical practice. The presence of PH is implicated in worse prognosis in patients with this disease. This case report approaches the process of investigation of pulmonary hypertension in adult patient with DHF, double mitral lesion, and sarcoidosis with poor clinical outcome.

Pityriasis rosea (PR) is a dermatological disorder seen commonly in clinical practice. Despite its prevalence and benign nature, there are still times when this common disorder presents in an uncommon way or course posing diagnostic or management problems for the physician. The etiopathogenesis of PR has always been a dilemma, and extensive research is going on to elicit the exact cause. This review and case report focus mainly on the difficult aspects of this benign common disorder such as etiopathogenesis, atypical manifestations, recurrent cases, differential diagnosis and treatment.

Assessment of varicose vein and chronic venous insufficiency and making a differential diagnosis in patients suffering from a variety of sign and symptoms is sometimes a big challenge in daily clinical practice. Here, we present a female patient with varicose vein symptoms and ecchymosis on her lower extremities. Several irregular shaped ecchymotic lesions on both extremities in different stage of healing with deep purple have been treated by using micronized purified flavonoid fraction.

Differential diagnosis of ventricular and supraventricular wide QRS complexes arrhythmias remains one of the most urgent and serious problems of modern cardiology. High-precision intracardiac and transesophageal electrophysiological examination cannot be widely used in routine clinical practice because of the complexity of using, the risk of complications, and the necessary of special equipment. The availability and simplicity of electrocardiogram registration was the reason for study and development of ECG criteria and algorithms for differential diagnosis. However, their low accuracy, variety, complexity and laboriousness of application often leads to diagnostic errors and low usage of clinicians.

Meningiomas are most commonest benign intracranial neoplasm which are encountered in clinical practice. One of the most uncommon variant is intradiploic meningiomas, which can present as osteoblastic or osteolytic calvarial lesion and should be considered  as differential diagnosis. Primary glandular tumors of the urachus and urinary bladder are an intriguing group of clinically and morphologically diverse neoplasms for which there have been recent refinements in diagnostic subclassification and advances in molecular pathology. In addition, the urachus and urinary bladder may be secondarily involved by tumors with glandular differentiation that demonstrate remarkable morphologic, immunophenotypic, and molecular overlap. Thus, surgical pathologists need to be aware of the broad differential diagnosis of glandular tumors that involve the urachus and urinary bladder and have a practical diagnostic framework to evaluate these lesions in routine clinical practice. In this review, we summarize the salient clinical, morphologic, immunohistochemical, and molecular features of glandular tumors of the urachus and urinary bladder, including mucinous cystic tumors of the urachus, noncystic urachal adenocarcinomas, urothelial carcinomas with glandular or pseudoglandular features, primary urinary bladder adenocarcinomas, and Müllerian-type carcinomas, highlighting the strengths and limitations of various diagnostic features and ancillary tests, as well as the need for close clinical and radiographic correlation.
