The most severe spleen lesions with conquasation and devascularisation of entire organ, when it is practically impossible to do any preservating surgical procedure, are the true indications for the transplantation of this extremely important immunological organ. We have performed the evaluation of the surgical procedure of heterotopic auto transplantation in the 30 dogs with severe spleen lesions. Simulation of totally devascularized spleen with the lesions of V degree was performed by disrupting all segmental blood vessels with deep and long longitudinal transhilar incision. During the 3 months follow-up period, animals were subjected to numerous explorations in order to macroscopically and histologically valuate the implant. In most cases (80-85%) implants had complete vitality with the preservation of normal tissue architecture, while 15-20% of implants had partial or total fibrosis. There were no mortality and no complications after this preservation procedure. The presence of fibrosis in some implants suggests that the implant preparation should be better performed and that transplantation of larger tissue volume is needed. Enriched with this experimental experience we have performed heterotopic auto transplantation in 2 patients with spleen lesion of V degree (car accident and injury at work) with very satisfactory results.

Fibrosis is a dynamic process with the potential for reversibility and restoration of near-normal tissue architecture and organ function. Herein, we review mechanisms for resolution of organ fibrosis, in particular that involving the lung, with an emphasis on the critical roles of myofibroblast apoptosis and clearance of deposited matrix.

An algorithm of cancer recognition of ultrasound images is developed in this paper. As we pointed out in our previous work, in order for cancer to survive it develops its own blood supply system, which is different than the supply system of normal tissue. The velocity of the blood flowing through the cancerous blood vessels is different than the velocity of the blood flowing through blood vessels of normal tissue. Due to this fact the ultrasound signal is absorbed differently in the cancerous areas than in the normal tissue areas. The energy of the signal, the continuity of the signal, the autocorrelation function and frequency domain properties are different in the normal tissue than in cancerous tissue. All of these indicators are weighted here for the purpose of classifying the image of the tissue as being cancerous or non cancerous. Preliminary results based on limited number of ultrasound images show that our method has the ability to recognize cancer in ultrasound images.

Introduction Photodynamic therapy (PDT) is a method of treatment of choroidal neovascularization (CNV) with a diode laser used after intravenously administered verteporfin. Verteporfin is a light-activated drug initiating photochemical reactions in the target tissue. This leads to the selective occlusion of blood vessels in the CNV with no damage of photoreceptors, retinal pigment epithelium and retinal blood vessels. Objective To show the results of the treatment of predominantly classic subfoveal CNV with PDT with verteporfin used for the first time in our country. Method From 2003 to 2005, we treated 15 eyes in 15 patients using PDT and verteporfin, because of predominantly classic subfoveal CNV. If macular oedema was present as proved by fluorescein angiography, triamcinolone was administered intravitreally after PDT. Average follow-up period was 7 months (3 months to 2 years). Study design: retrospective, noncomparative, consecutive case series. Results Two thirds of patients had CNV due to AMD, while in others it was caused by pathologic myopia, chorioretinitis, angioid streaks, choroidal hemangioma, except for one patient who had idiopathic CNV. Visual acuity was stabile in 60% (9/15) of patients, of whom in 60% (6/10) of patients with AMD, as well as in patients with pathologic myopia, idiopathic CNV and choroidal hemangioma. Retreatment with PDT was indicated in 40% (6/15) and in 50% (5/10) of patients with AMD, mostly 4-6 months after first PDT, but was done only in one patient (economic reasons). In two patients with AMD, triamcinolone was administered intravitreally for 2-4 months, which resulted in the stabilization of visual acuity. Conclusion Visual acuity was stabile in 60% of all treated patients with predominantly classic subfoveal CNV after only one application of PDT with verteporfin during the average follow-up of 7 months (3 months to 2 years). Retreatment was indicated in 40% of the treated patients, and in 50% of patients with AMD. As confirmed, intravitreal administration of triamcinolone after PDT could stabilize visual acuity. Side effects were not noticed.

<p class="abstract"><span lang="EN-US">Chondroid syringomas are uncommon cutaneous neoplasms of sweat gland origin which are slow-growing, nontender, subcutaneous or intracutaneous in location and often occurring in the head and neck region. Chondroid syringoma should be considered in the differential diagnosis of any subcutaneous nodule over the face. The clinician may miss the diagnosis of this lesion and if it is suspected, tumour should be excised with a margin of normal tissue and regular follow up should be done.</span></p>

Hip’s open reduction combined with Salter innominate osteotomy and femoral osteotomy is the treatment of choice when treating Developmental Dislocation of the Hip (DDH) at walking age. We report a case of a five-year-old girl who underwent a failed surgical procedure of hip’s open reduction, Salter innominate osteotomy, and the femoral osteotomy. One year later, one-stage surgical procedure of hip’s open reduction, Dega pelvic osteotomy, and femoral de-rotation varus shortening osteotomy was performed. During the five-year follow-up, the clinical outcome was evaluated as excellent. The Dega pelvic osteotomy is an effective pelvic osteotomy for DDH, in a case of a failed Salter osteotomy.

A 3 year old child with primary hepatocellular carcinoma and high AFP concentrations is described. Following hemihepatectomy, a sharp decrease and return to normal of serum AFP concentrations indicated the completeness of the surgical procedure. Repeat-normal serum AFP concentrations (less than 19 ng/ml), found during a three year follow-up, correlated well with the absence of clinical, laboratory and x-ray evidence of tumor recurrence. The differential diagnosis of abnormal AFP concentrations in childhood is discussed, and the importance of the AFP assay in the follow-up of post-hemihepatectomy patients for the assessment of the completeness of the surgical procedure, the prognosis, and the early detection of tumor recurrence is stressed.

The tumor microenvironment (TME) is a complex ecosystem, including blood vessels, immune cells, fibroblasts, extracellular matrix, cytokines, hormones, and so on. The TME differs from the normal tissue environment (NTE) in many aspects, such as tissue architecture, chronic inflammation, level of oxygen and pH, nutritional state of the cells, as well as tissue firmness. The NTE can inhibit the growth of cancer at the early tumorigenesis phase, whereas the TME promotes the growth of cancer in general, although it may have some anticancer effects. In particular, the TME plays a crucial role in the generation and maintenance of cancer stem cells, which lie at the root of cancer growth. Therefore, normalization of the TME to the NTE may inhibit cancer growth or improve cancer therapeutic efficiency. This review focuses on the recent emerging approaches for this normalization and the action mechanisms.

Objectives In the application of the Monro-Kellie doctrine in spontaneous intracranial hypotension, the brain tissue volume is generally considered as a fixed constant. Traditionally, cerebral venous dilation is thought to compensate for decreased cerebrospinal fluid. However, whether brain tissue volume is invariable has not yet been explored. The objective of this study is to evaluate whether brain tissue volume is fixed or variable in spontaneous intracranial hypotension patients using automatic quantitative methods. Methods This retrospective and longitudinal study analyzed spontaneous intracranial hypotension patients between 1 January 2007 and 31 July 2015. Voxel-based morphometry was used to examine brain volume changes during and after the resolution of spontaneous intracranial hypotension. Brain structure volume was analyzed using Statistical Parametric Mapping version 12 and FMRIB Software Library v6.0. Post-treatment neuroimages were used as surrogate baseline measures. Results Forty-four patients with spontaneous intracranial hypotension were analyzed (mean [standard deviation] age, 37.8 [8.5] years; 32 female and 12 male). The whole brain tissue volume was decreased during spontaneous intracranial hypotension compared to follow-up (1180.3 [103.5] mL vs. 1190.4 [93.1] mL, difference: −10.1 mL [95% confidence interval: −18.4 to −1.8 mL], p = 0.019). In addition, ventricular cerebrospinal fluid volume was decreased during spontaneous intracranial hypotension compared to follow-up (15.8 [6.1] mL vs. 18.9 [6.9] mL, difference: −3.2 mL [95% confidence interval: −4.5 to −1.8 mL], p < 0.001). Longer anterior epidural cerebrospinal fluid collections, as measured by number of vertebral segments, were associated with greater reduction of ventricular cerebrospinal fluid volume (Pearson’s r = −0.32, p = 0.036). Conclusion The current study found the brain tissue volume and ventricular cerebrospinal fluid are decreased in spontaneous intracranial hypotension patients. The change in ventricular cerebrospinal fluid volume, but not brain tissue volume change, was associated with the severity of spinal cerebrospinal fluid leakage. These results challenge the assumption that brain tissue volume is a fixed constant.
