The metabolic syndrome is characterized by several cardiovascular risk factors and is associated with an increased incidence of diabetes, cardiovascular events and mortality. The prevalence of metabolic syndrome is increasing in epidemic proportions worldwide. The present study aimed to investigate the prevalence of MS and its components in healthy populations in Sana'a, Yemen. This study was a cross-sectional study conducted from February 2019 to April 2019. A total of 120 healthy populations (>=40 years old) were selected. The study protocol was approved by the institutional ethical committee and informed consent was obtained from all the enrolled study patients for their inclusion in the screening and participation in the research. In the present study, the diagnosis of metabolic syndrome based on the American Heart Association/National Heart, Lung, and Blood Institute(AHA/NHL BI) and to a joint statement from several large organizations. In the current study, the presence of more than or equal to any three of the above mentioned factors is required for the diagnosis of metabolic syndrome. The total prevalence of MS among the study subjects was 40.0% (P< 0.001) and 62.5%of them were within 40-49 years old. In the present study, there was not statically significant difference between the khat chewing and the metabolic syndrome. According the distribution of metabolic syndrome criteria among subjects with metabolic syndrome, the prevalence of fasting blood glucose (FBG) was the highest (85%). The prevalence of metabolic syndrome among healthy Yemeni populations was very high and it is associated with increased morbidity and mortality.  This emphasizes the need for more attention to investigate this condition to decreasing the prevalence of cardiovascular morbidity and mortality in these subjects. AbstractThe association between milk consumption and the metabolic syndrome remains inconclusive, and data from Chinese populations are scarce. We conducted a cross-sectional study to investigate the association between milk consumption and the metabolic syndrome and its components among the residents of Suzhou Industrial Park, Suzhou, China. A total of 5149 participants were included in the final analysis. A logistic regression model was applied to estimate the OR and 95 % CI for the prevalence of the metabolic syndrome and its components according to milk consumption. In addition, the results of our study were further meta-analysed with other published observational studies to quantify the association between the highest v. lowest categories of milk consumption and the metabolic syndrome and its components. There was no significant difference in the odds of having the metabolic syndrome between milk consumers and non-milk consumers (OR 0·86, 95 % CI 0·73, 1·01). However, milk consumers had lower odds of having elevated waist circumference (OR 0·78, 95 % CI 0·67, 0·92), elevated TAG (OR 0·83, 95 % CI 0·70, 0·99) and elevated blood pressure (OR 0·85, 95 % CI 0·73, 0·99). When the results were pooled together with other published studies, higher milk consumption was inversely associated with the risk of the metabolic syndrome (relative risk 0·80, 95 % CI 0·72, 0·88) and its components (except elevated fasting blood glucose); however, these results should be treated with caution as high heterogeneity was observed. In summary, the currently available evidence from observational studies suggests that higher milk consumption may be inversely associated with the metabolic syndrome.

ObjectivesThe aim of these analyses was to examine the association of cortisol, DHEAS and the cortisol:DHEAS ratio with the metabolic syndrome (MetS) and its components.DesignThe analyses were cross-sectional.MethodsParticipants were 4255 Vietnam era US army veterans. From military service files, telephone interviews and a medical examination, occupational, socio-demographic and health data were collected. MetS was ascertained from data on body mass index; fasting blood glucose or a diagnosis of diabetes; blood pressure or a diagnosis of hypertension; high-density lipoprotein cholesterol; and triglyceride levels. Contemporary morning fasted cortisol and DHEAS concentrations were determined. The outcomes were MetS and its components. Analysis was by logistic regression, first adjusting for age and then additionally for an array of candidate confounders.ResultsCortisol, although not in the fully adjusted analysis, and DHEAS were both related to MetS. Whereas high cortisol concentrations were associated with an increased risk of MetS, high DHEAS concentrations appeared protective. By far, the strongest associations with MetS were observed for the cortisol:DHEAS ratio; the higher the ratio, the greater the risk of having MetS. The ratio was also significantly related to four of the five MetS components.ConclusionsThe cortisol:DHEAS ratio is positively associated with MetS. Prospective analyses are needed to help untangle direction of causality, but this study suggests that the cortisol:DHEAS ratio is worthy of further study in this and other health contexts.

Metabolic syndrome is a cluster of risk factors for type 2 diabetes and cardiovascular disease, with insulin resistance proposed as a linking factor. It is common and is increasing in prevalence worldwide, largely attributed to increasing obesity and sedentary lifestyles, and now is both a public health and clinical problem. This community-based, cross-sectional descriptive study was conducted during 2015 to identify metabolic risk factors and associated morbidities among adult urban people in Pyin Oo Lwin Township. A total of 355 participants, 94 men (26.5%) and 261 women (73.5%) were enrolled. Age distribution was from 18 to 85 years with mean of 49.98 (SD:15.22) years. Metabolic risk factors were identified according to National Cholesterol Education Programme Adult Treatment Panel III (NCEP ATP III) guideline. In this study, central obesity was the highest component with 145 subjects (40.8%) followed by elevated triglycerides 129(36. 3%), elevated blood pressure 104(29.3%), low HDL-cholesterols 85(23.9%), and elevated fasting glucose in only 48 individuals (13.5%). The total prevalence of metabolic syndrome was 35.2% with 26.6% in men and 38.3% in women, respectively. There were statistically significant associations above all five metabolic risk factors with metabolic syndrome (p value <0.001). Regarding its associated morbidities, ECG examinations showed normal 296 cases (83.4%) and abnormal in 59 cases (16.6%) but there was no significant difference between metabolic syndrome and abnormal ECG findings. In conclusion, this study recognizes the high prevalence rate of metabolic syndrome and can be depicted about the metabolic risks as the baseline data for implementation of further activities to reduce the incidence of non-communicable diseases.

IntroductionMetabolic syndrome is a significant problem in the schizophrenia patients. Previous research demonstrated that single nucleotide polymorphisms in the serotonin 2C receptor (5HTR2C) genes are associated with metabolic syndrome related to schizophrenia patients taking atypical anti-psychotic drugs. This study aimed to investigate whether the effect of 3 SNPs in 5HTR2C gene on the presence of the metabolic syndrome in Thai schizophrenia patients.MethodWe conducted a cross-sectional study and 154 patients were recruited. The schizophrenia patients were identified from a diagnostic and statistical manual of mental disorders, 4th edition, (DSM-IV) and criterion and determined the metabolic syndrome according to the 2005 international diabetes federation (IDF) Asia criteria. Patients were genotyped for the 5HTR2C rs51,8147, rs126,881,02, rs128,367,71 polymorphisms.ResultsThe preliminary analysis from 154 patients showed the metabolic syndrome prevalence was 38.73%, with 46.50% in male and 53.48% in female patients. The results showed that the patients who have heterozygous and homozygous variant on 5HTR2C gene (rs518,147 and rs126,881,02) showed a significant difference in the presence of metabolic syndrome when compare with patients who carry homozygous wild type (P = 0.007), especially in male patients (P = 0.002). The association between 5HTR2C polymorphisms and metabolic syndrome was found in male patients but not found in female patients.ConclusionThese findings suggest that 5HTR2C genotypes are associated with the metabolic syndrome in patients taking atypical anti-psychotics. However, the metabolic syndrome results from the multigenetic effects. Background This study investigated the prevalence of risk factors associated with the metabolic syndrome (MetSyn) among individuals of Japanese descent exposed to different cultural environments. Design A cross-sectional study to assess component risk factors for the diagnosis of MetSyn was undertaken in urban areas in Japan and Brazil. A total of 773 men and women aged 35 years or over were included in three groups: 249 native Japanese, 269 Brazilian individuals of Japanese ancestry residing in Japan, and 255 Brazilian individuals of Japanese ancestry residing in Brazil. Results Higher rates of metabolic abnormalities with respect to central obesity and serum lipid profiles were observed among Brazilian individuals of Japanese ancestry residing in Brazil compared with those residing in Japan and native Japanese. Likewise, an increased risk of hypertension was observed among Japanese Brazilian individuals residing in Japan. The prevalence of MetSyn in men was significantly higher among Brazilians of Japanese ancestry residing in Brazil (37.5%) compared with those residing in Japan (25.3%) or native Japanese (21.4%), whereas no significant difference was observed among women. In the logistic model, Brazilian individuals of Japanese ancestry residinginBrazil weretwice as likely to develop MetSyn compared with native Japanese, whereas no significant differences were found among those residing in Japan. Conclusions These findings underscore the significant heterogeneity in risk factors among communities of Japanese ancestry residing in Brazil and Japan, and suggest that immigrants exposed to the Brazilian cultural environment are more susceptible to the development of risk factors associated with MetSyn than native Japanese.

SummaryPlatelet-derived microparticles (PDMPs), a procoagulant factor, are reportedly elevated in type 2 diabetes mellitus and acute coronary syndrome. The metabolic syndrome (MS) is strongly associated with cardio- and cerebrovascular disease-related atherothrombotic events. To clarify the level, distribution and correlates of PDMPs with special reference to MS, we conducted a cross-sectional study of 467 healthy Japanese volunteers without signs, symptoms, or a history of cardio- or cerebrovascular disease. They were 211 men and 256 women (median age 39 and 35 years, respectively). Using an ELISA kit and monoclonal antibodies against CD42b and CD42a (glycoprotein Ib and IX) we assayed the PDMP levels. Total cholesterol, low-density and high-density lipoprotein cholesterol, remnant cholesterol, triglycerides, C-reactive protein, and traditional cardiovascular risk factors were also recorded. There was a significant difference in the level of PDMPs between men and women. The median value and the interquartile range of PDMPs was 8.3 IU/ml and 6.2 – 10.5 IU/ml and 6.8 IU/ml and 5.2 – 8.6 IU/ml, respectively, in men and women. PDMPs were significantly associated with MS criteria in men (p<0.001) and women (p=0.040). Logistic regression analysis revealed a significant odds ratio of 3.9 [95% confidence interval (CI): 1.4–10.5] in men and of 4.2 [95% CI: 1.6–10.7] in the entire study population. Our results suggest that PDMPs identified by glycoprotein CD42b and CD42a are positively associated with MS.

Objective: This study aimed to compare the prevalence of metabolic syndrome and hyperhomocysteinemia and to specify predictors of the metabolic syndrome among patients with schizophrenia and bipolar disorder. Method: This cross sectional study was conducted on 100 patients with schizophrenia and 100 patients with bipolar disorder. The participants' metabolic syndrome was determined according to the criteria set by Third Report of the National Cholesterol Education Program–Adult Treatment Panel III. Hyperhomocysteinemia was considered as homocysteine levels higher than 15 µmol/L. Chi-square test, Fisher's exact test, student t test, Mann-Whitney test, and logistic regression were used for data analysis. Results: The prevalence of metabolic syndrome was not significantly different (P = 0.07) between patients with schizophrenia (27%) and bipolar disorder (39%). No statistically significant difference (P = 0.17) was observed between patients with schizophrenia (82%) and bipolar disorder (74%) in the prevalence of hyperhomocysteinemia. The results of multivariable logistic regression model showed a significant association of smoking and BMI with metabolic syndrome in patients with schizophrenia (OR = 3.69, 95% CI: 1.13-12.05, and OR = 1.38, 95% CI: 1.20-1.60, respectively). In patients with bipolar disorder, BMI was a significant predictor of developing metabolic syndrome (OR = 1.29, 95% CI: 1.14-1.47). Metabolic syndrome was more prevalent in women than in men in both diagnostic groups (P < 0.05). No significant difference was observed in hyperhomocysteinemia prevalence between male and female patients with schizophrenia (P = 1.00). However, hyperhomocysteinemia was more prevalent in males than in females among patients with bipolar disorder (P = 0.001). Conclusion: Findings showed a high prevalence of metabolic syndrome and hyperhomocysteinemia among patients with schizophrenia and bipolar disorder. To deal with this problem, regular monitoring and conducting early interventions are recommended to determine the metabolic risk profile and to prevent the cardiovascular diseases.

Overweight and obesity are highly prevalent in Kuwait and they are associated with the metabolic syndrome (MS). The present study aims to assess the prevalence of the MS among Kuwaiti female adolescents aged 10–19 years. A cross-sectional random sample of 431, apparently healthy, Kuwaiti female adolescents drawn from several randomly selected schools was studied for the prevalence of the MS using the International Diabetes Federation (IDF) and the National Cholesterol Education Program Third Adult Treatment Panel (ATP III) modified for age diagnostic criteria. Clinical assessment included measurements of waist circumference, blood pressure, fasting blood glucose, HDL and TAG. Whichever criteria are used, the prevalence of the MS among female Kuwaiti adolescents was found to be high, which indicates an urgent need for intervention programmes to prevent increased CVD and type 2 diabetes mellitus. The IDF criteria tend to give higher values for the prevalence of the MS in comparison with the modified ATP III criteria (14·8 v. 9·1 %). There have been no diagnostic criteria specific for the MS for the Gulf Arab population as yet.

Objective: The present study was taken up to assess the prevalence of the metabolic syndrome in schizophrenic patients receiving second-generation antipsychotic agents and to determine the most sensitive and specific clinical parameters used for screening of metabolic syndrome in these patients. Materials and Methods: The present study was taken up in a tertiary care hospital. Eighty patients diagnosed with schizophrenia and receiving a single second-generation antipsychotic for 3 months or more were enrolled in the study after obtaining written informed consent. Patients were screened for metabolic syndrome using American Heart Association and National, Heart, Lung and Blood Institute (AHA NHLBI)-modified National Cholesterol Education Program, Adult Treatment Panel III (NCEP ATP III) criteria. Results: Prevalence of metabolic syndrome was found to be 35%. The clinical parameter with highest sensitivity for screening of metabolic syndrome is low-serum high-density lipoprotein cholesterol with a sensitivity of 89.28%. Elevated fasting blood glucose and increased waist circumference were found to have highest and equal specificity of 90.38%. Conclusion: There is a high prevalence of the metabolic syndrome in patients with schizophrenic patients receiving second-generation antipsychotic agents. Increasing awareness of this fact among psychiatrists will help to prevent, detect, and treat this condition that is associated with considerable morbidity and mortality.
