Shallow groundwater is the main water source among many alternatives in the Kathmandu Valley, Nepal, which has a rapidly growing population and intermittent piped water supply. Although human pathogens are detected in groundwater, its health effects are unclear. We estimated risk of diarrhoea from shallow groundwater use using quantitative microbial risk assessment. Escherichia coli, Giardia cyst and Cryptosporidium oocyst levels were analysed in dug and tube wells samples. E. coli concentrations were converted to those of enteropathogenic E. coli (EPEC). Risks from EPEC in dug wells and from Cryptosporidium and Giardia in both dug and tube wells were higher than the acceptable limit (<10−4 infections/person-year) for both drinking and bathing exposures. Risk from protozoan enteropathogens increased the total risk 10,000 times, indicating that ignoring protozoans could lead to serious risk underestimation. Bathing exposure considerably increased risk, indicating that it is an important pathway. Point-of-use (POU) water treatment decreased the risk six-fold and decreased risk overestimation. Because removal efficiency of POU water treatment has the largest impact on total risk, increasing the coverage and efficiency of POU water treatment could be a practical risk management strategy in the Kathmandu Valley and similar settings.

Cryptosporidium and Giardia (oo)cyst concentrations are frequently used for assessing drinking water safety. The widely used USEPA Method 1623 provides total counts of (oo)cysts, but may not be accurate for human health risk characterization, since it does not provide infectivity information. The total counts and infectious fraction of Cryptosporidium oocysts and the total counts of Giardia cysts were assessed in major fecal pollution sources. Fresh calf and cow feces, their manure, and the discharge point were sampled in a small rural sub-watershed (n = 20, 21, 10, 10). Median concentrations for total (oo)cysts were higher in calves (333 oocysts g−1; 111 cysts g−1) than in cows (52 oocysts g−1; 7 cysts g−1). Infectious oocysts were found in 17 (7%) of the samples and none were found in manure or at the discharge point. Urban sources were sampled in the influent and effluent (n = 19, 18) of two wastewater treatment plants. Peak concentrations were 533 oocysts L−1 and 9,010 cysts L−1 for influents and 89 oocysts L−1 and 472 cysts L−1 for effluents. Infectious oocyst fractions varied from below the detection limit to 7–22% in the effluent and influent respectively. These infectious fractions are significantly lower than those currently used for quantitative microbial risk assessment estimates.

To develop effective groundwater pollution control strategies for the Kathmandu Valley, Nepal, seasonal variations in microbial quality and their underlying mechanisms must be understood. However, to date, there are no studies that address these topics. In this study, groundwater samples from dug wells were collected during the dry and wet seasons from 2009 to 2012, and Escherichia coli (E. coli) and total coliforms were analysed. Three wells were monitored each month for a year. Microbial concentrations in shallow groundwater were significantly higher during the wet season than during the dry season. Analyses of rainfall and E. coli concentrations in different seasons indicated that a high level of faecal material infiltration during the rainy season may have caused the seasonal variations in microbial quality. A moderate to strong relationship between E. coli concentrations and groundwater level suggested that the rise in groundwater levels during the wet season may be another reason for this variation. This long time-scale survey detected a significant decline in the microbial quality of shallow groundwater during the wet season as compared with the dry season. We propose that the infiltration of contaminants and change in groundwater level are the two probable mechanisms for the observed seasonal differences.

Municipal water disinfection systems in some areas are not always able to meet water consumer needs, such as ensuring distributed water quality, because household water management can be a contributing factor in water re-contamination. This fact is related to the storage options that are common in places where water is scarce or is distributed over limited time periods. The aim of this study is to assess the removal capacity of a multiple-barrier water disinfection device for protozoa, bacteria, and viruses. Water samples were taken from households in Mexico City and spiked with a known amount of protozoa (Giardia cyst, Cryptosporidium oocyst), bacteria (Escherichia coli), and viruses (rotavirus, adenovirus, F-specific ribonucleic acid (FRNA) coliphage). Each inoculated sample was processed through a multiple-barrier device. The efficiency of the multiple-barrier device to remove E. coli was close to 100%, and more than 87% of Cryptosporidium oocysts and more than 98% of Giardia cysts were removed. Close to 100% of coliphages were removed, 99.6% of the adenovirus was removed, and the rotavirus was almost totally removed. An effect of site by zone was detected; this observation is important because the water characteristics could indicate the efficiency of the multiple-barrier disinfection device.

A laboratory based microcosm study utilising intact non-sterile sediment cores was undertaken to determine the survival of the faecal indicator organisms Escherichia coli, Enterococcus faecium and somatic coliphage in both recreational coastal water and sediment. Overlying water was inoculated with the test organisms and incubated at 10°C, 20°C or 30°C. E. coli, enterococcus and coliphage were enumerated from the water column and sediment by the membrane filtration method, Enterolert (IDEXX Laboratories) and the double-agar overlay methods respectively on days 0, 1, 2, 7, 14 and 28 following inoculation. It was demonstrated that for all organisms, greater decay (k; d-1) occurred in the water column compared to sediment. Sediment characteristics were found to influence decay, with lowest decay rates observed in sediment consisting of high organic carbon content and small particle size. Decay of E. coli was significantly greater in both the water column and sediment compared with enterococcus and coliphage under all conditions. Decay of enterococcus was found to closely resemble that of coliphage decay. Survival of all organisms was inversely related to temperature, with greatest decay at 30°C. However, increased temperature had a less significant impact on survival of enterococcus and coliphage compared with E. coli. The importance of this study for estimating risk from recreational exposure is great if some pathogenic microorganisms behave similarly to the organisms tested in this study. In particular if survival rates of pathogens are similar to enterococcus and coliphage, then their ability to accumulate in coastal sediment may lead to an increased risk of exposure if these organisms are resuspended into the water column due to natural turbulence or human recreational activity.

Background: The antidepressant mianserin has been shown to extend the lifespan of Caenorhabditis elegans (C. elegans), a well-established model organism used in aging research. The extension of lifespan in C. elegans was shown to be dependent on increased expression of the scaffolding protein (ANK3/unc-44). In contrast, antidepressant use in humans is associated with an increased risk of death. The C. elegans in the laboratory are fed Escherichia coli (E. coli), a diet high in protein and low in carbohydrate, whereas a typical human diet is high in carbohydrates. We hypothesized that dietary carbohydrates might mitigate the lifespan-extension effect of mianserin. Objective: To investigate the effect of glucose added to the diet of C. elegans on the lifespan-extension effect of mianserin. Methods: Wild-type Bristol N2 and ANK3/unc-44 inactivating mutants were cultured on agar plates containing nematode growth medium and fed E. coli. Treatment groups included (C) control, (M50) 50 μM mianserin, (G) 73 mM glucose, and (M50G) 50 μM mianserin and 73 mM glucose. Lifespan was determined by monitoring the worms until they died. Statistical analysis was performed using the Kaplan-Meier version of the log-rank test. Results: Mianserin treatment resulted in a 12% increase in lifespan (P<0.05) of wild-type Bristol N2 worms but reduced lifespan by 6% in ANK3/unc-44 mutants, consistent with previous research. The addition of glucose to the diet reduced the lifespan of both strains of worms and abolished the lifespan-extension by mianserin. Conclusion: The addition of glucose to the diet of C. elegans abolishes the lifespan-extension effects of mianserin.

Abstract There is an urgent need for an effective and long-lasting ceramic filter for point-of-use water treatment. In this study, silver-diatomite nanocomposite ceramic filters were developed by an easy and effective method. The ceramic filters have a three-dimensional interconnected pore structure and porosity of 50.85%. Characterizations of the silver-diatomite nanocomposite ceramic filters were performed using scanning electron microscopy, transmission electron microscopy, X-ray diffraction, and X-ray photoelectron spectroscopy. Silver nanoparticles were confirmed to be formed in situ in the ceramic filter. The highest silver concentration in water was 0.24 μg/L and 2.1 μg/L in short- and long-term experiments, indicating very low silver-release properties of silver-diatomite nanocomposite ceramic filter. The nanocomposite ceramics show strong bactericidal activity. When contact time with Escherichia coli (E. coli) and Staphylococcus aureus (S. aureus) of 105 colony forming units (CFU)/mL exceeded 3 h, the bactericidal rates of the four different silver content ceramics against E. coli and S. aureus were all 100%. Strong bactericidal effect against E. coli with initial concentration of 109 CFU/mL were also observed in ceramic newly obtained and ceramic immersed in water for 270 days, demonstrating its high stability. The silver-diatomite nanocomposite ceramic filters could be a promising candidate for point-of-use water treatment.

Seizures are among the most common symptoms of meningioma. This retrospective study sought to identify risk factors for early and late seizures in meningioma patients and to evaluate a modified STAMPE2 score. In 556 patients who underwent meningioma surgery, we correlated different risk factors with the occurrence of postoperative seizures. A modified STAMPE2 score was applied. Risk factors for preoperative seizures were edema (p = 0.039) and temporal location (p = 0.038). For postoperative seizures preoperative tumor size (p < 0.001), sensomotory deficit (p = 0.004) and sphenoid wing location (p = 0.032) were independent risk factors. In terms of postoperative status epilepticus; sphenoid wing location (p = 0.022), tumor volume (p = 0.045) and preoperative seizures (p < 0.001) were independent risk factors. Postoperative seizures lead to a KPS deterioration and thus an impaired quality of life (p < 0.001). Late seizures occurred in 43% of patients with postoperative seizures. The small sub-cohort of patients (2.7%) with a STAMPE2 score of more than six points had a significantly increased risk for seizures (p < 0.001, total risk 70%). We concluded that besides distinct risk factors, high scores of the modified STAMPE2 score could estimate the risk of postoperative seizures. However, it seems not transferable to our cohort

Abstract Background Despite the recent emergence of plasmid-mediated colistin resistance, the epidemiology and mechanisms of colistin-resistant Enterobacterales (CORE) infections remain poorly understood. Methods A case-case-control study was conducted utilizing routine clinical isolates obtained at a single tertiary health system in Ann Arbor, MI. Patients with CORE isolates from January 1st 2016 to March 31st 2017 were matched 1:1 with patients with colistin-susceptible Enterobacterales (COSE) and uninfected controls. Multivariable logistic regression was used to compare clinical and microbiologic features of patients with CORE and COSE to controls. A subset of available CORE isolates underwent whole genome sequencing to identify putative colistin resistance genes. Results Of 16,373 tested clinical isolates, 166 (0.99%) were colistin-resistant, representing 103 unique patients. Among 103 CORE isolates, 103 COSE isolates, and 102 uninfected controls, antibiotic exposure in the antecedent 90 days and age &gt; 55 years were predictors of both CORE and COSE. Of 33 isolates that underwent WGS, a large variety of mutations associated with colistin resistance were identified, including 4 mcr-1/mcr-1.1 genes and 4 pmrA/B mutations among 9 Escherichia coli isolates; 5 mgrB and 3 PmrA mutations among 8 Klebsiella pneumoniae isolates. Genetic mutations found in Enterobacter species were not associated with known phenotypic colistin resistance. Conclusions Increased age and prior antibiotic receipt were associated with increased risk for patients with CORE, and for patients with COSE. Mcr-1, pmrA/B, and mgrB were the predominant colistin resistance-associated mutations identified among E. coli and K. pneumoniae, respectively. Mechanisms of colistin resistance among Enterobacter species could not be determined.
