<p>The impact of dementia on informal or family caregivers became a public health issue. One well-established tool for the assessment of emotional, physical and social impact on caregivers is the Zarit Burden Interview. Worldwide, it is widely used in epidemiological studies, drug or psychosocial clinical trials, and health services research. The original focus on burden among dementia caregivers has spread to other clinical contexts, mostly in old age and palliative care. Given these diverse applications, issues around the validity and reliability of national translations are crucial to assure that all evidence gathered is indeed of high quality. Moreover, caution is needed on the use of cut-offs for categorizing levels of caregiver strain or of subscales derived from recurrent exploratory factor analyses in small-scale local studies. As with other translations of measures in the health field, researchers and clinicians in Portugal must be aware of how to address bias in using the Zarit Burden Interview and interpreting findings.</p>

Previous health services research has failed to account for the role played by clinical staff in the collection of data. In this paper we use the work of Roth on hired hand research to examine the politics of evidence production within health services research. Sociologies of work predict lack of engagement in the research tasks by subordinated groups of workers. We examine the role of midwives in researching ante-natal screening for sickle cell and thalassaemia in England, and construct three ideal types: repairers, refractors, and resisters to account for the variable engagement of health staff with research. We find some features of the hired hand phenomenon predicted by Roth to be in evidence, and suggest that the context of our project is similar to much health services research. We conclude that without concerted attempts (1) to change the social relations of research production; (2) to mitigate hired hand effects; (3) to assess the impact of the hired hand effect on the validity and reliability of findings, and (4) to report on these limitations, that health services research involving large teams of subordinated clinical staff as data collectors will be prone to produce evidence that is of limited trustworthiness.

IntroductionSuicide is a major public health issue. It is the leading cause of death among younger adults in the UK. Suicide by jumping is an uncommon method. About 23 people die each year by jumping from the cliffs at beachy head, Sussex. The beachy head chaplaincy established a suicide-prevention patrol at beachy head in August 2004. To date there have been no studies evaluating the impact of a suicide patrol as a prevention strategy. This study aimed to assess the impact of this suicide-prevention patrol.MethodsData from local and national official statistics was gathered to examine the overall suicide numbers and rates of suicide by jumping vs. other methods. This included an in-depth scrutiny of coroners’ data and reports from the beachy head chaplaincy.A qualitative, phenomenological approach using in-depth interviews was used to evaluate the “lived experiences” of members of the suicide-prevention patrol.ResultsThe statistics reveal unexpected and at times, conflicting, results which will be offered for discussion.The thematic analysis of the interviews reveals insights into the motivations for volunteering; how a faith-based patrol works; the physical and psycho-social impact of the work; volunteers’ stories; the centrality of God within their work and motivation.For copyright reasons full details of the analyses cannot be made available before the conference.DiscussionWe welcome an interactive discussion of the results.Disclosure of interestThe authors have not supplied their declaration of competing interest.

Tourism plays an important role in the economic and cultural development of society, producing many positive and negative effects. This study aimed to (a) assess the impact of tourism on modern life in Montenegro based on the attitudes of residents and (b) standardize instrument (questionnaire) for assessing these impacts. The sample formed by 521 adults from three different tourist regions (coastal, continental, mountainous). The questionnaire has high validity and reliability. Two components (two independent scales) have been extracted - the negative and positive social impact of tourism. It was found that the increase in positive impacts is followed by an increase in negative effects too. Type of the region, level of municipal development and engagement of respondents in tourism influence significantly the differences between attitudes of examinees. Those who live by tourism most respect its importance for local development.

Purpose – The purpose of this paper is to identify the key determinants of employee engagement and their predictability of the concept. It also studies the impact of employee engagement on employee performance. Design/methodology/approach – Causal study was done to study the impact of relationships. A survey questionnaire was developed and validated using a pilot data (a=0.975). Simple random sampling was used to select the employees from middle and lower managerial levels from small-scale organisations. A total of 700 questionnaires were distributed and 383 valid responses collected. Regression and structural equation modelling were used to predict and estimate the relationships. Findings – It was found that all the identified factors were predictors of employee engagement (r2, 0.672), however, the variables that had major impact were working environment and team and co-worker relationship. Employee engagement had significant impact on employee performance (r2, 0.597). Practical implications – Special focus and effort is required specifically on the factors working-environment and team and co-worker relationship as they have shown significantly higher impact on employee engagement and hence employee performance. Organisations shall focus on presenting a great environment for employees to work and promote programmes that would enhance peer relationships. Social implications – The determinants of employee engagement connote a healthy working atmosphere that reflects on the social impact created by the organisation. Employees would enjoy considerable attention in terms of the determinants being addressed. Originality/value – The research emphasises the growing importance and need for crystallisation of the concept of employee engagement. The research is unique in respect to the comprehensive model that is developed and validated.

The global obesity epidemic, dubbed “globesity” by the World Health Organisation, is a pressing public health issue. The aetiology of obesity is multifactorial incorporating both genetic and environmental factors. Recently, epidemiological studies have observed an association between microbes and obesity. Obesity-promoting microbiome and resultant gut barrier disintegration have been implicated as key factors facilitating metabolic endotoxaemia. This is an influx of bacterial endotoxins into the systemic circulation, believed to underpin obesity pathogenesis. Adipocyte dysfunction and subsequent adipokine secretion characterised by low grade inflammation, were conventionally attributed to persistent hyperlipidaemia. They were thought of as pivotal in perpetuating obesity. It is now debated whether infection and endotoxaemia are also implicated in initiating and perpetuating low grade inflammation. The fact that obesity has a prevalence of over 600 million and serves as a risk factor for chronic diseases including cardiovascular disease and type 2 diabetes mellitus is testament to the importance of exploring the role of microbes in obesity pathobiology. It is on this basis that Massachusetts General Hospital is sponsoring the Faecal Microbiota Transplant for Obesity and Metabolism clinical trial, to study the impact of microbiome composition on weight. The association of microbes with obesity, namely, adenovirus infection and metabolic endotoxaemia, is reviewed.

Objetivo: correlacionar o impacto da sobrecarga do cuidador na qualidade de vida do paciente oncológico em cuidados paliativos Método: trata-se de um estudo quantitativo, descritivo, observacional, transversal, com 50 pacientes oncológicos em cuidados paliativos e 50 cuidadores. Aplicou-se um questionário sociodemográfico para os cuidadores e familiares, a Escala Zarit Burden Interview, o Questionário EORT QLQ –C15-PAL, a Escala de Resiliência Adaptada de Wagnild & Young e a Escala de Pensamento Catastrófico da Dor, uso do Teste t, Correlação de Spearman, Correlação de Pearson e regressão linear. Utilizou-se o Programa SPSS, versão 20.0 para a análise dos dados. Resultados: informa-se que os domínios da qualidade de vida que mostraram associação independente com a sobrecarga do cuidador foram a fatiga, a falta de apetite, a constipação e o impacto global. Manteve-se associação independente pelo catastrofismo da dor do paciente com a sobrecarga do cuidador, não houve associação entre a resiliência do paciente e a sobrecarga do cuidador e 40% dos cuidadores apresentaram sobrecarga severa. Conclusão: nota-se que o aumento da sobrecarga do cuidador diminui a qualidade de vida do paciente oncológico em cuidados paliativos. An independent association was maintained due to the catastrophism of the patient's pain with the burden of the caregiver, there was no association between the patient's resilience and the burden of the caregiver and 40% of the caregivers presented severe burden. Conclusion: it is noted that the increase in caregiver burden reduces the quality of life of cancer patients in palliative care. The theoretical part of this article presents the meaning of sporting events for the tourism industry and the importance of sporting events at a small scale, as well as heritage sporting events for sustainable development of tourist destinations. The literature review shows that there is a clear research gap with respect to the analysis of economic and social impacts of small-scale events. The empirical research was conducted by the method of a diagnostic survey during three sporting events of different rank, which took place in Poland and represented various sports disciplines—running, horse riding, and swimming. A total of 2098 respondents took part in the study, including sports tourists and resident-hosts who took part in the studied events. The aim of the study was to examine whether there is a link between the rank of a sporting event, the development of tourism, and the impact on the host city’s image. The results of the study showed that the highest additional tourism activity in the city on the occasion of participation in an event as an athlete or supporter was undertaken by the participants of the lowest ranked sporting event (Half-Marathon). They also gave the highest rating of the positive impact of a sporting event on the host city’s image. The hosts also rated the highest social impact from the Half-Marathon (i.e., increased sense of pride, social integration, and local identity). Events related to the heritage of the sport of a given region and other smaller-scale sporting events, including mass sports and recreation events (e.g., marathons and other running events), provide an alternative to homogenized, commercial, and very expensive sporting events in the world and may contribute to the sustainable development of tourist regions. The research results show that big social potential lies in the organization of small-scale sports events. The reflections contained in this article may inspire the organization of small-scale sporting events and the continued care for sporting events related to the cultural heritage of a given region. Events of this kind successfully impact tourist revival and improvement of the regions’ image.

The worsening trend of childhood obesity is a global public health issue. Digital technology evolution is a contributing factor towards physical inactivity and obesity among children. In order to overcome this growing problem, exercise games have been introduced in early eighties to ameliorate this obesogenic environment. Exercise games utilize innovative technologies that provide an interactive environment, requiring gestures and body movements to simulate on-screen gameplay. The objective of this review is to assess the effectiveness of exercise gaming intervention in managing childhood obesity. A systematic review was performed. Six articles examining exercise video games impact on weight management among children published between January 2013 and December 2017 in the English language were selected from a total of 54 articles identified through five major search engines. Majority of the research was conducted in developed countries particularly in the United States whereby some developing countries had started with small-scale researches in this area. The result indicated that there was a significant increase in physical activity level, reduction of BMI, reduced adipose tissue composition and a positive psychological impact after the intervention of exercise games. However, the impact of exercise games on physiological and metabolic parameters were inconclusive and requires further study. In conclusion, exercise games are the effective way to combat childhood obesity, along with other modules of lifestyle.

Abstract Introduction: Maternal health is a critical public health issue, and the Coronavirus Disease 2019 (COVID-19) pandemic has had a multifaceted impact on the health and wellbeing of pregnant individuals and mothers. Methods: A scoping review was conducted to compile evidence on direct and indirect impacts of the pandemic on maternal health and provide an overview of the most significant outcomes thus far. Working papers and news articles were considered appropriate evidence along with peer-reviewed publications in order to capture rapidly evolving updates. Narrative descriptions were written about subject areas for which the authors found the most evidence.Results: While the search yielded numerous publications, most studies were methodologically weak. Pregnant individuals were not found to experience more severe symptoms than the general population, and intrauterine, vertical, and breastmilk transmission were unlikely. Labor, delivery, and breastfeeding guidelines for COVID-19 positive patients varied. Severe increases in maternal mental health issues, such as clinically relevant anxiety and depression, were reported. Domestic violence appeared to spike. Prenatal care visits decreased, healthcare infrastructure was strained, and potentially harmful policies implemented with little evidence. Women were more vulnerable to losing their income due to the pandemic than men, and working mothers struggled with increased childcare demands. Conclusion:Pregnant women and mothers were not found to be at higher risk for COVID-19 than the general population, however they seem to face disproportionate adverse socio-economic consequences. High income and low- and middle-income countries alike faced significant struggles. Further resources should be directed towards quality epidemiological studies.
