Background: Suicidal ideation can often lead to suicide attempts and completed suicide. Studies have shown that Sri Lanka has one of the highest rates of suicide in the world but so far no studies have looked at prevalence of suicidal ideation in a general population in Sri Lanka. Aims: We wanted to determine the prevalence of suicidal ideation by randomly selecting six Divisional Secretariats (Dss) out of 17 in one district. This district is known to have higher than national average rates of suicide. Methods: 808 participants were interviewed using Sinhala versions of GHQ-30 and Beck’s Scale for Suicidal Ideation. Of these, 387 (48%) were males, and 421 (52%) were female. Results: On Beck’s Scale for Suicidal Ideation, 29 individuals (4%) had active suicidal ideation and 23 (3%) had passive suicidal ideation. The active suicidal ideators were young, physically ill and had higher levels of helplessness and hopelessness. Conclusions: The prevalence of suicidal ideation in Sri Lanka is lower than reported from the West and yet suicide rates are higher. Further work must explore cultural and religious factors.

The production levels of agriculture have remained low in eastern region of India mainly due to lack of location-specific production technologies, natural calamities like floods, water logging, drought, inadequate timely supply of critical inputs and social constraints. Bihar is one of the important agrarian states of Eastern India. The crop yields are low and almost stagnating in Bihar compared to the north-western and other parts of the country. To improve the productivity in this region mechanization of farms is of critical importance. The farm power availability in Bihar in 2017 is 2.80 kW/ha and is more than the national average of 2.03 kW/ha. Still there are 14 districts in Bihar which is below national average. The number of marginal farmers has increased from 84.18 to 91.21 during 2014-17, an increase of 7.03 percent. Increase in Small and fragmented land will further hindrance the farm mechanization process. The present study was conducted to understand dynamics of farm power availability in Bihar, so as to take substantial measures for improved mechanization and in turn crop productivity in the region.

Abstract Background The national average of identification and susceptibility for organisms isolated from positive blood culture to final susceptibility based on growth on solid media is 48 hours. The goal of this research was to prove that the Vitek®2 (bioMérieux, Inc.) system can provide an accurate and reliable susceptibility result directly from positive blood culture for Gram negative rods and reduce the turnaround time (TAT) from positive blood culture to the final susceptibility. Methods An FDA-modified validation procedure was performed on positive blood cultures directly from the bottle to the VITEK®2 System for susceptibility testing. The protocol tested and validated an aliquot of 50uL of blood directly from the positive bottle into 10 mL of saline (1:200). The solution was vortexed and 3mL were placed in the VITEK®2 test tube. This protocol was intended only for Gram negative rods using the AST-GN70, AST-GN81 & AST-GN801 cards. This protocol followed the CLSI M52 and M100 guidelines. Results 515 organisms from clinical blood culture samples from July 2018 to October 2019 were evaluated. Organisms included, but were not limited to: E. coli, K. pneumoniae, Enterobacter spp., and P. aeruginosa, Proteus spp., Salmonella spp., Acinetobacter spp., and S. maltophilia. There were 5,201 drug/bug combinations. AdventHealth Orlando achieved an essential agreement of 99.32% (n=5,166), minor error 0.74% (n=39) major error 0.02% (n=1) and very major error 0.49% (n=2). A 100% agreement was achieved on detection of ESBL, CRE, and MDR organisms. Conclusion Rapid direct blood culture protocol using the VITEK®2 System and the AST-GN cards is accurate, reliable and can be performed with less than 1 minute hands-on time. The protocol can be implemented in any laboratory at no additional costs or modification where the current VITEK®2 AST-GN panels are in use. This protocol was clinically implemented at AdventHealth Orlando on July 15, 2019. Compared with the national average of 72 hours, the TAT obtained during this study was 23 hours from positive blood culture to final susceptibility, a significant reduction of 25 hours. The authors encourage bioMérieux Inc. to evaluate and explore the opportunity to expand the use of the VITEK®2 system for this application with the appropriate clinical trial. The Impact

The carbon intensity of China’s resource-based cities (RBCs) is much higher than the national average due to their relatively intensive mode of development. Low carbon transformation of RBCs is an important way to achieve the goal of reaching the carbon emissions peak in 2030. Based on the panel data from 116 RBCs in China from 2003 to 2018, this study takes the opening of high-speed railway (HSR) lines as a quasi-experiment, using a time-varying difference-in-difference (DID) model to empirically evaluate the impact of an HSR line on reducing the carbon intensity of RBCs. The results show that the opening of an HSR line can reduce the carbon intensity of RBCs, and this was still true after considering the possibility of problems with endogenous selection bias and after applying the relevant robustness tests. The opening of an HSR line is found to have a significant reducing effect on the carbon intensity of different types of RBC, and the decline in the carbon intensity of coal-based cities is found to be the greatest. Promoting migration of RBCs with HSR lines is found to be an effective intermediary way of reducing their carbon intensity.

The World War II baby boom, coupled with increasing life expectancy, will lead to increasing numbers of deaths for the next 40 years. The last year of life represents a large proportion (55%) of lifetime hospital bed occupancy. This is called the nearness to death effect. However, the nearness to death effect has not been factored into NHS capacity planning, which largely relies on age-based forecasting, often called the ageing population. In certain locations, deaths are predicted to rise far more rapidly than the national average of 1% per annual growth. These locations are highly susceptible to capacity pressures emanating from the nearness to death effect, which is not compatible with recent policies that aim to build smaller hospitals. This article is the first of a two-part series discussing these trends in deaths and bed demand, as well as the likely impact on NHS capacity and the implications for the NHS funding formula.
