Peculiarities of the syndrome of ‘‘dry eye’’ in children with congenital glaucoma

Authors analyze the results of examination of 33 children with congenital glaucoma in the age from 3 till 17 years old. Two groups were sorted out. Children of the first group were treated with 0.5 % timolol maleat, the minimal duration of treatment was 6 months (6 months–7 years). The second group included patients with congenital glaucoma without conservative treatment. The control group included 226 healthy children in the age from 3 till 17 years old without complaints and symptoms of dry eye. The decrease of tear break-up time was revealed in all children with congenital glaucoma. In patients receiving β-blocking treatment the decrease of basal tear secretion was also detected.

Introduction: Dry eye is a common disorder affecting a significant percentage of the population. Materials and methods: This study is a prospective, hospital-based, case-controlled study conducted between January 2009 and May 2010. Seventy-six pterygium cases and 152 age-and- sex matched controls presenting to the OPD of Manipal Teaching Hospital were included. The TBUT Schirmer’s test and basal tear secretion were estimated in all patients. A TBUT of less than 10 seconds and a Schirmer’s test of less than 10 mm were considered abnormal. Results: Pterygium was bilateral in 15 (19.7 % ) and unilateral in 61 (80.3 %) patients. Ninety-two (92.1 %) pterygium patients reported either one or more of the six dry eye symptoms. Redness was the most common (67 %) symptom reported. The mean Schirmer’s test I, mean basal secretion and mean TBUT values were 16.19 mm, 10.01 mm and 10.56 seconds in pterygium cases and 20.22 mm,13.25 mm and 26.25 seconds in the control group respectively. There was a statistically significant difference in the dry eye results between the pterygium cases and the control group (p value < 0.05). The odds ratio between pterygium and dry eye was 3.28. Dry eye was present in 26 % of the normal patients and in 54 % of the pterygium cases. Conclusion: There is a strong relationship between dry eye and pterygium. Aim: To study the role of ‘Xerosis Meter' in screening of dry eye cases in a large population and compare its results with Schirmer's test and tear break-up time. Materials and method: In a cross sectional study with a control group dry eye was evaluated with Xerosis Meter, Schirmer test and tear film break-up time (TBUT). Cases included in the study were divided into two groups. Group I (Control Group) comprised of asymptomatic patients while Group II (Test Group) had patients showing symptoms and signs of dry eye. The Group II was further divided into two subgroups. Group IIa had cases showing positive result with either of the two tests Schirmer test or TBUT. Group IIb had cases which showed positive results with Xerosis Meter but normal results with the other two tests. Statistics: All the observations were statistically analyzed using SPSS version 11.5 software. The results obtained were compared statistically using unpaired t-test and chi-square test. The p value of < 0.05 was considered as significant. Results: There was a statistically significant difference between the means of Xerosis Meter, Schirmer test and TBUT of the two groups: Group I and Group IIa (p value < 0.001). On comparing Group I and Group IIb the difference was found to be statistically significant with Xerosis Meter (p value < 0.001). The sensitivity and specificity of Xerosis Meter were 85.7% and 80.2% respectively. This was higher than that of the Schirmer's test (81.3% and 74.9%) and TBUT (73.2% and 68.7%). Conclusion: The "Xerosis Meter" is an effective alternate in screening of dry eye cases. Purpose. To optimize a rabbit dry eye model induced by topical instillation of benzalkonium chloride (BAC), reduce the days of instillation of the original model by increasing the concentration of BAC from 0.1% to 0.2%. Materials and Methods. An experimental, prospective, and randomized study was performed on 10 male New Zealand white rabbits, divided into two groups, considering both eyes: 5 rabbits as control (n = 10) and 5 rabbits with 0.2% BAC treatment (n = 10). Saline solution (control) and 0.2% BAC were instilled for 5 consecutive days, twice daily. Tear secretion with and without anesthesia, tear breakup time, tear osmolarity, corneal staining, conjunctival hyperemia, density of goblet cells, height of mucin cloud, and transcript levels of IL-6 were measured before and after the treatment. Results. After the instillation of 0.2% BAC for 5 consecutive days, there was a significant increase in tear secretion without anesthesia P<0.001, corneal staining P<0.001, conjunctival hyperemia P<0.001, and levels of IL-6 mRNA P=0.005 compared to the control group. Conversely, there was a decrease in tear secretion with anesthesia P<0.001, tear breakup time P=0.007, tear osmolarity P<0.001, density of goblet cells P<0.001, and height of mucin cloud P<0.001. Conclusions. The topical instillation of 0.2% BAC for 5 consecutive days, twice daily, was a proper procedure to induce a rabbit dry eye model, reducing the number of days of instillation compared to the original model (14 days).

Background: To study the prevalence of dry eye among the patients of pterygium and to find the clinical correlation between dry eye and pterygium.Methods: The study was conducted at postgraduate department of Ophthalmology of GMC Jammu over a period of one year. 90 pterygium patients and 180 age and sex matched controls presenting to the eye OPD of GMC Jammu were included in the study. The TBUT, Schirmer’s test, basal tear secretion and Rose Bengal staining score were estimated in all patients. A TBUT of less than 10 seconds, a Schirmer’s test of less than 10mm, basal tear secretion of less than 10mm, Rose Bengal staining score of more than 3 were considered abnormal.Results: Redness 57 (63.33%) was the most frequently occurring symptom in pterygium patients followed by cosmosis 49 (54.44%). The mean TBUT, mean Schirmer’s, mean basal secretion and mean Rose Bengal staining score values were 9.88±3.39 seconds, 13.17±4.57mm, 10.11±4.81mm and 3.27±1.85 in pterygium patients and 14.22±3.99 seconds, 16.40±5.21mm, 12.19±5.05mm and 2.49±1.86 in the control group respectively. There was a statistically significant difference in the dry eye results between the pterygium patients and control groups (p<0.001). The odd’s ratio between dry eye and pterygium was 3.83, dry eye was present in 53 pterygium patients.Conclusions: The prevalence of dry eye in pterygium patients was 58.89% which suggests that there is a strong positive clinical correlation between dry eye and pterygium.

Objectives Malnutrition and loss of appetite represent a serious problem in children with chronic renal failure. Ghrelin is a newly described hormone involved in control of growth hormone secretion, stimulation of food intake, and regulation of energy balance. Methods Plasma ghrelin levels were compared between 12 children on automated peritoneal dialysis (APD) and 9 children on conservative treatment of chronic renal failure. Eight healthy children matched for age and body mass index (BMI) served as a control group. Results Plasma ghrelin levels were similar in children on APD (698.3 ± 59.7 pg/mL) and children on conservative treatment (675.4 ± 41.9 pg/mL) compared to healthy controls (700.1 ± 24.7 pg/mL). There was no difference in plasma ghrelin levels in children with chronic renal failure regardless of the method of treatment (peritoneal dialysis vs conservative treatment). The plasma ghrelin index was similar in all three investigated groups: APD 40.2 ± 8.7 vs conservative treatment 39.1 ± 5.6 vs controls 41.0 ± 7.8 (pg/mL)/BMI (kg/m2). Plasma ghrelin levels did not correlate with age, duration of dialysis treatment, height, weight, BMI, creatinine and urea levels, adequacy parameters, or nightly glucose load. Conclusion Plasma ghrelin levels in children on APD were not different from levels in children on conservative treatment or healthy controls with comparable BMI. The persistent state of toxic influence of uremic end-products could be responsible for such a lack of correlation with anthropometrical parameters. Further studies on a larger group of children on APD are needed to clarify the effect of ghrelin on nutritional status in children with chronic renal failure.

The objective: evaluating the effectiveness and safety of the drug complex hinekit during preconception preparation for women with complicated pregnancy loss history and uncomplicated mixed genital infections. Patients and methods. The study involved 65 women with reproductive losses in history, who turned over the abnormal vaginal discharge. Diagnosis of sexual transmitted infections (C. trachomatis, T. vaginalis, HSV-II, HPV), assessment of vaginal microbiota was performed by PCR with detection results in real time. Bacterial Vaginosis verified by the Amsel system. Results. All the examinees in the history have miscarriage in the first (56.9%) or the second (43.1%) trimester of pregnancy. According to the results of the patient survey were divided into clinical groups. In 26.2% of women infectious pathology of the lower genital tract has not been confirmed, 26.2% of patients did not give consent to participate in the study and were formed in the control group that received therapy according to existing clinical settings. 31 patients with mixed genital infection (study group) received complex preparation gіnekit. This positive trend was observed in all the patients of the main group (the disappearance of subjective sensations on average 2.2±0.3 hours). After treatment T. vaginalis, BV were not detected in one patient, active mycotic process in the absence of complaints remained at 4.8% of women, complete eradication of C. trachomatis was achieved in 92.3% of patients. Conclusions. Reducing the duration of treatment of mixed genital infections at the preconception in women with reproductive losses in the history is achieved in terms of clinical, microbiological and pharmaco-economic benefits by prescribtion an gіnekit preparation. Summary of clinical and microbiological effects of the proposed approach is 96.0 ± 0.9%, which corresponds to all requirements for multimodal therapy of genital infections, and no side effects, short course of treatment provides a high compliance of patients (9.2±0.7 points). THE AIM. To determine the concentration of natriuretic peptide in the blood serum in children with congenital malformations of the urinary system (CM US) and to compare with the activity of renin-angiotensin-aldosterone system (RAAS).MATERIALS AND METHODS.119 patients with CM US aged 3 to 18 years were examined. A control group of 10 clinically healthy children. 3 groups were assigned: group I – 55 children with  congenital vesicoureteral reflux, and group II – 34 children with  congenital hydronephrosis and ureterohydronephrosis, III group – 30 children with other forms of dysembryogenesis of the US. Following indicators were identified by ELISA in the blood: renin, aldosterone,  N – terminal propeptide natriuretic hormone (NT-рroВNР). RESULTS.NT-рroВNР, renin and aldosterone hyperproduction were diagnosed in 59,6%, 69,7%, 54.6 % of sick children relatively. Concentrations were higher in all variants of  malformations in comparison with the control group. Significant  differences were revealed in obstructive species, where arterial  hypertension (AH) was diagnosed more often. Patients with AH  recorded significantly higher concentrations of NT-proВNР and renin.CONCLUSION.The key point in pathological processes developmentand progression in the cardiovascular system and kidneys is the  activation of RAAS. The system of natriuretic factors is important in maintaining the compensated state of patients due to the blockade of RAAS.

Расширение спектра изучаемых клонов Тхелперов определило более сложные иммунные механизмы реализации аллергического воспаления. Цель. Характеристика показателей и взаимосвязей цитокинового профиля сыворотки и субпопуляционного состава Тлимфоцитов периферической крови у детей с бронхиальной астмой и аллергическим ринитом. Материалы и методы. Проведено комплексное обследование 150 детей в возрасте 311 лет с верифицированным диагнозом бронхиальной астмы, аллергического ринита и 30 здоровых сверстников. Иммунологические параметры крови оценивали методом проточной цитометрии, концентрации интерлейкинов и IgE в сыворотке крови определяли методом твердофазного иммуноферментного анализа. При статистической обработке использовали программы Statistica 10 с критическим уровнем значимости р0,05. Результаты. У детей с аллергическими заболеваниями в сыворотке крови определены высокие уровни содержания интерлейкинов4, 8, 13, 17А, сопоставимый с показателями группы контроля уровень IL17F и низкое содержание IFNy. При бронхиальной астме и аллергическом рините у детей выявлено увеличение количества CD3CD8CD45RO, CD3CD8CD45RACD45RO Тлимфоцитов и CD3CD4 Тхелперов и повышение количество Th17 при снижении CD3CD4CD45RO клеток памяти. В группе здоровых детей популяция Th17 составляла 9,491,6, у детей с аллергическими заболеваниями количество данных клеток было значимо выше 14,50,77 (р0,001). Анализ сывороточного содержания цитокинов у детей с изолированным течением БА и в сочетании с аллергическим ринитом выявил разнонаправленные корреляции, отличающиеся по силе и направленности от таковых в группе здоровых детей. Заключение. У детей при изолированном течении бронхиальной астмы и в сочетании с аллергическим ринитом выявлены: сопоставимое с показателями здоровых детей количество CD3CD4 Тклеток, дисбаланс в субпопуляционном составе Тхелперов за счет преобладания Th2 и Th17, активация синтеза IL17A, IL4, IL8, IL13, низкий уровень сывороточного IFNy, изменения силы и направленности взаимосвязей цитокинового профиля и спектра субпопуляций Тлимфоцитов.Expansion of the range of examined Thelper clones has determined more complex immune mechanisms for the implementation of allergic inflammation. Objective. To characterize the parameters and relationships between the serum cytokine profile and Tlymphocyte subpopulation in peripheral blood of children with bronchial asthma and allergic rhinitis. Materials and methods. 150 children aged between 311 years old with bronchial asthma, and allergic rhinitis and 30 healthy volunteers were examined. Immunological parameters were assessed by flow cytometry, the concentration of serum interleukins and IgE were determined by means of enzymelinked immunosorbent assay. Statistical analysis was performed with Statistica 10 program with a critical level of significance p0.05. Results. High levels of interleukins 4, 8, 13, 17A were determined, IL7F level was not significantly different from that in control group and low level of IFNy was found in the serum of children with allergic diseases. The number of CD3CD8CD45RO, CD3CD8CD45RACD45RO Tlymphocytes, CD3CD4 Thelper cells and Th17 were increased and at the same time CD3CD4CD45RO memory cells were decreased In bronchial asthma and allergic rhinitis children. Number of Th17 cells in healthy children was 9.491.6, in allergic children it was significantly higher 14.50.77 (p0.001). Analyses of serum cytokine count in children with isolated BA and in association with allergic rhinitis revealed multidirectional correlations differing in strength and direction from those in the group of healthy children. Conclusion. In children with isolated bronchial asthma and associated with allergic rhinitis the following parameters were found: CD3CD4 Tcells count was comparable to that in healthy children, the imbalance of Thelper subpopulation: prevalence of Th2 and Th17, activation of IL17A, IL4, IL8, IL13 synthesis and low level of serum IFNy.

Background: A good survival rate among patients with beta thalassemia major (beta-TM) has led to the appearance of an unrecognized renal disease. Therefore, we aimed to assess the role of serum cystatin-C as a promising marker for the detection of renal glomerular dysfunction and N-acetyl beta-D-glucosaminidase (NAG) and kidney injury molecule 1 (KIM-1) as potential markers for the detection of renal tubular injury in beta-TM children. Methods: This case-control study was implemented on 100 beta-TM children receiving regular blood transfusions and undergoing iron chelation therapy and 100 healthy children as a control group. Detailed histories of complete physical and clinical examinations were recorded. All subjected children underwent blood and urinary investigations. Results: There was a significant increase in serum cystatin-C (p < 0.001) and a significant decrease in eGFR in patients with beta-TM compared with controls (p = 0.01). There was a significant increase in urinary NAG, KIM-1, UNAG/Cr, and UKIM-1/Cr (p < 0.001) among thalassemic children, with a significant positive correlation between serum cystatin-C, NAG and KIM-1 as regards serum ferritin, creatinine, and urea among thalassemic patients. A negative correlation between serum cystatin-C and urinary markers with eGFR was noted. Conclusion: Serum cystatin-C is a good marker for detection of glomerular dysfunction. NAG and KIM-1 may have a predictive role in the detection of kidney injury in beta-TM children.

Abstract Objectives There are several hypotheses on the effects of the rs1738074 T/C single nucleotide polymorphism in the TAGAP gene; however, there has been no study on Turkish pediatric patients. We aimed to investigate the association of celiac disease (CD) and type 1 diabetes mellitus (T1DM) comorbidity with the polymorphism in the TAGAP gene of Turkish pediatric patients. Methods Totally, 127 pediatric CD patients and 100 healthy children were included. We determined the polymorphism by the allele-specific polymerase chain reaction method. We used IBM SPSS Statistics version 25.0 and Arlequin 3.5.2 for the statistical analyses. The authors have no conflict of interest. Results It was determined that 72% (n=154) of only CD patients had C allele, whereas 28% (n=60) had T allele. Of the patients with celiac and T1DM, 42.5% (n=17) and 57.5% (n=23) had T and C alleles, respectively. Of the individuals in control group, 67% (n=134) had C allele, whereas 33% (n=66) had T allele. Conclusions There was no significant difference in the genotype and allele frequencies between the patient and control groups (p>0.05). There was no significant association between the disease risk and the polymorphism in our study group.
