Introduction: The prevalence of diabetes is on the increase and an estimated 239 million people worldwide are expected to have the condition by the year 2020 (1). Diabetes mellitus (DM) represents a serious healthcare challenge. The aim of the study was to evaluate the patient clinical characteristics and risk factors for long term complications in the endocrine clinic of Hospital Penang, Malaysia.Methods: Descriptive Prospective cross-sectional study design was chosen. To achieve a power of 0.7 with alpha set at 0.05, 186 subjects were required for the study but researcher increase the sample to 297 in caseof drop out. Self-developed data collection form was used to collect the patient information.Results: 297 (100%) patients were enrolled from OPD diabetic clinic of Hospital Palau Pinang. Among the sample 150 (50.5%) were males and rest 147 (49.5%) females. Malay males mean weight at the time of diagnosis significantly higher (p<0.001,) as compared to other ethnics, same results found among Malay females (p<0.001). Findings suggested increased number of risk factors among the study population. Finding alsoshowed that hypertension found among all the classes of diagnosis. Signifi cant variable are diagnosis class and medication consideration.Conclusion of the study suggested that majority of patients are at high risk of long-term complications and comorbidies. It has been found that increased rate of risk factors have been found among the study population and non-significant to sociodemographic differences.

BACKGROUND: It is widely accepted that hypertension is a pro-inflammatory disease and that the immune system plays a vital role in mediating hypertensive outcomes, end organ damages . and modulation of hypertensive pathology [2]. Patient with MetS usually present increased levels of C-reaktive protein (CRP) wich is a prototypic marker of inflammation [5], however the data of the influence of incresed levels of CRP on the control of the aterial hypertension in patients with MetS, are scarce. OBJECTIVE: We sought to determine the incresed CRP levels influence on the control of the aterial hypertension in patients with MetS. METHODS: We conducted a multicenter observational cross-sectional study. The study population recruited from a coule of outpatient clinic between june 2018 and june 2019. The population study consisted of 420 patients with MetS aged ≥ 18 years, divided in two groups:211 with level of CRP>3mg/l , and 209 participans with CRP level <3mg/l. RESULTS: Among those with CRP level > 3mg/l(N=211) controlled BP according to evidence and current guidelines, was achieved in 23.6% of participans, whereas among those with CRP level <3mg/l(N=209) controlled BP was achived in 48.3%; p=0.000). There was independent association of CRP levels >3mg/l with uncontrolled BP (OR=3.1, 95%CI 2.06 - 4.75). There were signifiacant association of uncontrolled BP with: uncontrolled glycemia (OR =1.4,95%CI 0.97-1.84); increased BMI (OR=4.4; 95%CI 3.02-4.05) and five risk factors for MetS.(OR=2.3, 95%CI 1.93-2.81). CONCLUSIONS: We think,we brought some good evidence,in our present study ,that patients with MetS and higher CRP level have a higher prevalence of unconrolled BP.

To determine the prevalence and risk factors of IBS in children 8–13 years of age in Suzhou city, a cross-sectional study was conducted on children in grades 1 through 6 in public elementary schools in three districts of Suzhou. A multistage stratified random-sampling survey was conducted in a primary investigation using standardized questionnaires. Rome II criteria were used to confirm IBS and their risk factors were analyzed. Of 8,000 questionnaires 7,472 responded satisfactorily for a response rate of 93.4%. IBS was diagnosed in 10.81%. A decrease in the prevalence of IBS was significantly associated with advancing age and grade in school (trend test,P<0.05). The prevalence of IBS in females was higher but not significantly different than males. The significant risk factors for IBS included young age (OR = 0.94), food allergy (OR = 1.53), gastroenteritis during childhood (OR = 1.29), eating fried food (OR = 1.62), anxiety (OR = 1.49), psychological insults in early childhood (OR = 1.47), and parental history of constipation (OR = 1.81; allP<0.05). IBS prevalence of 10.81% in study population warrants preventive measures such as encouraging dietary changes, preventing gastroenteritis and childhood psychological insults.

Background: Hypertension, the “silent killer” - is a modern day’s epidemic and is becoming a public health emergency worldwide, especially in the developing countries. Sedentary life-style and stress are important risk factors for hypertension. The job of bank employees is both sedentary in nature and accompanies high levels of mental stress, thereby at a higher risk of developing hypertension. Hence the present study was conducted to study the prevalence and risk factors of hypertension among the bank employees in Western Maharashtra.Methods: A cross-sectional study was conducted during January 2014 to December 2015 to assess the prevalence and risk factors of hypertension among the bank employees in Western Maharashtra. By using multistage sampling method, total 340 bank employees from 40 branches were included in the study after fulfilling the inclusion and exclusion criteria. After all ethical considerations, the data were collected by using pre-designed, pre-tested and semi-structured questionnaire and then analysed using MS Excel, SPSS-22 and Primer of Biostatistics.Results: Overall prevalence of hypertension was 39.7% and that of pre-hypertension was 41.8% among the study population. Socio-demographic factors like age, male gender, family history of hypertension, mode of travel, physical activity, overweight, years of service, intake of coffee and smoking had shown significant association with hypertension.Conclusions: Prevalence of hypertension was found as significantly higher in bank employees than the general population of India. Among the non-hypertensives large part was pre-hypertensives which are also at high risk of developing hypertension in the future.

Abstract Introduction: Few data of long-term outcomes of cardiovascular (CVRF) risk factors of transgender women (TW) undergoing gender-affirming hormone therapy (GAHT) are available. Objectives: Evaluate CV risk factors after long-term GAHT in TW. Methods: A cross-sectional study was carried out with 8 TW (average age of 34.0 ±4.8 yo), 8 matched CM and 8 CW on age, body mass index (BMI) and activity level. All TW were non-gonadectomized subjects and were in estrogen [(E); transdermal estradiol (n=2), oral estradiol (n=3) and conjugated estrogen (n=3)] plus cyproterone acetate (CA) (n=8) therapy in an average time of 15.6 ±8.7 years. Results: Total Testosterone (ng/dL) level of TW, CW and CM were 83,5 (range 12,0-637,0), 20,5 (range 12,0-41,0) and 480,5 (range 264,0-843,0), respectively. It was similar between TW and CW (p=0,7284) and different in the comparison TWvsCM (p=0,0325). In TW group, the median of blood glucose was 84 mg / dL, HBA1c 5.1%, total cholesterol 146 mg / dL, HDLc 43 mg / dL, LDLc 89 mg / dL and triglycerides 81.5 mg / dL. In the comparison with other groups, there was no difference from the statistical point of view. It is necessary to emphasize the HDLc of TW (43 mg/dL) which was exactly the same of CM (p&gt;0,999) and lower than CW (60 mg/dL)(p=0,0720). Systolic Blood Pressure (SBP)(mmHg) of TW (126±13) was higher than that of CW (95±11;p&lt;0.001) and equal to that of CM (115±9;p=0.1489). Regards Diastolic Blood Pressure (DBP) (mmHg), the medians of TW, CW and CM were 80, 60 and 80, respectively, and in the comparison TWxCW p = 0.0070 and TWxCM p&gt; 0.9999. Discussion: Youth TW (16.3 ± 1.4 yo) taking an average estradiol dose of 1.5 ± 1.0 mg/day, with an average AGHT duration of 12.3 ± 9.9 months matched to controls on age and BMI did have higher HDL than CW and TW participants were more insulin resistant than CM. About SBP of that youth TW (107 ± 12), it was lower than CW 113 ± 7 (p&gt;0,05) and CM 116 ± 8 (p&lt;0,001). Other previous study showed that after 6 months of estradiol use, in doses ranging from 2 to 8 mg daily glucose enhanced 6 mg/dL (from 86 to 92) as well as TC from 170 to 178 mg/dL, HDLc from 50 to 54 mg/dL, TGL from 102 to 115 mg/dL, and LDL did not change (93), while a systematic review and meta-analysis showed increased only in TG levels. SBP and DBP increased on average of 7,2 mmHg and 5,7 mmHg, respectively. Conclusion: Metabolic findings observed after the first few months of TW GAHT appear to remain at long term, except for HDLc. SBP and DBP appear to increase in the long term, after a drop initially observed.

AbstractThe global spread of non-tuberculous mycobacteria (NTM) may be due to HIV/AIDS and other environmental factors. The symptoms of NTM and tuberculosis (TB) disease are indistinguishable, but their treatments are different. Lack of research on the epidemiology of NTM infections has led to underestimation of its prevalence within TB endemic countries. This study was designed to determine the prevalence and clinical characteristics of pulmonary NTM in Bamako. A cross-sectional study which include 439 suspected cases of pulmonary TB. From 2006 to 2013 a total of 332 (76%) were confirmed to have sputum culture positive for mycobacteria. The prevalence of NTM infection was 9.3% of our study population and 12.3% of culture positive patients. The seroprevalence of HIV in NTM group was 17.1%. Patients who weighed <55 kg and had TB symptoms other than cough were also significantly more likely to have disease due to NTM as compared to those with TB disease who were significantly more likely to have cough and weigh more than 55 kg (OR 0.05 (CI 0.02–0.13) and OR 0.32 (CI 0.11–0.93) respectively). NTM disease burden in Bamako was substantial and diagnostic algorithms for pulmonary disease in TB endemic countries should consider the impact of NTM.
