Background: Chronic kidney disease (CKD) is an increasing health concern worldwide. In addition to systemic changes, kidney disease patients also present with oral complications. The objectives of the study were to assess and compare the oral health status in patients with CKD (with diabetes and without diabetes) undergoing dialysis and not undergoing dialysis aged between (30-70) years and controls in Mangalore, South India.Methods: A descriptive cross-sectional study was conducted among 160 adults with various types of kidney disease and controls, attending a tertiary care hospital in Mangalore. Oral health status was assessed using WHO oral health assessment form for adults 2013. Oral hygiene was assessed by using simplified oral hygiene index and gingival status was assessed by using modified gingival index.Results: Number of decayed teeth was more among controls, and number of missing teeth was more among CKD patients. Diabetic CKD patients who were not on dialysis had highest number of teeth with periodontal pocket and more sextants with loss of attachment. Non-diabetic CKD patients who were on dialysis had more gingival bleeding and gingival inflammation. Multinomial logistic regression analysis showed that CKD patients have more odds of having poor oral health than controls.Conclusions: CKD patients had poor oral hygiene, gingival and periodontal status. In contrast, dental caries was significantly lower in CKD patients than the control group. Good oral health condition mitigates the risk of infection, contributing to a better quality of life among CKD patients.

Oral health status is known to be associated with lifestyle-related diseases such as diabetes and chronic kidney disease. In Japan, around 40% of hemodialysis cases are patients with diabetic nephropathy. The aim of this study was to clarify the association between oral health status and diabetic nephropathy-related indices in Japanese middle-aged men. Sixty-six men (age range: 55–64 years) with ≥20 remaining teeth and who received public medical checkups and oral examinations were enrolled. We examined correlations of age, body mass index, HbA1c, HDL-C, LDL-C, neutral fat, serum creatinine, and the estimated glomerular filtration rate (eGFR) with the number of remaining teeth or the community periodontal index (CPI) score (periodontal pocket<4 mm: 0, 4–6 mm: 1, ≥6 mm: 2). A positive correlation between the CPI score and serum creatinine and a negative correlation between CPI score and eGFR (Spearman’s rank correlation coefficient, r=0.459, p<0.01, and r=−0.460, p<0.01, respectively) were observed. The mean eGFR in the CPI score 0 group was significantly higher than that in the CPI score 1/2 group (82.6 vs. 70.7, Student’s t-test, p<0.01). Logistic regression analysis using eGFR as a dependent variable and age, CPI score, body mass index, HbA1c, and neutral fat as independent variables suggested that low eGFR (<60) could be attributed to CPI score (OR=3.169, 95% CI: 1.031–9.742, p=0.044). These results suggest a possible association between periodontal status and renal function in Japanese middle-aged men. Periodontal condition is controlled by oral prophylaxis, and periodontal disease and chronic kidney disease have some common risk factors. Thus, periodontal management can contribute to the prevention of severe chronic kidney disease.

Does non-institutionalized elders have a better oral health status compared to institutionalized ones? A systematic review and meta-analysis

Abstract This systematic review compared the oral health status between institutionalized and non-institutionalized elders. The following electronic databases were searched: PubMed (Medline), Scopus, Web of Science, Lilacs and Cochrane Library, in a comprehensive and unrestricted manner. Electronic searches retrieved 1687 articles, which were analyzed with regards to respective eligibility criteria. After reading titles and abstracts, five studies were included and analyzed with respect their methodological quality. Oral status of institutionalized and non-institutionalized elderly was compared through meta-analysis. Included articles involved a cross-sectional design, which investigated 1936 individuals aged 60 years and over, being 999 Institutionalized and 937 non-institutionalized elders. Studies have investigated the prevalence of edentulous individuals, the dental caries experience and the periodontal status. Meta-analysis revealed that institutionalized elderly have greater prevalence of edentulous (OR = 2.28, 95%CI = 1.68-3.07) and higher number of decayed teeth (MD = 0.88, 95%CI = 0.71-1.05) and missed teeth (MD = 4.58, 95%CI = 1.89-7.27). Poor periodontal status did not differ significantly between groups. Compared to non-institutionalized, institutionalized elders have worse dental caries experience.
