Breast cancer has a high mortality rate and caused about 13.7% of all cancer types deaths in women. Mammography imaging, having a good sensitivity to cancer, is used along with biopsy in a routinely manner, to differentiate between malignant and benign masses. Biopsy is an invasive procedure, and to reduce the necessity for performing it, ultrasound elastography was proposed. Elastography is a potential imaging technique to characterize breast masses, and to differentiate malignant from benign lesions, based on imaging estimated tissue strains under compression. This can result in lowering the number of unnecessary biopsies. Using 3D elastography, lesion relative stiffness with the surrounding soft tissue is estimated at different compression levels, and used as a classification parameter to judge the malignancy of the lesion. In addition, elastography provided a means of emphasizing the strain difference of the lesion from the surrounding soft tissue, which can be used as an additional classification parameter. A pilot study on volunteered patients was performed, and results were compared with biopsy diagnosis as a reference. Initial elastography results showed good agreement with biopsy outcomes. Moreover, we constructed different strain elastograms including first principal, maximum shear and von Mises strains. Those new types of elastographic volumes incorporated the normal axial and shear strains together, which provided better distinction of the hard lesion from the soft tissue. In summary, the proposed elastographic techniques can be used as a noninvasive quantitative characterization tool for breast cancer, with the capability of visualizing and separating the masses in three dimensional space.

The diagnostic and therapeutic approach for pregnant women with thyroid nodules can present a challenge, especially concerning surgical procedures. In the context of malignant diagnosis, by fine needle aspiration (FNA), during pregnancy, the uncertainty lies in performing surgery. This article reports the case of a 41-year-old pregnant woman in her first gestation, who sought medical care complaining of right shoulder pain. Imaging workup depicted the destruction of the humeral head and involvement of the surrounding soft tissue. She was 20 weeks pregnant. The histological report favored the diagnosis of malignancy and the thyroid as the primary site. At 30 weeks of gestation, the patient underwent a cesarean section, a total thyroidectomy, and total resection of the metastasis. The child was born healthy, but one year after the diagnosis, the patient died. Bone and soft tissue metastasis of thyroid neoplasms are not very common and indicate poor prognosis.

BACKGROUND: Late occurrence of solitary soft tissue upper extremity metastasis of breast cancer is very rare. We hereby present a case of metastasis to the biceps muscle of the ipsilateral arm, detected by a physiotherapist six years after mastectomy. The aim of this report is to highlight the rarity of this presentation, to emphasize the role of the physiotherapist as a member of the multidisciplinary treatment team and the possibility of curative treatment despite the poor prognosis. CASE DESCRIPTION: A 2 * 3 cm well-defined isolated metastasis of breast cancer was diagnosed in the left arm of a 31-year-old woman 6 years after successful treatment of her primary tumor. Tumor characteristics, diagnostic plan, and treatment options are discussed. CONCLUSION: Due to its scarcity, there is a lack of knowledge about the frequency, interval, characteristics, best diagnostic modality, adequate treatment, and prognosis of isolated breast cancer metastasis to the soft tissue, and these can be found out by proper reporting. As an important member of the multidisciplinary team in the care and treatment of breast cancer patients, physiotherapists should be aware of this type of rare presentation.

Gastrointestinal stromal tumors (GISTs) are the most common malignant mesenchymal tumors of the gastrointestinal tract. The most common sites of metastasis are the liver and the peritoneum, whereas metastasis to soft tissue is rare. The authors present the case of a 78-year-old male with a soft tissue metastasis of a GIST and the current literature is reviewed.
