Use of Mini Implants for Replacement and Immediate Loading of 2 Single-tooth Restorations: A Clinical Case Report

Abstract Numerous long-term studies have shown that treatment with dental implants can provide edentulous patients with a more stable alternative to complete dentures and partially edentulous patients with a more conservative form of tooth replacement than conventional fixed partial dentures. Until recently, commercially available dental implants have been limited to diameters ranging from 3.0 mm to 7.0 mm. Although this range of diameters has been able to address most clinical needs, partially edentulous patients who could not accommodate a 3.0-mm-diameter implant without damaging adjacent dental structures were excluded from implant therapy. This article reports on the surgical treatment and immediate restoration of a patient who received mini implants that were 2.4 mm in diameter.

Abstract Background An accurate impression is crucial to the long-term success of dental implants. This investigation evaluated the accuracy of the open and closed implant impression techniques in partially edentulous patients who received two adjacent implants. Material and methods Forty patients received Osstem Implants (Osstem Implant System, Seoul, Korea). Two impressions were made for each patient, one using an open tray and a second with a closed tray technique. The horizontal distances between two impression copings were measured and compared to similar measurements on the master casts. Also, under a stereomicroscope (AmScop14370, Myford Road, #150, Irvine, CA 92606 USA) at a 50-fold magnification, the presence or absence of the marginal discrepancies was evaluated. Results There were no statistically significant differences regarding horizontal measurements and in the marginal relationship for the two impression techniques, except between the anterior and posterior regions, for the closed tray technique. There were also no statistically significant differences in the impression accuracy between maxillary and the mandibular arches. In addition, there were no statistically significant differences for the intraoral horizontal distances, compared to similar horizontal measurements on master casts, between the open and closed tray techniques. Conclusions Within the limitations of the present study, there were generally no differences in the impression accuracy between the open and closed tray techniques in partially edentulous patients with two adjacent implants.

ABSTRACTAfter tooth loss, an individual may seek tooth replacement so that his/her function and esthetics could be restored. Clinical prosthodontics, during the past decade, has significantly improved and developed according to the advancements in the science and patient's demands and needs. Conventional options in prosthodontics for substituting a missing single tooth include the removable partial denture, partial and full coverage bridgework, and resin-bonded bridgework. Dental implants have gained increasing popularity over the years as they are capable of restoring the function to near normal in both partial and completely edentulous arches. With substantial evidence available, fixed implant-supported prosthesis are fully acknowledged as a reliable treatment option for the replacement of single or multiple missing teeth nowadays. While dental implants are increasingly becoming the choice of replacement for missing teeth, the impediments associated with them are progressively emerging too.

The study compares subjective experiences of patients, wearing complete dentures. Two different methods of determining a centric relation were performed: the traditional method using wax occlusal rims and the Gerber method, based on gothic arch tracings. The success rate of establishing a centric relation in both methods was evaluated (rentgenodiagnostics). The influence of the method used to obtain the centric relation on patients’ stomatognathic system (condyle centralization, pain) was also evaluated. Better results were achieved in gothic arch tracing method. Before every prosthetic treatment of edentulous patients, a functional analysis of the TMJ is necessary. The lack of centric relation, in a long term adaptation patients, does not lead to TMD symptoms. This trial is registered with NCT03343015.

The aim of the present study was to evidence the improved behavior of our modified PMMA-TiO2 nanocomposite material used to obtain 3D printed complete dentures. A protocol for long-term rapid prototyping complete denture manufacturing using our nanocomposite material, completely characterized and the evaluation of the clinical performance and complications after eighteen months of continuous wearing was also described. Thirty-five fully edentulous patients were enrolled in this prospective clinical study and forty-five complete dentures (31 maxillary and 14 mandibular) have been obtained using an additive manufacturing technology. Due to the good performances of the nanocomposite material, no major functional complications for the obtained medical devices occurred and the clinical outcomes for the 3D printed dentures were favorable evaluated at eighteen months follow up.

This article provides a narrative review of the use of dental implants in patients with periodontitis. Using clinical examples where possible, consideration is given to the survival and success of implants, peri-implantitis, comparison of periodontally compromised teeth to implants and to treatment planning to help achieve favourable outcomes. The challenges associated with restoring an edentulous arch or partially dentate dentition with implants where significant alveolar atrophy has occurred can be considerable. Compromised outcomes may be commonplace. Dental implant treatment is more likely to be successful for those patients who attain and maintain excellent plaque control. Professional support should focus on managing underlying periodontitis prior to commencing implant therapy and providing long term, regular supportive periodontal care upon completion of treatment.

Mini dental implants can be used to support crowns and partial and complete dentures in compromised edentulous sites. Lack of bone width or site length may be treated with mini implants. Mini implants have less percutaneous exposure and displacement that may reduce complications. Nonetheless, mini implants transmit about twice the load to the supporting bone and thus control of occlusal loading is important. In fixed prosthetics, rounded flat cusps, splinting, implant protective occlusal schemes and only placement in dense bone sites are features of successful mini implant treatment. With removable prosthetics, multiple mini implants may be needed for appropriate retention and load resistance. Maxillary lateral incisor and mandibular incisor sites may be best suited for mini implant treatment.Caveat: Past research of dental implants has been directed at standard sized implants. While mini implants are dental implants, indeed, they behave somewhat differently under functional load and the clinician should be circumspect and very judicious in their use.
