Pre-prosthetic Orthodontic Implant for an Aesthetic Rehabilitation : A Case Report

A combination of dental problems such as missing teeth, proclination of teeth and occlusal problems cannot be satisfactorily treated by prosthodontic approach alone. The rehabilitation of patients requiring an esthetic smile demands the collaboration of multiple dental specialists. the need for orthodontic tooth movement prior to restorative treatment is necessary for some patients, in order to optimize both esthetic and functional aspects of dental treatment.The present case report achieved successful implant based oral rehabilitation in a patient with  absence of the right maxillary central incisor utilizing a preprosthetic orthodontic implant site preparation for the purpose of space gain.

Resin bonded fixed dental prosthesis offers most conservative and cost-effective option with missing teeth in aesthetic zone specially when an implant prosthesis is not indicated. This case report describes a treatment option for the replacement of a missing maxillary central incisor using a double-retainer resin-bonded fixed partial denture (RBFPD), fabricated from zirconium dioxide (ZrO) ceramic. No clinical complications were observed at follow-up examination after placement of the prosthesis. Satisfactory functional and aesthetic results were achieved. A treatment modality using a ZrO ceramic RBFPD is an alternative for single anterior tooth replacement.

The aim of this animal study was to develop a model of orthodontic tooth movement using a microimplant as a TSAD in rodents. A finite element model of the TSAD in alveolar bone was built usingμCT images of rat maxilla to determine the von Mises stresses and displacement in the alveolar bone surrounding the TSAD. Forin vivovalidation of the FE model, Sprague-Dawley rats (n=25) were used and a Stryker 1.2 × 3 mm microimplant was inserted in the right maxilla and used to protract the right first permanent molar using a NiTi closed coil spring. Tooth movement measurements were taken at baseline, 4 and 8 weeks. At 8 weeks, animals were euthanized and tissues were analyzed by histology and EPMA. FE modeling showed maximum von Mises stress of 45 Mpa near the apex of TSAD but the average von Mises stress was under 25 Mpa. Appreciable tooth movement of 0.62 ± 0.04 mm at 4 weeks and 1.99 ± 0.14 mm at 8 weeks was obtained. Histological and EPMA results demonstrated no active bone remodeling around the TSAD at 8 weeks depicting good secondary stability. This study provided evidence that protracted tooth movement is achieved in small animals using TSADs.

Abstract Aim The aim of this report is to describe the restorative treatment of an 18-year-old patient diagnosed with autossomal recessive hypocalcified-hypoplastic amelogenesis imperfect (AI). Background Esthetic and functional rehabilitations in AI cases are challenging and should consider individual aspects, such as age, socioeconomic status, AI type, and intraoral condition. Report AI was diagnosed in an 18-year-old patient. Summary Considering the short length of crowns and roots, patient life expectancy, minimal invasiveness, amount of treatment time required, and lower costs, oral rehabilitation with overdentures provided an adequate functional and esthetic rehabilitation of the patient. Clinical Significance This report demonstrated overdentures to be a viable, relatively inexpensive, and noninvasive treatment choice of a patient with AI with concerns about treatment longevity, invasiveness, cost, esthetics, and long-term maintenance. ABSTRACT The loss of tooth substance due to attrition and abrasion may lead to sensitivity to hot and cold, compromised esthetics, loss of anterior guidance and decreased masticatory efficiency. This case report describes the oral rehabilitation of a patient with abrasion, attrition and missing teeth. Treatment consisted of endodontic treatments of multiple teeth followed by their full coverage restorations, and fixed partial dentures for replacement of missing teeth. Esthetic and functional rehabilitation resulted in patient satisfaction, and no complications were noted related to the treatment after 1 year. How to cite this article Kulkarni RS. A 26-year-old man with an aggressive periodontitis sought for orthodontic treatment to improve the appearance of his smile. He presented with generalised anterior spacing, missing lowerleft central incisor and deep traumatic bite. He was treated successfully with a combination of orthodontic and periodontal treatment. After 18 months of orthodontic treatment and follow upby the periodontist, his alignment of teeth was improved, a stable occlusion was achieved, and occlusal trauma was prevented. As a result, the patient’s smile appearance and selfconfidence were improved. Orthodontic tooth movement is not only to correct the alignment of his teeth but also to improve the bone level especially at the anterior region. This case report shows the successful treatment outcome in aggressive periodontitis patient which requires good collaboration between the orthodontist and the periodontist.
