Histologic evaluation of direct pulp capping by the using of calcium hydroxide and octacalcium phosphate in dental pulp of cats

The objective of this study was to measure temperature changes in the pulp chamber induced by polymerization of resin-based dental restoratives following a simulated procedure of direct pulp capping. Class I cavities with a microperforation at the pulp horn were prepared in extracted human molar teeth. The complete procedure of direct pulp capping and cavity restoration was performed with the root part of extracted teeth fixed in a water bath at 37 ?C. Mineral trioxide aggregate, bioactive dentin substitute or calcium-hydroxide paste were used as pulp capping materials. Cavities were restored with a light-cured or chemically-cured resin-modified glass ionomer, universal adhesive and a bulk-fill composite, cured with a high-intensity LED unit. Pulp capping materials caused a slight temperature decrease. Lower temperature increase was recorded during light-curing of the glass ionomer liner after direct capping with mineral trioxide aggregate and calcium-hydroxide than that recorded for the bioactive dentin substitute. Adhesive light-curing increased temperature in all groups with higher mean temperatures in groups with chemically-cured as compared to those for the light-cured glass ionomer liner. Direct pulp capping with mineral trioxide aggregate or calcium-hydroxide followed by the light-cured resin-modified glass ionomer liner and a bonded bulk-fill composite restoration induced temperature changes below the potentially adverse threshold of 42.5?C.

O objetivo deste estudo é avaliar as propriedades físico-químicas e biológicas dos principais materiais indicados para capeamento pulpar direto. Revisou-se a literatura, nos bancos de dados Pubmed, Scopus e Scielo, entre 2007 e 2017, buscando artigos em português e em inglês, utilizando os descritores combinados: “Dental Pulp Capping”, “Biocompatible Materials” e “Calcium Hydroxide” como estratégia de busca. Foram obtidos 55 artigos em que 21 estavam em repetição e foram excluídos, dos demais 6 eram revisão de literatura, 3 eram ensaios clínicos e 25 estudos laboratoriais. Através de uma leitura crítica dos títulos e dos resumos, foram selecionados 10 artigos segundo sua relevância para o estudo. Os materiais mais relatados foram o hidróxido de cálcio, o agregado trióxido mineral (MTA) e o silicato tricálcio (biodentine®). O primeiro contém propriedades antibacterianas e biocompatibilidade, entretanto possui a sua alta solubilidade como desvantagem. O segundo apresenta resultados mais previsíveis que o primeiro na formação da barreira dentinária, melhor biocompatibilidade e boa capacidade seladora. O terceiro mostra maior formação de dentina terciária, promovendo um bom selamento marginal e mostra a menor solubilidade entre eles. Assim, os materiais MTA e biodentine mostram-se materiais viáveis ao tradicional hidróxido de cálcio.Palavras-chave: Dental Pulp Capping. Biocompatible Materials. Calcium Hydroxide.

Melatonin plays an essential role in the regulation of bone growth. The actions that melatonin exerts on odontoblasts may be similar to its action on osteoblasts. This research aimed to evaluate the pulp response to melatonin used for direct pulp capping to evaluate the antioxidant effect of melatonin administered orally and its influence on dental pulp. Direct pulp capping was performed on the upper molars of Sprague Dawley rats using melatonin or Mineral Trioxide Aggregate (MTA). The study groups were: MTA; Melatonin; MTA + Melatonin administered orally; and Melatonin + Melatonin administered orally. In the latter two groups, the animals drank water dosed with melatonin ad libitum (10 mg/100 mL). After 30 days, the animals were sacrificed, and 5 ml of blood, the kidneys, and the liver were extracted in order to evaluate oxidative stress using thiobarbituric acid reactive substances testing (TBARS). Fragments of the maxilla containing the study molars were prepared for histological evaluation. The degree of pulp inflammation and pulp necrosis, the presence of reparative dentin and dentin bridging the pulp chamber, the presence and regularity of the odontoblastic layer, and the presence of pulp fibrosis were evaluated. No significant differences were found between the four study groups for any of the studied histological variables. The oral administration of melatonin did not modify the local effects of MTA or melatonin on dental pulp, or reduce basal-level oxidative stress. The effect of melatonin on pulp is similar to that of MTA and may be used as an agent for direct pulp capping.

This study was performed to examine whether calcium silicate could induce reparative dentin formation without eliciting any adverse effect in direct pulp capping of premolar teeth. Twenty participants who need extraction of their 4 healthy permanent premolar teeth for orthodontic reasons were included in this study. Following the surgical procedure, the exposed pulp tissue was treated either with calcium silicate or covered with calcium hydroxide paste. On day 3, 7, 14 and 28, the experimental teeth was extracted and examined using light microscopy and histometric analysis to observe the inflammatory changes and the amount of reparative dentin formation. The results showed that in the calcium silicate treated teeth, substantial amounts of dentine-like tissue was formed on day 14 and mostly located on the exposure site. It was also observed in the calcium hydroxide treated teeth but dentin-like tissue located at a distance from the exposure site. The total amount of reparative dentine formed in the calcium silicate-treated teeth was significantly higher (p<0.005) than in the calcium hydroxide-treated specimens. In conclusion that the calcium silicate indices pulpal wound healing and reparative formation in the exposed teeth without affecting the normal function of the remaining pulp.
