ATYPICAL HYPOPARATHYROIDISM

ABSTRACT Three patients with atypical, intermittent tetany of recent origin, hypocalcaemia, hypocalciuria and normal serum inorganic phosphorus concentration are described. Two patients were postoperative from thyroidectomy by more than 20 years and one had recently experienced metastatic malignant invasion of the thyroid area. A variety of studies showed that these patients were neither normal nor typically hypoparathyroid in the laboratory. Two hypotheses, each with some support are: 1. that the patients were not totally aparathyreotic, and 2. that, while aparathyreotic, they had adapted to the abnormal state by some mechanism operative in the absence of parathyroid hormone.

Проведено дослідження вмісту 25-гідроксихолекальциферолу (25-ОНD3), паратиреоїдного гормону (ПТГ) та кальцитоніну (КТ), а також кальцію загального, зв'язаного з білком та ультрафільтрувального, неорганічного фосфору, маґнію, активності лужної фосфатази, неетерифікованих жирних кислот (НЕЖК), глюкози і білка у крові корів, хворих на післяродову гіпокальціємію. Встановлено, що у крові корів із клінічними ознаками післяродової гіпокальціємії вміст 25-ОНD3 був вищим, а вміст ПТГ і КТ – нижчим, порівняно зі здоровими коровами в 1–2-й день після отелення. Водночас у крові корів, хворих на післяродову гіпо-кальціємію, знижується вміст загального, білокзв’язаного та ультрафільтрувального кальцію, неорганічного фосфору, глюкози, загального білка й підвищується кількість НЕЖК, маґнію, а також активність лужної фосфатази. It was researched the content of 25-hydroxycholecalciferol (25-OND3), parathyroid hormone (PTH) and calcitonin (CT) and total calcium bounded with the protein and ultrafiltered inorganic phosphorus, magnesium, alkaline phosphatase, nonetherified fatty acids (NEFA), glucose and protein in the blood of cows suffering from postpartum hypocalcemia. It was found that in the blood of cows with clinical signs of postpartum hypocalcemia ,25- OHD3 content was higher, but the content of PTH and CT - lower in coparisant with healthy cows within 1-2 days after calving. However, in the blood of cows suffering from postpartum hypocalcemia was decreasing concentration of total, protein-linked and ultrafiltrates calcium, inorganic phosphorus, glucose, total protein and was increasing concentration of NEFA, magnesium and activity of alkaline phosphatase.

Introduction: Parathyroid cancer is a rare endocrine malignancy. These tumors are typically functional, causing severe hypercalcemia due to primary hyperparathyroidism. Nonfunctional parathyroid cancer with normal serum calcium and parathyroid hormone levels is extremely rare. The disease is usually indolent but progressive with a tendency to metastasize. It is very difficult to diagnose this malignancy. The definitive diagnosis is made by histopathological examination. Radical surgery with ipsilateral lobectomy and en bloc neck dissection is considered to be the most appropriate therapeutic approach. There is no evidence of efficiency of adjuvant cancer therapy and its indication has not been defined. Disease recurrence is common. Case report: We report the case of a 26-year-old female patient who underwent left hemithyroidectomy for growth progression of a hypoechoic lesion behind the left thyroid lobe detected by ultrasonography. Preoperative cytology and imaging assessments were not suspicious for malignancy. Serum parathyroid hormone and calcium levels were normal. The diagnosis of nonfunctional parathyroid carcinoma was determined based on histopathological examination. No further surgery or adjuvant therapy was indicated. No signs of recurrence or generalization have been observed at 36 months after the surgery. Conclusion: Nonfunctional parathyroid cancer is extremely rare. In many cases, the diagnosis is made in advanced stages of the disease. No formal classification or treatment protocol has been established so far. A new staging system has been proposed in the 8th edition of AJCC/UICC. Early detection, radical surgery and close follow-up are crucial aspects to affect the mortality and morbidity of patients with this type of malignancy.

The aim of this work was to clarify levels of serum parathyroid hormone (PTH) in healthy adult women and to study the relationship between PTH and 25-hydroxyvitamin D [25(OH)D]. One hundred sixty-nine healthy women aged 40 years or older in a community were studied. The average age of the subjects was 65.3 years (SD 8.2). All subjects had normal serum creatinine levels. Serum intact PTH and 25(OH)D were measured in these subjects. The mean intact PTH concentration was 2.19 pmol/A (SD 1.02). High intact PTH levels above the reference range were observed in four women (2.4%), all of whom were aged 70 years or older. Intact PTH increased with age with a correlation coefficient of 0.192 (p = 0.013). However, there was no correlation (r = –0.125, p = 0.105) between intact PTH and 25(OH)D whose concentrations were more than 37.5 nmol/l. In conclusion, PTH levels of healthy adult Japanese women are lower than previous reports from Western countries, which may be due to the high 25(OH)D levels of the present subjects and/or an ethnic difference. In addition, there is no association between serum 25(OH)D and PTH levels in this Japanese population, supporting a cutoff level of 25(OH)D less than 37.5 nmol/l for the elevated PTH level.

The Yarlung Zangbo River basin ecosystem is fragile. The distribution and transportation of phosphorus is of great significance for aquatic environmental protection and ecological security. The sequential extraction method and molybdenum antimony anti-spectrophotometry were used to measure the concentrations of different forms of phosphorus in the surface sediments from 15 sampling sites along the middle reaches of the Yarlung Zangbo River and its tributaries. The results show that the total phosphorus concentration in the surface sediments is 194.0~540.7 mg/kg, which is mainly composed of inorganic phosphorus. The concentrations of various phosphorus forms ranked as calcium-bound phosphorus (355.6 ± 86.0 mg/kg) > soluble phosphorus (15.9 ± 10.0 mg/kg) > iron-bound phosphorus (12.4 ± 12.3 mg/kg) > organic phosphorus (9.6 ± 6.1 mg/kg) > occluded phosphorus (9.2 ± 3.8 mg/kg) > aluminum-bound phosphorus (5.4 ± 2.3 mg/kg). On the whole, phosphorus concentration is greater in wet season than dry season. Regarding the spatial distribution characteristics, there are great disparities in the different forms of phosphorus in the middle reaches of the Yarlung Zangbo River. Comprehensive analysis shows that phosphorus of this area is mainly self-generated, and concentration of bioavailable phosphorus is small, demonstrating there will not be a large release. We also drew a “specific triangle” of the different forms of phosphorus concentrations in the research area and defined the “α” angle to determine the nutrient status of the overlying water quickly and effectively. Finally, phosphorus flux of the mainstream was estimated. This research may provide information on the phosphorus of Plateau Rivers.
