Ways of improvement of specialized medical care for acute coronary syndrome in the regional vascular center

The study was carried out to improve results of treatment of patients with acute coronary pathology. The retrospective analysis was implemented concerning functioning of the regional vascular center «The road clinical hospital on the Chelyabinsk railway station» during 2012-2014. The three years experience of rendering of specialized medical care under myocardium infarction within the framework of regional program numbers 1192 cases that amounts to 51.4% of all patients admitted to hospital with acute coronary syndrome. The re-perfusion interventions were applied to 845 patients (70.9%). The emergency transcutaneous coronary interventions were applied to 776 patients (65.1%). The coefficient of involvement of «The road clinical hospital on the Chelyabinsk railway station» in diagnostic and treatment activities among regional vascular centers made up to 19.6 and 26.2% correspondingly. The three-years lethality (2012-2014) made up to 10.8% (2012 - 10,4%, 2013 - 12,5%, 2014 - 10,3%). The best indicators of lethality among patients with reperfusion therapy made up to 7.5% and the worst indicators made up to 18.5% among patients without re-perfusion therapy. The functioning of «The road clinical hospital on the Chelyabinsk railway station» in the field of rendering of specialized medical care to patients with acute coronary pathology is characterized by significant decreasing of lethality from 16% in 2007-2011 when endovascular technologies were not applied up to 10.8% during analyzed period 2012-2014.

A patient with acute coronary syndrome requires not only urgent hospitalization, continuous medical observation, but also medical and diagnostic measures that meet standard standards.Purpose of the study. Аnalysis of the management of patients with acute coronary syndrome with ST segments elevation in the prehospital phase.Materials and methods. Analysis of emergency medical care patients with acute coronary syndrome with ST-segment elevation in 2017. In the study group, men (126 people) and women (119 people).Results. The average time of arrival of the team is 14 minutes, the total time of patient care is 28.9 minutes (excluding time for delivery to the hospital). The time from the call to the moment of delivery to the emergency department is 53 minutes. Analysis of drug therapy revealed that the frequency of nitrates was 88.8 %, antiplatelet therapy – 88.2 %, narcotic analgesics – 82.3 %, anticoagulants – 67.3 %, β-blockers – 18.7 %, oxygen therapy – 36.3 %, antiarrhythmic drugs – 6.4 %, fibrinolytic therapy was performed in 54.3 % of patients.Conclusions. А patient with acute coronary syndrome with ST elevation segments in the prehospital hospital provided sufficient medical care in accordance with clinical guidelines. A low percentage of thrombolytic therapy is associated with the presence of absolute contraindications for the patient.

The aim of our research was to study the structure of patients with acute coronary syndrome and determine the organizational components of medical care for patients with suspected acute coronary syndrome in Poltava region. Material and methods: For our study was on the one hand statistical reports of health care institutions for 2016-2018, and on the other – a survey of doctors and paramedics of emergency medical care teams on tactics management of patients with AMI. The methodological apparatus used to achieve the set goals was based on the determination of relative values and analysis of time series. Results: The number of patients hospitalized with acute coronary syndrome is growing every year: from 82 people in 2012 to 489 in 2018. According to the monitoring conducted in Poltava region, emergency medical care for patients with acute coronary syndrome with ST segment elevation in 2018 in the cities was provided in the amount of 717, while in rural areas – 288. Conclusions: Data from the analysis of emergency medical care for patients with acute coronary syndrome showed that among patients with ST-segment elevation, urban residents predominate; in 90.6% of cases, the time to the patient’s hospitalization was less than 120 minutes.

The aimwas to study early outcomes in patients with suspected acute coronary syndrome without ST elevation who were transported by emergency to the vascular center.Material and Methods. We studied medical records from 396 patients with suspected acute coronary syndrome without ST elevation. A telephone survey of patients or their relatives was conducted within two months after the emergency call.Results. In-hospital diagnosis of acute coronary syndrome was confirmed only in 30.6% of patients with suspected acute coronary syndrome without ST elevation admitted to the vascular center. Cardiologists in the vascular center were ruling out diagnosis of acute coronary syndrome without ST elevation based on data of clinical examination and electrocardiography. During the following two months, 6.4% of the patients with ruled out diagnosis of acute coronary syndrome called emergency again; 2% of the patients were admitted with acute coronary syndrome to the vascular center; and 2.4% of the patients died at home.Conclusion. In real clinical practice, the assessment of myocardial necrosis biomarkers has been used not enough in cases of suspected acute coronary syndrome without ST elevation.
