Main Challenge

The risks of systemic anti-coagulation or its reversal are well known but accepted as necessary under certain circumstances. However, particularly in the plastic surgical patient, systemic alteration to hemostasis is often unnecessary when local therapy could provide the needed adjustments. The aim of this review was to provide a summarized overview of the clinical applications of topical anti- and pro-coagulant therapy in plastic and reconstructive surgery. While not a robust field as of yet, local tranexamic acid (TXA) has shown promise in achieving hemostasis under various circumstances, hemostats are widely used to halt bleeding, and local anticoagulants such as heparin can improve flap survival. The main challenge to the advancement of local therapy is drug delivery. However, with increasingly promising innovations underway, the field will hopefully expand to the betterment of patient care.

Purpose: The objective of this paper is to review concepts of this type of occlusion and provide clinical guidelines based on the currently available literature. Materials and Methods: A literature review was conducted in MEDLINE and PubMed since September 2015 to January 2016. Results: The database searches resulted in 1481potentially relevant articles. After inclusion/exclusion criteria 41 articles were considered. Conclusions: Based on the literature reviewed it can be concluded that the objective of implant-protected occlusion is to minimize occlusal overload on the implant / bone interface and on prostheses keeping the masticatory loads within physiological limits, providing stability and longevity to rehabilitation treatments with implants.

Although a recognized condition, growing teratoma syndrome (GTS) has no guidelines for management, and patients diagnosed with the condition are managed empirically by the most appropriate teams. We report a case of GTS in a 33-year-old patient who was initially treated with unilateral salpingo-oophorectomy and subsequent chemotherapy for a germ cell ovarian tumor. GTS was subsequently diagnosed with massive pelvic and upper abdominal masses as well as lung tumors. We also conducted a literature review on cases of GTS presenting with large tumors. Based on this, we suggest a management plan to guide the care of women with GTS. The condition is best managed in a multidisciplinary team involving the relevant surgeons, including gynecologist, abdominal and thoracic surgeons.
