The considerable prevalence of dental diseases in the patients with hemophilia is due to the absence of prevention and the proper care of the oral cavity, as well as regular medical examination. This category of patients reluctantly turns to the dentists for fear of bleeding, and the doctors for the same reason, avoid interventions in the mouth. Dental aspects of hemophilia are relevant problem of modern medicine due to the high intensity of organs and tissues of the oral cavity, a real risk of complications during dental procedures of therapeutic, surgical and orthopedic profiles. Epidemiological studies on lesions of the oral cavity in the patients with hereditary coagulative pathologies show a high prevalence of various dental disease, poor hygienic condition of the mouth, which entails a considerable need for dental treatment and prevention activities. To ensure timely quality dental care, control over the hygienic condition of oral cavity it is necessary to conduct regular examination of the state of the oral cavity in the patients with hemophilia. Dentists refuse to the patients in need of assistance because of fear of bleeding and the risk of infection with hepatitis and HIV infection. The high level of dental diseases at hemophilia may be due to the poor hygienic condition of the mouth, change of physicochemical properties of saliva and local immunity. Most patients with hemophilia brush their teeth not regularly and unsatisfactory. A vast number of microorganisms contained in this RAID, low local immunity can cause various diseases of the oral cavity. In this group of patients it is more important to prevent the development of dental disease than to treat them, for fear of complications.

According to the European Dialysis and Transplantation Association (EDTA), the incidence of chronic kidney disease is 4–6 cases per million children under the age of 15 and it increases with the age of the child. In order to optimize dental care for children with chronic kidney disease (CKD), an algorithm for dental treatment and prevention was developed. A questionnaire was conducted among 57 children with CKD and 57 parents of this group of children to assess parental and child knowledge of the basics of preventive measures for kidney diseases. Based on the results of the questionnaire, nutritional recommendations were developed. It was found that 100% of parents know the basic nutritional guidelines for children with CKD. Particular attention was paid to children’s consumption of food containing salt. All parents surveyed (100%) were convinced that their children do not consume salty foods, while 23.1% of children claimed to consume them 3 times a day. Thus, it is advisable to implement the developed complex method of treatment and prevention of dental diseases with the inclusion of sanitary and hygienic education on the issues of nutrition for patients with CKD in order to further prevent not only recurrence, but also the progression of existing pathology. Subject. At the present stage of society development cancer incidence remains among the most pressing medical and social problems. In the structure of oncopathology a special place is occupied by malignant neoplasms of lymphoid and hematopoietic tissues which include chronic myeloid leukemia (CML). In some patients, the disease can appear with dental symptoms associated with a decrease in local immunity of the oral cavity — the appearance of aphthae, plaque on the oral mucosa, angular cheilosis, bleeding gums, dry mouth. It is diagnostically significant that such oral symptoms may precede general clinical symptoms, that is, they can be considered as early symptoms of CML. In addition, their severity often prompts a patient with CML to see a dentist. Thus, the importance of the professional knowledge of the dentist in general clinical and dental symptomatology of CML, the correct routing of the patient for further clinical and laboratory examination, treatment and observation by a hematologist is undeniably high. The goal is to study and identify the features of the dental status of patients with chronic myeloid leukemia. Methodology. The work carried out a comprehensive assessment of the state of the oral cavity and the need for dental treatment of patients with chronic myeloid leukemia in which patients with preserved systemic status and patients with chronic myeloid leukemia took part. Results. According to the results of a complex dental examination and the calculation of the main dental indices, patients with CML were found to have serious disorders in the state of their oral health. Conclusions. Patients with CML showed a high need for various types of dental care, primarily in conservative oral cavity sanitation, periodontal treatment and rationalization of oral hygiene protocols (professional and individual).

Knowledge to recognize dental and oral diseases from an early age is very much needed to be able to maintain healthy teeth and mouth. Lack of knowledge about these oral and dental diseases will result in severe damage to teeth and mouth. This study aims to measure the severity of oral disease and provide a diagnosis of oral and dental disease so that later it can be used as a reference for consultation with a doctor. The method used in this research is Forward Chaining to represent the rules of 27 symptom facts and 8 diseases described by experts. The results of testing for this method are as many as 10 patient data were diagnosed to get the same result as the doctor's analysis so that the accuracy rate is 80%. The expert system designed with the Codeigniter Framework can provide insight by being able to identify the disease suffered by patients and measure the severity of the oral and dental disease suffered.

Cystic fibrosis (CF) promotes the development of gingivitis in children. The high prevalence and early manifestation of gingivitis is due to a combination of many factors. The pathogenic mechanisms of cystic fibrosis lead to disruption of normal physiological processes in the oral cavity, in particular, to a decrease in the level of salivation, to an increase in the viscosity of saliva, to a suppression of local immunity. Therefore, poor oral hygiene contributes to oral diseases. The aim of the study was to determine the level of oral hygiene in children with CF as one of the reasons for the development of gingivitis, and to select dental hygiene products that active towards factors that affect plaque. Material and methods. A total of 24 children with CF in the Ukrainian population participated in the study. The hygienic index OHI-S (J. C. Green, J. R. Wermillion, 1964) was calculated. The data were analyzed using Student’s criterions. The analysis was performed with a confidence level of 95 %. Results. The level of hygiene of the oral cavity was unsatisfactory among patients (OHI-S=2.19±0.11). Hygiene methods and dental hygiene products, to taking into account the manifestation of the pathogenesis of somatic disease, were selected. A follow-up study after 3 months revealed a significant positive trend in indicators of oral hygiene. Conclusions. This study has shown the low level of oral hygiene in children with CF. Hygienic education and motivation of these patients could help them achieve a higher level of oral health knowledge and are important starting points for the treatment and prevention of gingivitis. There was a significant improvement of the oral hygiene level after training children and after the proper selection of hygiene products. The dentists should use means and methods that have not contraindicate in patients of this category. It is recommended to monitore the hygiene indices to establish the effectiveness of the measures and necessity for individually therapy correction.

Purpose: to study the effect of type 2 diabetes mellitus on the state of the mucous membrane and tissues of the oral cavity to optimize the prevention and increase the effectiveness of dentition defects complex treatment in this contingent of patients. Materials and methods. We examined 112 patients with type 2 diabetes mellitus aged 35 to 64 years, who were further divided into three groups. The state of the oral mucosa, morphological changes, local immunity, lysozyme were determined. The work used morphological, microscopic methods, enzyme immunoassay and spectrophotometry. To test the statistical significance of intergroup differences, the nonparametric Mann- Whitney test was used. Results. Oral use in patients with diabetes mellitus type 2, applications of 1% Clotrimazole cream and Echinacea Compositum S had a positive preventive and therapeutic effect, which was accompanied by an increase in capillary resistance, a decrease in the degree of migration of leukocytes, an increase in the functional activity of immune system cells and an increase in the level of regenerative processes. In patients with defects in the dentition against the background of type 2 diabetes mellitus, violations of the oral cavity local immunity were revealed, characterized by a significant increase in the S-IgA content and a decrease in lysozyme.Orthopedic treatment with partial removable denturesled to a significant decrease in the concentration of S-IgA and an increase in the bactericidal activity of saliva due to increased lysozyme synthesis. It was concluded that type 2 diabetes mellitus affects the state of the oral mucosa, which must be taken into account when providing orthopedic and dental treatment.

Subject. Despite the many pathogenetic links in the lesions of the gastrointestinal tract and oral mucosa, unresolved issues related to their etiopathogenesis, diagnosis and high-quality therapeutic and preventive measures. Among the various pathological processes manifesting on the oral mucosa, recurrent aphthous stomatitis is a chronic disease characterized by the appearance of secondary pathomorphological elements (aphthae) in response to the development of acute proliferative inflammation. The goal is the analysis and systematization of literature data on the problem of damage to the oral mucosa in acid-dependent diseases of the gastrointestinal tract. Methodology. Recurrent aphthae of the oral cavity belong to the chronic pathology of the oral mucosa, are characterized by a prolonged course and periodic relapses, which are most often associated with the state of somatic status, including the presence of acid-dependent diseases of the gastrointestinal tract. Results. The main pathomorphological element inherent in this pathology is aphtha. Its localization is the mucous membrane of the vestibule of the oral cavity, cheeks, and lateral surface of the tongue, visually oval or round, covered with a dirty white fibrinous coating. Treatment of recurrent aphthae of the oral cavity includes a complex consisting of general systemic therapy of the underlying disease. Local treatment includes the use of antihistamines, immunocorrection, drugs that increase nonspecific hypersensitivity, normalization of cellular metabolism and stimulate nonspecific defense mechanisms, vitamin therapy and physiotherapy methods. Conclusions. This treatment should ultimately be aimed at eliminating not only pain, but also at increasing local immunity factors, combating pathogenic microflora and promoting the epithelization of pathological processes on the oral mucosa. Of great importance in the treatment and prevention of recurrent aphthae is a comprehensive examination of the patient in order to identify risk factors.

ABSTRACT Aim The aim of this study is to present a literature review on the effects of the ionizing radiation from radiotherapy treatment on dental tissues. Background Among the effects of increasing global life expectancy and longevity of the teeth in the oral cavity, increasing rates of neoplastic diseases have been observed. One of the important treatment modalities for head and neck neoplastic diseases is radiotherapy, which uses ionizing radiation as the main mechanism of action. Therefore, it is essential for dentists to be aware of the changes in oral and dental tissues caused by ionizing radiation, and to develop treatment and prevention strategies. Results In general, there is still controversy about the effects of ionizing radiation on dental structures. However, qualitative and quantitative changes in saliva and oral microbiota, presence of oral mucositis and radiation-related caries are expected, as they represent the well-known side effects of treatment with ionizing radiation. Points that still remain unclear are the effects of radiotherapy on enamel and dentin, and on their mechanisms of bonding to contemporary adhesive materials. Conclusion Ionizing radiation has shown important interaction with organic tissues, since more deleterious effects have been shown on the oral mucosa, salivary glands and dentin, than on enamel. Clinical significance With the increasing number of patients with cancer seeking dental treatment before and after head and neck radiotherapy, it is important for dentists to be aware of the effects of ionizing radiation on the oral cavity. Diseases of teeth and oral cavity have a high prevalence in pediatric population. In adolescents of 15-17 years, the incidence of caries in permanent teeth is higher than in children of 0-14 years. Epidemiological studies of dental health of children in various regions of the Russian Federation in 2010-2015 years showed no tendency to decrease the level of diseases of mouth and teeth. Prevalence and intensity of diseases remains at a high level. More than 80% of the surveyed children are in need of complex dental treatment. The current situation of pathology of teeth and mouth in children is due to several reasons such as low level of hygiene education, environmental degradation, poor nutrition, presence of chronic diseases, among older children, the presence of harmful habits, health-care organizations lack of effective preventive work. In turn, untreated inflammatory diseases of the oral cavity and teeth, being the source of the infection, may lead to somatic pathology of different organs and systems. The existing organization of children dental care is focused more on treatment than on prevention of diseases of teeth and oral cavity. As a rule, parents bring children to dental clinic for treatment of already developed disease. Referrals to the dentist for preventive measures are rare. In this regard, it is necessary to develop new organizational forms of dental care, from the point of view of the preventive orientation, close to children's educational organization.

Hereditary coagulopathies characterized by reduced blood clotting occupy a special place among diseases of the blood system. The most common of these are hemophilia and Willebrand's disease. Hemophilia-the disease of "royals" - is an old, but still relevant disease. Hereditary coagulopathies, characterized by reduced blood clotting, occupy a special place among diseases of the blood system. Based on who data and the likely frequency of research on these diseases, the prevalence of hemophilia ranges from 3 to 25 patients per 10,000 population. Few studies of the oral cavity in patients with hemophilia have shown a high level of dental diseases, poor hygiene of the oral cavity. This is due to the lack of proper oral care, and dispensary monitoring, fear of prolonged bleeding after dental manipulations. For this reason, dentists try to avoid treating dental diseases in this category of patients. At the same time, modern achievements of medical science in the field of Hematology and the development of effective replacement therapy allow to prevent almost all types of bleeding. However, this group of patients is still unable to receive timely and high-quality dental treatment and prevention, which largely determines the high prevalence of inflammatory diseases of the maxillofacial region. All of the above is evidence of the presence of many predisposing factors of damage to the dental system in patients with hemophilia, as well as the need for preventive, therapeutic, including orthopedic dental measures for this category of patients.
