The paper presents the ophthalmological diagnostic complex based on slit lamp, equipped with equipment for laser pa-nretinal photocoagulation. Pre-existing principle of construction of the ophthalmologic lasers involves a cascade of work of waves of different length, while in the new version of the device the ability to use two waves simultaneously and implementation of adjustment of the intensity of each wavelength. Pattern-scanning laser ophthalmo-coagulator works at two wavelengths 532 nm and 810 nm. Use of multi-waves laser source expands the range of possible applications in the treatment of retinal diseases. Medical-diagnostic complex is intended for stereoscopic observation, investigation of the anterior segment of the eyeball, lens, retina in medical, medical-preventive and research medical institutions and private clinics. This device allows to effectively conduct operations as pattern retinal laser photo-coagulation and laser trabeculoplasty green laser, and transpupilar thermotherapy of choroidal neovascularization, transcleral retinal photocoagulation and ciliary body by means of infrared (IR) laser. In the multi-colored photocoagulator, the technologies of "automatic positioning of the field of coagulation"(PASCAL) are used. The paper discusses the technical features of the instrument and its development prospects.

Purpose: We report a case of keratitis that improved after removal of a causative plant foreign body from below the posterior surface of an opaque cornea. The foreign body was revealed by anterior segment optical coherence tomography (AS-OCT) and gonioscopy.Case summary: A 79-year-old woman was referred with an impression of left-eye keratitis; the eye had been injured by a branch of a tangerine tree 1 month prior. She had been given the usual topical antibiotics by a local clinic, but they were ineffective. At her initial visit, her visual acuity was only hand motion in the left eye; slit-lamp examination revealed a 3 × 3-mm corneal infiltration with a hypopyon in the anterior chamber. Despite administration of strong topical antibiotics on an hourly basis, the corneal lesion worsened. AS-OCT and gonioscopy revealed a small foreign body below the posterior surface of the cornea; this was surgically removed. The corneal opacity and corneal epithelial defects dramatically improved, and the hypopyon disappeared.Conclusions: The possibility of a residual foreign body should be considered if trauma precedes infectious keratitis that does not improve with conventional treatment and the posterior surface of the cornea is not visible because of corneal opacity. In such a case, AS-OCT and gonioscopy can be useful.

Currently, one of the most important tasks is to improve the quality of medical services, which becomes possible in retrospective register analysis of medical and organizational technologies, which include algorithms for providing medical care, continuity of medical institutions, protocols for diagnosis and treatment of diseases. The stroke register was created on the basis of medical documentation of hospitals, which are the most typical for the Podillia region of Ukraine. Statistical processing of the results was performed using the licensed statistical package "Statistica 5.5" using parametric evaluation methods. Significant differences in the organization of the medical-diagnostic process and approaches to treatment have been established between the two medical institutions of Vinnytsia in the dynamics of time. There was a positive trend in the time of stroke treatment, the use of drugs with a high degree of evidence and the coverage of patients with specialized, multidisciplinary care in a stroke

Assessment of the anterior chamber angle (ACA) is an essential part of the ophthalmological examination. It is intrinsically related to the diagnosis and treatment of glaucoma and has a role in its prevention. Although slit-lamp gonioscopy is considered the gold-standard technique for ACA evaluation, its poor reproducibility and the long learning curve are well-known shortcomings. Several new imaging techniques for angle evaluation have been developed in the recent years. However, whether these instruments may replace or not gonioscopy in everyday clinical practice remains unclear. This review summarizes the last findings in ACA evaluation, focusing on new instruments and their application to the clinical practice. Special attention will be given to the comparison between these new techniques and traditional slit-lamp gonioscopy. Whereas ultrasound biomicroscopy and anterior segment optical coherence tomography provide quantitative measurements of the anterior segment’s structures, new gonio-photographic systems allow for a qualitative assessment of angle findings, similarly to gonioscopy. Recently developed deep learning algorithms provide an automated classification of angle images, aiding physicians in taking faster and more efficient decisions. Despite new imaging techniques made analysis of the ACA more objective and practical, the ideal method for ACA evaluation has still to be determined.

PurposeTo study the correlation between intraocular pressure (IOP) reduction and the number of hyper-reflective particles appearing in the anterior chamber following selective laser trabeculoplasty (SLT).Material and methodsIn this prospective interventional study, we included primary open-angle glaucoma patients. All participants received a standardised SLT session, which consisted of 100 pulses of 0.9 mJ over 360°. Anterior segment optical coherence tomography (4×4 mm volume scan, 101 horizontal cross-sectional scans) and applanation tonometry were performed before SLT and 15 min, 1 day, 1 week, 1 month and 3 months after. Particles were counted on cross-sectional scans using a standardised algorithm.ResultsIn this study, we included 25 patients (25 eyes), 14 males and 11 females, with a mean age of 68.9±10.5 and baseline IOP of 21.4±4.5 mm Hg. IOP at month 1 and month 3 after SLT was 18.0±4.0 and 17.4±3.3 mm Hg, respectively. The mean number of anterior chamber particles before and 15 min after SLT was 0.62±0.2 and 7.1±2.0 particles/mm2, respectively (p=0.036). There was a statistically significant correlation between the mean number of anterior chamber particles 15 min after SLT and IOP reduction at 1 month (r=0.62, p=0.03) and 3 months (r=0.71, p=0.01).ConclusionThe number of the anterior chamber particles graded using anterior segment optical coherence tomography after the procedure correlates with the IOP-lowering effect of SLT.

With the development of digital image processing technology and computer technology, the capturing and processing of anterior segment image by computer to acquire feature information is possible. This paper captures the anterior eye segment by slit lamp, and then uses image processing technology to preprocess, extract the edge and fit the edge of the captured image. At the same time the paper establishes the information management system based on SSH framework, which can register patient information, manage patient files and images, and finally realize the paperless medical procedure.
