Background: Equity in the distribution of equipment and resources in the health sector constitutes one of the main dimensions of justice, improving health status and increasing people's satisfaction. Objectives: The current study aimed at determining how health resources and equipment are distributed in the poorest region of Iran. Methods: The current retrospective, cross-sectional study was performed in Sistan and Baluchistan Province, as the poorest region of Iran, in 2020. Data were collected using a standard checklist from all five health centers affiliated to Zahedan University of Medical Sciences (ZAUMS). Excel and STATA software was used for data analysis; the Gini index was also calculated. Results: The Gini index for the distribution of midwives and rural health workers was equitable (0.216 and 0.287, respectively). The distribution of urban healthcare providers, nutrition and psychology counselors, and active beds in ZAUMS were 0.675, 0.545, 0.454, and 0.526, respectively. The distribution of general practitioners, based on the Gini index (0.492), was not fair. Also, the distribution of specialists and nurses was unfair (Gini index: 0.494 and 589, respectively). Conclusions: The distribution of most resources in the poorest region of Iran was not fair. Unfair distribution and disparities can affect population health status in the future. Then, it is suggested that health policymakers and managers pay more attention to deprived regions.

Abstract Background Healthcare organisations have undergone organisational change to respond to COVID-19. This pandemic has presented challenges for employee adjustment, with impacts on the availability and coordination of human resources in healthcare. This study aimed to characterise the organisational actions regarding the coordination of human resources in healthcare within Bogotá, Colombia, to respond to the COVID-19 pandemic. Methods We followed a case study approach to understand the response to the emergency taking into account the narratives of the actors who have been directly involved in the planning of guidelines oriented to face the pandemic or in the implementation of health services for COVID-19. Twenty-two interviews with multiple health system organisations within Bogotá were conducted between May and September 2020 and analysed thematically. Results Three themes emerged from the analysis of the interview data: to retain human resources, to implement actions to improve the mental and physical health of the healthcare workers, and to enhance healthcare workers knowledge, skills and availability to respond to COVID-19. Conclusions Organisational actions led by hospital managers to retain, protect, and train human health resources in the dynamic context of the COVID-19 pandemic were identified. Other system-wide organisations like scientific associations contributed to the coordination of human resources across hospitals to respond to COVID-19 in Bogotá, Colombia. The actions of hospital managers, and roles of system-wide intermediary organisations, in coordinating human resources need to be explored in other health system contexts facing COVID-19.

During the COVID-19 pandemic times, non-COVID patients faced many difficulties to access health care because of diversion of health resources to manage the pandemic, partial or complete closure of routine OPD services and travel restrictions due to lockdown. Increased use of technology enhanced the effectiveness of the healthcare system to maintain treatment avenues in this challenging time. Also, the pandemic accelerated diffusion of technological innovations like teleconsultation among healthcare workers and patients. The present case study is about a young, breast cancer, female patient from rural India who was diagnosed with the disease just before the pandemic times. She was put on neo-adjuvent chemotherapy in a tertiary care hospital about 150 kms away from her native place but meanwhile lockdown was imposed in India to control COVID-19 spread. She was maintained on follow up through teleconsultation services. She was facilitated through travel restrictions for chemotherapy cycles by e-pass. Her concerns regarding disease, treatment options, follow ups, effect of treatment on her fertility and adverse effects of chemotherapy during her treatment course were satisfactorily addressed through teleconsultation. Necessary investigations done nearby were sent digitally to treating doctor that minimized her visits. After completion of chemotherapy she underwent breast conservation surgery.
