This paper describes effects of a probability of detection (POD) of a stress corrosion crack on the reliability of the piping in a nuclear power plant (NPP). Various POD curves were proposed using the results of Japanese study on the detection for the stress corrosion crack which is frequently observed in austenitic stainless steel piping of boiling water reactor (BWR). Based on the proposed POD curves, the reliability of a flawed pipe was analyzed using probabilistic fracture mechanics (PFM) code. The results suggest that the detectable crack depth and oversight probability (e.g. human error) is important on the reliability of piping with the stress corrosion crack. The reliability of piping depends on the detectable crack depth rather than the oversight probability when the detectable crack depth is larger than 3mm. Meanwhile, it depends on the oversight probability, when the detectable crack depth is 3mm or less.

PT “X” adalah industri tembakau yang memiliki tingkat kecelakaan kerja cukup tinggi, olehkarena itu diperlukan tindakan untuk menghindari atau meminimalkan terjadinya kecelakaan. HazardIdentification and Risk Analysis (HIRA) adalah metode yang bertujuan untuk mengidentifikasi semuajenis bahaya yang terkandung dalam kegiatan kerja. Sedangkan metode Standardized Plant AnalysisRisk Human Reliability Assessment (SPAR-H) adalah sebuah metode yang bertujuan untuk mengetahuikemungkinan terjadinya kesalahan manusia ketika melakukan aktivitas kerja. Dari penelitian yang telahdilakukan, disimpulkan bahwa terjadinya kecelakaan di tempat kerja PT “X” disebabkan oleh adanyakondisi lingkungan dan tindakan pekerja yang tidak aman serta ketidaksesuaian dengan undang-undangK3. PT “X” perlu melakukan beberapa tindakan korektif sehingga kecelakaan dapat diminimalkan. Dalamstudi tersebut, PT “X” perlu membuat perbaikan melalui tindakan administrasi dan penggunaan peralatanperlindungan.Kata kunci: kecelakaan kerja, HIRA, SPAR-HABSTRACTThe accident occurred in industry is one of the causes of work disruption or cessation. Work accidentshould be avoided because of causing risk for company. PT “X” is a tobacco company which has a high levelof work accident. Therefore, it is necessary to establish a particular effort in order to prevent and reduce theaccident. Hazard Identification and Risk Analysis (HIRA) is the method aims to identify any risk typesincluded in the work operation. Where as Standardized Plant Analysis Risk Human Reliability Assessment(SPAR-H) is the method aims to find out the possibility of human error occurrence in the work operations.Based on the research conducted, it can be concluded that the accident occurred in PT. “X” is caused byenvironmental condition and insecure workers’ action, and inappropriateness with HSE procedures. PT“X” should enroll some corrective actions so the accident can be reduced. In this study, PT “X” should createimprovement through administrative action and the use of safety equipment.Key words: work accident, HIRA, SPAR-H

Human error has played a critical role in the events precipitating the road accidents. Such accidents can be predicted and prevented by risk assessment, in particular assessing the human contribution to risk. As part of the Human Reliability Assessment (HRA) process, it is usually necessary not only to define what human errors can occur, but how often they will occur. Lack of understanding of the failure distribution characteristics of drivers on roads at any given time is a factor impeding the development of human reliability assessment and prediction of road accidents in order to take best proactive measures. The authors developed the complete investigation methodology for crash data collection. Furthermore, they have experimentally tested the proposed predictive behavioral characteristics of drivers in light of their instantaneous error rate over the course of driving period to assist processing and analysis of data collection as part of risk assessment. The findings of this research can assist road safety authorities to collect the necessary data, to better understand the behavioral characteristics of drivers on roads, to make more accurate risk assessments and finally to come up with right preventive measures.

This paper describes some of the major areas of interest in the field of human reliability. The nature of system reliability assessment is described, and the importance of considering human reliability emphasized. Human error is then discussed from several standpoints, and techniques for its quantitative assessment described. The review concludes with a description of the various sources of human reliability data and the research that is currently in progress.

Seluruh industri pasti memiliki keinginan untuk memiliki zero accident. Namun pada kenyataannya banyak perusahaan yang memiliki angka kecelakaan yang tinggi tiap tahunnya. Human Reliability Assessment merupakan salah satu metode untuk memberi usulan alternatif pengurangan terhadap kecelakaan kerja yang terjadi. Dimana langkah yang digunakan yakni mengumpulkan data kecelakaan kerja,data Task Analysis Sistem dan data identifikasi kegagalan. Pengolahan yang dilakukan yakni dengan penggambaran kecelakaan kerja menggunakan Fault Tree Analysis,kemudian kuantifikasi nilai Human Error Probability dengan metode Human Error And Reduction Technique dan pada akhirnya akan ditemukan usulan alternatif pengurangan kecelakaan kerja. Hasil dari penelitian ini adalah mengidentifikasi kesalahan manusia yang menimbulkan kecelakaan kerja. Kesalahan karyawan tersebut antara lain posisi pemotongan kurang benar dengan probabilitas tertinggi yaitu 0,728, untuk yang lain seperti gagal memposisikan saat pengambilan material, salah posisi dalam melakukan prosedur,tidak fokus dalam melakukan proses,tidak memperhatikan posisi kayu dan terburu-buru dalam melakukan prosedur probabilitasnya dibawah 0,728.
