<b><i>Introduction:</i></b> To determine whether combined exercise training with an energy-restricted diet leads to improved physical fitness and body composition when compared to energy restriction alone in free-living premenopausal women with clinically severe obesity. <b><i>Methods:</i></b> Sixty premenopausal women (BMI of 40.4 ± 6.7) were randomised to energy restriction only (ER) or to exercise plus energy restriction (EXER) for 12 months. Body composition and fitness were measured at baseline, 3, 6 and 12 months. <b><i>Results:</i></b> VO<sub>2 peak</sub> improved more for EXER compared to ER at 3 (mean difference ± SEM 2.5 ± 0.9 mL ∙ kg<sup>–1</sup> ∙ min<sup>–1</sup>, <i>p</i> = 0.006) and 6 (3.1 ± 1.2 mL ∙ kg<sup>–1</sup> ∙ min<sup>–1</sup>, <i>p</i> = 0.007) but not 12 months (2.3 ± 1.6 mL ∙ kg<sup>–1</sup> ∙ min<sup>–1</sup>, <i>p</i> = 0.15). Muscle strength improved more for EXER compared to ER at all time points. No differences between groups for lean mass were observed at 12 months. <b><i>Conclusion:</i></b> Combining exercise training with an energy-restricted diet did not lead to greater aerobic power, total body mass, fat mass or limit lean body mass loss at 12 months when compared to energy restriction alone for premenopausal women with clinically severe obesity in free-living situations. Future research should aim to determine an effective lifestyle approach which can be applied in the community setting for this high-risk group.

Diets utilising intermittent fasting (IF) as a strategic method to manipulate body composition have recently grown in popularity, however, dietary practices involving fasting have also been followed for centuries for religious reasons (i.e., Ramadan). Regardless of the reasons for engaging in IF, the impacts on lean body mass (LBM) may be detrimental. Previous research has demonstrated that resistance training promotes LBM accrual, however, whether this still occurs during IF is unclear. Therefore, the objective of this review is to systematically analyse human studies investigating the effects of variations of IF combined with resistance training on changes in LBM in previously sedentary or trained (non-elite) individuals. Changes in body weight and fat mass, and protocol adherence were assessed as a secondary objective. This review followed the preferred reporting items for systematic reviews and meta-analyses (PRISMA) guidelines. MEDLINE, CINAHL, PubMed and SportDiscus databases were searched for articles investigating IF, combined with resistance training that reported measures of body composition. Eight studies met the eligibility criteria. LBM was generally maintained, while one study reported a significant increase in LBM. Body fat mass or percentage was significantly reduced in five of eight studies. Results suggest that IF paired with resistance training generally maintains LBM, and can also promote fat loss. Future research should examine longer-term effects of various forms of IF combined with resistance training compared to traditional forms of energy restriction. Failure of various weight-loss programs and long-term maintenance of favorable body composition in all kinds of people is high, since the majority go back to old dietary patterns. Many studies have documented the efficacy of a plant-based diet (PBD) for body mass management, but there are opinions that maintaining a PBD is difficult. We aimed to evaluate the long-term success of a whole-food plant-based (WFPB) lifestyle program. We investigated the differences in the obesity indices and lifestyle of 151 adults (39.6 &plusmn; SD 12.5 years), who were on our program for short (0.5&ndash;&lt;2 years), medium (2&ndash;&lt;5 years), or long term (5&ndash;10 years). Body-composition changes were favourable for all three groups, both genders and all participants. There were no differences in relative body-composition changes (BMI, body fat percentage and muscle mass index (MMI)) between the three groups. All participants improved their BMI (baseline mean pre-obesity BMI range (kg/m2): 26.4 &plusmn; 5.6 to normal 23.9 &plusmn; 3.8, p &lt; 0.001), decreased body mass (&ndash;7.1 &plusmn; 8.3 kg, p &lt; 0.001) and body fat percentage (&ndash;6.4 &plusmn; 5.6 % points, p &lt; 0.001). 85.6% (101 out of 118) of parents of underage children (&lt; 18 years), introduced WFPB lifestyle to their children. Those with the highest BMI at baseline lost the most of: a) BMI units, b) total body mass and c) body fat (a) (kg/m2) (&ndash;5.6 &plusmn; SD 2.9, &ndash;2.4 &plusmn; 1.8 and &ndash;0.9 &plusmn; 1.5), b) (kg) (&ndash;16.1 &plusmn; SD 8.8, &ndash;7.1 &plusmn; 5.4 and &ndash;2.5 &plusmn; 4.5) and c) (% points) (&ndash;9.5 &plusmn; SD 5.7, &ndash;6.6 &plusmn; 4.6 and &ndash;4.7 &plusmn; 5.3) for participants who had baseline BMI in obese, overweight and normal range, respectively; pbaseline vs. current &lt; 0.001 for all). WFPB lifestyle program provides long-term lifestyle changes for reversal of obesity and is effective transferred to the next generation.

Changing body composition has been suggested as a pathway to explain age-related functional decline. No data are available on the expected changes in body composition as measured by dual-energy X-ray absorptiometry (DXA) in a population-based cohort of older persons. Body composition data at baseline, 1-yr follow-up, and 2-yr follow-up was measured by DXA in 2,040 well-functioning black and white men and women aged 70–79 yr, participants of the Health, Aging, and Body Composition Study. After 2 yr, a small decline in total body mass was observed (men: −0.3%, women: −0.4%). Among men, fat-free mass and appendicular lean soft tissue mass (ALST) decreased by −1.1 and −0.8%, respectively, which was masked by a simultaneous increase in total fat mass (+2.0%). Among women, a decline in fat-free mass was observed after 2 yr only (−0.6%) with no change in ALST and body fat mass. After 2 yr, the decline in ALST was greater in blacks than whites. Change in total body mass was associated with change in ALST ( r = +0.58 to +0.70; P < 0.0001). Among participants who lost total body mass, men lost relatively more ALST than women, and blacks lost relatively more ALST than whites. In conclusion, the mean change in body composition after a 1- to 2-yr follow-up was 1–2% with a high interindividual variability. Loss of ALST was greater in men compared with women, and greater in blacks compared with whites, suggesting that men and blacks may be more prone to muscle loss.

Correlations of body composition with height vary by age and sex during childhood. Standard approaches to accounting for height in measures of body composition (dividing by height (in meters)2) do not take this into account. Using measures of total body mass (TBM), fat mass (FM) and fat free mass (FFM) at ages nine, 11, 13, 15 and 18 years from a longitudinal UK cohort study (ALSPAC), we calculated indices of body composition at each age by dividing measures by height (in meters)2. We then produced age-and sex-specific powers of height using allometric regressions and calculated body composition indices by dividing measures by height raised to these powers. TBM, FM and FFM divided by height2 were correlated with height up-to age 11 in females. In males, TBM and FM divided by height2 were correlated with height up-to age 15 years while FM divided by height2 was correlated with height up-to age 11 years. Indices of body composition using age-and sex-specific powers were not correlated with height at any age. In early life, age-and sex-specific powers of height, rather than height in meters2, should be used to adjust body composition for height when measures of adiposity/mass independent of height are required.

β-hydroxy-β-methylbutyrate (HMB) is a leucine metabolite that is purported to increase fat-free mass (FFM) gain and performance in response to resistance exercise training (RET). The aim of this systematic review and meta-analysis was to determine the efficacy of HMB supplementation in augmenting FFM and strength gains during RET in young adults. Outcomes investigated were: total body mass (TBM), FFM, fat mass (FM), total single repetition maximum (1RM), bench press (BP) 1RM, and lower body (LwB) 1RM. Databases consulted were: Medical Literature Analysis and Retrieval System Online (Medline), Excerpta Medica database (Embase), The Cumulative Index to Nursing and Allied Health Literature (CINAHL), and SportDiscus. Fourteen studies fit the inclusion criteria; however, 11 were analyzed after data extraction and funnel plot analysis exclusion. A total of 302 participants (18–45 y) were included in body mass and composition analysis, and 248 were included in the strength analysis. A significant effect was found on TBM. However, there were no significant effects for FFM, FM, or strength outcomes. We conclude that HMB produces a small effect on TBM gain, but this effect does not translate into significantly greater increases in FFM, strength or decreases in FM during periods of RET. Our findings do not support the use of HMB aiming at improvement of body composition or strength with RET.

The aim of the present study was to investigate the association of PLIN1 11482G>A (rs894160) and PLIN1 13041A>G (rs2304795) polymorphisms with body composition, energy and substrate metabolism, and the metabolic response to a 12-week energy-restricted diet in obese women. The study comprised a total of seventy-eight obese (BMI 34·0 (sd 2·8) kg/m2) women (age 36·7 (sd 7) years). We measured weight, height and waist circumference before and after a 12-week controlled energy-restricted diet intervention. Body fat mass and lean mass were measured by dual-energy X-ray absorptiometry. RMR and lipid oxidation rate were measured by indirect calorimetry. We also analysed fasting plasma glucose, insulin, cholesterol and leptin. Women carrying the 11482A allele had a lower reduction in waist circumference than non-A allele carriers (3·2 (sd 0·5) v. 4·6 (sd 0·6) %, respectively, P = 0·047; P for gene–diet interaction = 0·064). Moreover, women with the 11482A allele had a higher decrease in lipid oxidation rate than non-A allele carriers (58·9 (sd 6·7) v. 31·3 (sd 8·2) %, respectively, P = 0·012; P for gene–diet interaction = 0·004). There was no interaction effect between the 13041A>G polymorphism and diet-induced changes on the outcome variables (all P>0·1). These results confirm and extend previous findings suggesting that the PLIN1 11482G>A polymorphism plays a modulating role on diet-induced changes in body fat and energy metabolism in obese women.

Context:When weight loss (WL) is needed, it is recommended that athletes do it gradually by 0.5–1 kg/wk through moderate energy restriction. However, the effect of WL rate on long-term changes in body composition (BC) and performance has not been investigated in elite athletes.Purpose:To compare changes in body mass (BM), fat mass (FM), lean body mass (LBM), and performance 6 and 12 mo after 2 different WL interventions promoting loss of 0.7% vs. 1.4% of body weight per wk in elite athletes.Methods:Twenty-three athletes completed 6- and 12-mo postintervention testing (slow rate [SR] n = 14, 23.5 ± 3.3 yr, 72.2 ± 12.2 kg; fast rate [FR] n = 9, 21.4 ± 4.0 yr, 71.6 ± 12.0 kg). The athletes had individualized diet plans promoting the predetermined weekly WL during intervention, and 4 strength-training sessions per wk were included. BM, BC, and strength (1-repetition maximum) were tested at baseline, postintervention, and 6 and 12 mo after the intervention.Results:BM decreased by ~6% in both groups during the intervention but was not different from baseline values after 12 mo. FM decreased in SR and FR during the intervention by 31% ± 3% vs. 23% ± 4%, respectively, but was not different from baseline after 12 mo. LBM and upper body strength increased more in SR than in FR (2.0% ± 1.3% vs. 0.8% ± 1.1% and 12% ± 2% vs. 6% ± 2%) during the intervention, but after 12 mo there were no significant differences between groups in BC or performance.Conclusion:There were no significant differences between groups after 12 mo, suggesting that WL rate is not the most important factor in maintaining BC and performance after WL in elite athletes.

Abstract Purpose Increasing protein intake during energy restriction (ER) attenuates lean body mass (LBM) loss in trained males. However, whether this relationship exists in trained females is unknown. This study examined the impact of higher compared to lower protein intakes (35% versus 15% of energy intake) on body composition in trained females during 2 weeks of severe ER. Methods Eighteen well-trained females completed a 1-week energy balanced diet (HD100), followed by a 2-week hypoenergetic (40% ER) diet (HD60). During HD60, participants consumed either a high protein (HP; 35% protein, 15% fat) or lower protein (CON; 15% protein, 35% fat) diet. Body composition, peak power, leg strength, sprint time, and anaerobic endurance were assessed at baseline, pre-HD60, and post-HD60. Results Absolute protein intake was reduced during HD60 in the CON group (from 1.6 to 0.9 g·d·kgBM−1) and maintained in the HP group (~ 1.7 g·d·kgBM−1). CON and HP groups decreased body mass equally during HD60 (− 1.0 ± 1.1 kg; p = 0.026 and − 1.1 ± 0.7 kg; p = 0.002, respectively) and maintained LBM. There were no interactions between time point and dietary condition on exercise performance. Conclusion The preservation of LBM during HD60, irrespective of whether absolute protein intake is maintained or reduced, contrasts with findings in trained males. In trained females, the relationship between absolute protein intake and LBM change during ER warrants further investigation. Future recommendations for protein intake during ER should be expressed relative to body mass, not total energy intake, in trained females.

OBJECTIVE: To investigate the effectiveness of a body mass reduction programme entailing diet caloric restriction and moderate physical activity with or without supplementary treatment with recombinant (r) GH or steroids to improve body composition and muscle performance in severely obese women aged 61-75 years. METHODS: Twenty women were randomly assigned to one of three groups: body mass reduction alone; body mass reduction plus rGH; body mass reduction plus nandrolone undecanoate. Body composition, isotonic muscle strength and anaerobic power output during jumping were determined before and after the 3-week period. RESULTS: Whatever the experimental group considered, body mass (P<0.01), body mass index (P<0.05) and fat mass (P<0.05) decreased significantly, whereas muscle strength and power increased significantly (P<0.05) after the intervention. CONCLUSION: Small body mass reductions after 3 weeks of energy-restricted diet combined with moderate aerobic and strength exercise are associated with significant improvements in upper and lower limb muscle strength and power and reduction of fat mass in severely obese women aged 61-75 years. Although the association of rGH or nandrolone undecanoate does not appear to exert additional effects on body composition and muscle performance attained by body mass reduction alone, further additional studies with larger study groups, different dosages and more prolonged periods are required for definitive conclusions to be drawn.

Objectives. Nordic walking (NW) is relatively new and popular type of physical exercise with less studied effects than other sports activities. The aim of the study was to analyze possible changes in somatic indices, oxidant and antioxidant status, interleukins, and calcidiol levels in middle-aged women after a 12-week NW training program. Study Design. In this study, we examined the effects of NW training on selected measures and changes in body weight, fat mass, and calcidiol levels. Methods. The study group consisted of 13 women (46 ± 4.2 years), who took part in trainings. Before and after the training program, some anthropometric indices were determined and selected biochemical parameters were measured in blood. Results. NW training led to a significant decrease of the total body mass and fat mass and to an increase in lean body mass (p<0.05). It also contributed to a significant increase in total antioxidative status (TAS) and calcidiol levels (p<0.05). Before training, a reverse correlation between IL-6 and total oxidative capacity (TOC) levels (p<0.05) was found, while after training between IL-6 and calcidiol levels (p≤0.001). Conclusions. 12-week NW training undertaken by premenopausal women not only has a positive effect on body composition but also on the plasma antioxidative capacity.
