Linear psoriasis is a rare form of the disease characterized by the linear distribution of lesions involving dermatome or along Blaschko’s lines. Clinically, it may resemble inflammatory linear verrucous epidermal nevus; a combination of history, skin examination, and histopathology are required to ensure correct diagnosis and appropriate therapy. This paper describes a case of a 23-year-old male presenting with unilateral erythematous scaly plaques arranged in a linear path on the left leg. Etanercept was initiated after poor response to adalimumab. Improvement of his psoriasis was noted, with PASI 75 reduction after 24 weeks of treatment. Clinical studies have shown excellent efficacy of etanercept, and our patient well tolerated treatment with etanercept for 52 weeks without any adverse effects.

Linear psoriasis is a rare form of disease which is very difficult to differentiate from inflammatory linear verrucous epidermal nevus. Sometimes clinical history, physical examination and histopathology analysis may not be sufficient to confirm the diagnosis. We report a case of25 year-old male presented with a linear plaque covered with silvery scales on left upper extremity extending from tip of the index to mid forearm for last 2 years. A skin biopsy was consistent with psoriasis, and the unilateral distribution in a linear pattern led to a diagnosis of linear psoriasis, which is a rare variant of psoriasis. Although histopathologically it can be difficult to distinguish from inflammatory linear verrucous epidermal nevus (ILVEN), linear psoriasis presents in adulthood and responds to conventional topical antipsoriatic therapies.

Pregnancy- and lactation-associated osteoporosis (PLO) is a rare form of osteoporosis. It results in severe low back pain in the last trimester of pregnancy and in the postpartum period, decreases in height, and fragility fractures, particularly in the vertebra. The current case report presents a 32-year-old patient who presented with back and low back pain that began in the last trimester of the pregnancy and worsened at two months postpartum and who was diagnosed with pregnancy- and lactation-associated osteoporosis after exclusion of other causes; the findings are discussed in view of the current literature. PLO is a rare clinical condition causing significant disability. PLO must be kept in mind in the differential diagnosis in patients presenting with low back pain during or after pregnancy. The patients must be evaluated for the risk factors of PLO, and an appropriate therapy must be initiated.

CASE REPORT This article describes a case report with a review of the symptomatology diagnosis, and treatment of thrombophlebitis in the superficial dorsal vein of the penis. Penile Mondor’s disease is a benign condition, and after appropriate therapy, near complete recovery takes place within three weeks. DISCUSSION Thrombophlebitis of the superficial dorsal vein of the penis (Penile Mondor’s disease) is a rare, but important clinical diagnosis that any physician, and in particular general practitioners, should be able to recognize. Indeed, correct diagnosis and consequent reassurance can help to control the anxiety typically experienced by patients suffering from the disease.

Background: Recent literature and guidelines support treatment of severe acute exacerbation of COPD (AECOPD) with prednisone 40 mg (or equivalent) for 5 days. Objective: The purpose of this study was to determine whether systemic corticosteroids were being prescribed at the appropriate dose and duration in the treatment of severe AECOPD at a large academic medical institution. Methods: This was a single-center, retrospective, observational study conducted to evaluate corticosteroid prescribing patterns for adults admitted for severe AECOPD from March to May 2019 at a large academic medical institution. Appropriate therapy was defined as: prednisone 40 mg (or equivalent) for 5 days. The primary outcome was to determine the frequency of appropriate dose and duration. The secondary outcomes were to identify the frequency of appropriate dose or duration, compare the prevalence of adverse effects, such as new/worsening hyperglycemia or hypertension, and compare 30- and 90-day readmission rates. Results: Of the 190 patients included, 16 (8.4%) had an appropriate duration and 8 (4.2%) an appropriate dose. Only 4 patients (2.1%) had both appropriate corticosteroid dose and duration. New/worsening hyperglycemia occurred in 96 (50.5%), and hypertension developed in 13 (6.8%). Thirty-day readmission occurred in 46 (24.2%) and 90-day readmission in 78 (41.1%). These were more likely in those without appropriate dose and duration. Conclusion: Systemic corticosteroids for the treatment of severe AECOPD are not prescribed in accordance with evidence-based recommendations at this institution. This might result in a greater incidence of adverse effects and readmissions.

The association between cytomegalovirus infection and inflammatory bowel disease challenges both the clinician and the pathologist to establish the correct diagnosis and to prescribe the most appropriate form of therapy. To understand this association the authors report three patients who presented with signs and symptoms mimicking reactivated inflammatory bowel disease who responded poorly to aggressive treatment of inflammatory bowel disease. Microscopic examination, in all three cases revealed numerous nuclear and cytoplasmic viral inclusions, as demonstrated by cytomegalovirus immunohistochemistry, as well as histologic findings consistent with inflammatory bowel disease (ulcerative colitis and/or Crohn's disease). Because the clinical pathologic features of cytomegalovirus colitis and inflammatory bowel disease often overlap, and because of the possible coexistence of cytomegalovirus colitis with idiopathic colitis, the possibility of cytomegalovirus infection should be always considered, so that the most appropriate therapy can be instituted for these patients.

Introduction. Cystadenofibromas are tumors of the ovary which originate from the surface coelomic epithelium. Benign mucinous cystadenofibroma is a very rare form of these tumors, which consists of dominant stromal component of the connective tissue and one or more cysts. Case report. The case of a 62-year-old female with tumor of right ovary is reported in this paper. Histologically, tumor of the ovary had multilocular cystic formation, lined by a single-layer of mucoproductive cylindrical epithelium - endocervical type. In one area of tumor, the stromal component was abundant and made from partially hyalinised dense connective tissue. Mucinous cystadenofibroma was diagnosed on the basis of histological examination. Since the mucinous type of cystadenofibroma or adenofibroma is rare, this case has been chosen to be presented. Conclusion. Mucinous cystadenofibromas are differentially- diagnostically very similar to different malignant tumors and it is extremely important to make correct diagnosis of these neoplasms.

Inflammatory linear verrucous epidermal nevus is a variant of verrucous epidermal nevus, characterized by recurrent inflammatory phenomena. Despite well-established clinical manifestations, the differential diagnosis between inflammatory linear verrucous epidermal nevus and linear psoriasis remains difficult. Clinical history, physical examination and histopathology analysis may not be sufficient to confirm the diagnosis. We report the case of a 4-year-old girl in which the involucrin immunostaining was helpful in the diagnosis of inflammatory linear verrucous epidermal nevus. Our findings confirm that involucrin immunohistochemistry is a useful tool in such cases.

For practitioners, selecting successful therapy for vulvovaginal candidiasis is anything but trivial. The dominant problem; however, lies not with selecting the correct antimycotic agent, but with making the correct diagnosis and not treating non-yeast infections and noninfections as yeast-induced. Moreover, not all cases of vulvovaginal candidiasis are equal and practitioners owe patients the obligation of selecting appropriate therapy based upon the specific type and severity of vulvovaginal candidiasis. Uncomplicated candidiasis is readily treated with short-term oral or topical therapy, whereas complicated candidiasis needs additional strategies using the plethora of therapies available. Problematic refractory cases still abound and the pipeline for new, more potent antifungal agents is largely empty. Management strategies for complicated Candida vaginitis are discussed.

Cornual pregnancy is rare form of ectopic pregnancy where implantation occurs in the cavity of a rudimentary horn of the uterus. It is the diagnostic and therapeutic challenge with potential sever consequence if uterine rupture occurs with massive intra abdominal haemorrhage .We report a case of misdiagnosed rupture cornual pregnancy occurring at 12 weeks gestation. First USG revealed intra uterine pregnancy. Repeat USG showed abdominal pregnancy sac but cornual pregnancy was not diagnosed and uterine anomaly was not detected. The correct diagnosis was made at emergency laparotomy. An intact pregnancy sac of 12weeks fetus was seen in abdominal cavity. Placenta and pregnancy sac was adherent to omentum and torn end of left cornu of the bicornuate uterus. Left sided cornu was resected keeping both the ovaries and tubes intact.
