Bacterial esophagitis is a very rare condition usually occurring in patients with immunosuppression. To our best knowledge, bacterial esophagitis without underlying immunosuppressive disease has not been reported. We report an immunocompetent patient with bacterial esophagitis caused by B-hemolytic Streptococcus which resulted in an esophageal stricture. A 68-year-old female was admitted for odynophagia which had developed several days before. Upper endoscopy revealed extensive ulceration covered by whitish exudates with submucosal edema at the proximal esophagus. She was treated with steroids and empirical broad-spectrum antibiotics. Within 14 days the symptoms improved. Since growth of B-hemolytic Streptococcus was detected in nasal smear culture, bacterial esophagitis was suspected. Gram staining was carried out on the already obtained tissue that had been fixed with formalin. There was heavy infiltration with gram-positive cocci morphologically consistent with Streptococcus. Since the bacterial colony was demonstrated histologically, the diagnosis of bacterial esophagitis caused by B-hemolytic Streptococcus was confirmed. In addition, complete resolution of the inflammation following antibiotics therapy was further evidence of the bacterial cause of the esophagitis.

A preliminary morphological screening and isolation of bacterial colony from latex industrial wastewater was carried out. Bacteria colonies from latex processing wastewater were isolated from a local latex processing industry. It was found that 17 bacterial isolates had been purified grown on nutrient agar under 35˚C. The colonies were then purified and morphologically indicated via Gram staining and motility test. After morphological observation, it was identified that out of 17 isolates, 9 isolates were Gram positive and 8 isolates were Gram negative. There are 11 out of 17 colonies were rod-shaped bacterial colonies, while the other 6 colonies were cocci-shaped bacteria. There were 11 colonies of gliding bacteria, three colonies were non-motile bacteria and the other three colonies were flagellated bacteria. This study is only limited to morphological observation as the main aim of this study was to investigate the potential occurrence of viable growth in treated latex processing wastewater. The bacterial colonies were classified base on their morphological properties shown. This study has classified several genera such as Staphylococcus, Escherichia, Thiobacillus, Arthrobacter and other Genus. The growth curve of 17 isolates studied and the chemical oxygen demand were determined.

Sporothrix brasiliensis usually causes infection limited to the skin, subcutaneous tissue and regional lymph nodes. Contamination occurs through inhalation or accidental inoculation from animal scratches and bites. Meningitis is rare and mostly occurs in immunosuppressed patients. Here, we describe an immunocompetent person who developed chronic meningitis and discuss the diagnosis, differential diagnosis and treatment of this rare condition.

Streptococcus intermedius is a Gram-positive cocci, normally found in the oral cavity and gastrointestinal tract. It has been associated with deep-seated purulent abscesses commonly in the brain or liver in immunocompromised patients. Here, we discuss the case of a 21-year-old immunocompetent patient that presented with septic shock in the setting of multiple pyogenic liver abscesses with positive blood cultures for S. intermedius. The patient had a dental cleaning 3 months prior to admission. The abscesses resolved with ultrasound guided drainage and antibiotic therapy.

Carcinoid tumors are rare neuroendocrine malignancies that typically originate from the gastrointestinal tract. Patients who are diagnosed with carcinoid heart disease generally have poor prognoses because of advanced metastases during staging and few therapeutic options. We present the case of a 61-year-old woman with right-sided heart failure, secondary to carcinoid heart disease caused by a primary ovarian carcinoid tumor. After undergoing surgical resection of the left ovary and fallopian tube, the patient experienced complete resolution of her heart failure symptoms. In addition to the patient's case, we discuss the diagnosis, nature, and treatment of this rare condition.

AbstractBetween July 2 and July 16, 1979, three patients were reported to have gram-positive cocci as seen on Gram stain of excised, macerated heart valve tissue. No organisms were isolated in culture; however, our usual procedure for handling such valves is not adequate for isolation of strict anaerobes. The first patient was treated for presumed endocarditis; this resulted in three additional weeks of hospitalization.Review of the remaining 34 patients who had heart valve replacement surgery between February 1 and July 20, 1979 revealed three additional cases.Careful analysis revealed that nonviable, gram-positive cocci present in commercially prepared trypticase soy broth were being added to the tissue just prior to mortar and pestle grinding in the microbiology laboratory. The contaminated tubes were noted to have a visible sediment when carefully examined.The procedure for handling the broth has been revised; a simple screening procedure is used now to identify potentially contaminated broth. No further instances of false positive results on Gram-staining of heart value tissue have occurred since this screening procedure was introduced.

Abstract Enlargement of breast tissue in males, or gynecomastia, is a rare condition in prepubescent boys. While the majority of cases are idiopathic, we describe an 8-year-old patient who developed unilateral gynecomastia secondary to marked dietary soy consumption. Soy products, particularly those consumed by our patient, contain high levels of phytoestrogens which have been documented in limited case studies to contribute to abnormal development of breast tissue in adolescent and adult males. To our knowledge, this is the first documented case of gynecomastia occurring in a prepubescent patient resulting from excessive intake of dietary soy. Importantly, we also report a complete resolution of gynecomastia upon exclusion of dietary products containing significant amounts of soy. While soybeans and soy-derived products can be an important source of nutrition for some, those with abnormal sensitivity to phytoestrogens may benefit from limiting dietary soy consumption to avoid potential adverse effects, including gynecomastia.

This research was aimed to isolate the Gram-positive bacteria that contaminated milk processed with ultra high temperature (UHT) at 6 and 3 months before the date expires. A number of 3 different UHT milk products at 6 and 3 months before expire date were used as sample. Milk samples were taken from several supermarket in Banda Aceh. The identification was conducted using Gram staining method. One ml milk sample was transferred into 18 tubes containing nutrient broth (NB) media and incubated at 37 C for 24 hrs. The sample was then culture on 18 petri discs containing manitol salt agar (MSA) and 18 petri discs containing trypticase soy sucrose bacitracin (TYS20B) using streak plate method. After 2 days, bacterial colonies that grow were identified by Gram staining. The results showed that in UHT processed milk at 6 and 3 prior to expiration contaminated with Gram positive cocci bacteria, which comprised of Staphylococcus aureus and Streptococcus mutant. As conclusion, UHT processed milk at 6 and 3 prior to expiration is contaminated by Gram positive cocci bacteria.

AbstractRecurrent esophageal stenosis secondary to caustic ingestion may be challenging to treat. Self-expandable esophageal stents may be an alternative to repetitive endoscopic esophageal dilatation. We report a case of a 2-year-old male child with an extensive esophageal caustic stricture successfully treated using a combination of endoscopic dilatation and stenting. After 5 months of serial balloon dilatations, three nitinol internal silicone covered self-expandable stents were placed through the patient’s gastrostomy spanning the entire esophagus. The stents were positioned using a combination of both endoscopic and fluoroscopic guidance. The procedure was repeated with only one stent 3 months later. A new stricture in the proximal esophagus needed surgical resection and anastomosis, followed by two pneumatic dilatations with progressively longer asymptomatic intervals. The results are promising with the patient able to use his own esophagus; however, this is a single case and optimal stent standing time is still to be determined.

Gram staining is one of the first techniques used for the identification of group B Streptococcus agalactiae and one would expect to see gram-positive cocci under the microscope. The technique consists of applying a series of colorants and bleaches (acetone), which interact with the lipids of the membranes of gram-positive and gram-negative bacteria. The cell walls of gram-positive organisms retain the dye after acetone treatment and appear purple in color, whereas gram-negative organisms become discolored after acetone treatment and appear pink.

AbstractPostpartum takotsubo cardiomyopathy is a rare phenomenon caused by stress-induced, elevated levels of catecholamines and vasoconstrictive agents or by the administration of vasoconstrictive drugs such as bromocriptine and ergotrates. The condition has only been described in the past 20 years. The usual clinical course is complete resolution, although maternal mortality is estimated at 1%. An equally rare condition is postpartum reversible cerebral vasoconstriction syndrome. Like postpartum takotsubo cardiomyopathy, the etiology of postpartum cerebral vasoconstriction syndrome is thought to involve stress-induced, elevated levels of catecholamines and vasoconstrictive agents. Also, like postpartum takotsubo cardiomyopathy, the usual clinical course is complete resolution. This is the first reported case where both of these rare conditions occurred in the same patient after spontaneous vaginal delivery.
