AbstractObjectives:To examine the reasons why manic-depressive patients often fail to respond to lithium and, the chief purpose of this paper, to review the present state of knowledge concerning alternative pharmacotherapies to lithium in this disorder.Method:Literature review.Findings:Lithium produces a clinically useful effect in 70-80% of cases. Factors such as non-compliance, side-effects, serum levels, and enduring psychosocial influences on the patient must be considered before treatment failure is diagnosed. Carbamazepine is the most studied alternative drug to date, and the most likely predictors of response to this agent are rapid-cycling, absence of a family history of affective disorder, mania, severe illness, and non-response to lithium.Conclusions:In view of the 20-25% of patients who do not respond to lithium, and the increased likelihood that lithium-responders may fail to respond again once the drug has been stopped for a period, further research is needed to find better and safer prophylactics against depression in bipolar disorders and to refine our knowledge of the indications for use of those already available.

Intermittency, in which the normalized weight of large fluctuations grows for increasingly longer statistical samples, is seen as irregular bursting activity in time and is characteristic of the behavior of many brain and behavioral systems. This pattern has been related to the brain-stabilizing interplay of the general mechanisms of silence-evoked sensitization and activity-evoked desensitization, which can be found at most levels of neurobiological function and which vary more smoothly and at much longer times than the phasic observables. We use both the global Mandelbrot-Hurst exponent and the distribution of local Mandelbrot-Hurst exponents, in combination with dynamical entropies, to quantitate the property of nonuniform persistence which we treat as both deterministically expansive and statistically diffusive. For example, varying the parameter of the one-dimensional, Manneville-Pomeau intermittency map generated time series which demonstrated systematic changes in these statistical indices of persistence. Relatively small doses of cocaine administered to pregnant rats increased statistical indices of expansiveness and persistence in fetal motor behavior. These techniques also model and characterize a breakdown of statistical scaling in 72-hour time series of the amount of motor activity in some hospitalized manic-depressive patients.

The Eysenck Personality Inventory was used as a mood indicator instead of as a trait test with a group of manic-depressive patients associated with a research ward. Some patients completed the EPI while in a normal state, some while in a depressed or manic state, and some in two or more clinically defined states. Results showed that depression was associated with a significant decrease in extraversion and a significant increase in neuroticism. The manic state however, was not clearly differentiated from the normal one. It appears that the inventory may be a useful indicator of depressive mood.

SYNOPSISA group of bipolar manic depressive patients attending a routine lithium clinic were investigated. The results suggest that, when on treatment with lithium, manic depressive patients with a good prognosis tend to have a higher erythrocyte Na-K ATPase and higher plasma and erythrocyte lithium concentrations than those with a poor prognosis. There was no evidence to suggest that the erythrocyte: plasma lithium ratio was useful in predicting clinical response to lithium therapy. There was also a positive correlation between plasma lithium concentration and Na–K ATPase activity, confirming that in manic depressive subjects lithium produces a rise in erythrocyte Na–K ATPase activity.

ABSTRACT Ninety-six manic-depressive patients were studied during long-term lithium treatment. Highly significant elevations were observed respecting the levels of serum immunoreactive parathyroid hormone (P < 0.001) as well as the protein-corrected levels of serum calcium (P < 0.001) and serum magnesium (P <0.001), thus indicating a state of 'primary' hyperparathyroidism. The patients as a group had normophosphataemia and normophosphatasia supporting the impression of a rather mild state of biochemical hyperparathyroidism.
