Association between Hypouricemia and Reduced Kidney Function: A Cross-Sectional Population-Based Study in Japan

Background: Hypouricemia, conventionally defined as a serum uric acid level of ≤2 mg/dl, is considered a biochemical disorder with no clinical significance. However, individuals with renal hypouricemia have a high risk of urolithiasis and exercise-induced acute kidney injury, both of which are risk factors for reduced kidney function. Methods: To test the hypothesis that individuals with hypouricemia would be at a higher risk of reduced kidney function, we conducted a population-based cross-sectional study using data from the Specific Health Checkups and Guidance System in Japan. Logistic analysis was used to examine the relationship between hypouricemia and reduced kidney function, defined as estimated glomerular filtration rate <60 ml/min/1.73 m2. Results: Among 90,710 men (mean age, 63.8 years) and 136,935 women (63.7 years), 193 (0.2%) and 540 (0.4%) were identified as having hypouricemia, respectively. The prevalence of hypouricemia decreased with age in women (p for trend <0.001), but not in men (p for trend = 0.24). Hypouricemia was associated with reduced kidney function in men (odds ratio, 1.83; 95% confidence interval, 1.23-2.74), but not in women (0.61; 0.43-0.86), relative to the reference category (i.e., serum uric acid levels of 4.1-5.0 mg/dl) after adjusting for age, drinking, smoking, diabetes, hypertension, hypercholesterolemia, obesity, and history of renal failure. Sensitivity analyses stratified by diabetic status yielded similar results. Conclusions: This study is the first to provide evidence that hypouricemia is associated with reduced kidney function in men. Further research will be needed to determine the long-term prognosis of individuals with hypouricemia.

This observational, cross-sectional, epidemiological analysis investigated relationships of kidney function to urine calcium and other variables. The analyses targeted two population-based samples of adults (Gubbio study and Moli-sani study: n = 3508 and 955, respectively). Kidney function was assessed as estimated glomerular filtration rate (eGFR). Calcium/creatinine ratio (Ca/Cr) was used as index of urinary calcium in timed overnight urine under fed condition (Gubbio study), morning urine after overnight fast (Gubbio study), and first-void morning urine (Moli-sani study). Moli-sani study included also data for glomerular filtered calcium load, tubular calcium handling, and serum phosphorus, parathyroid hormone, 1,25-dihydroxyvitamin D, calcium, and 25-hydroxyvitamin D. eGFR positively and independently related to Ca/Cr (p < 0.001). In multivariate analyses, eGFR lower by 10 mL/min × 1.73 m2 related to overnight urine Ca/Cr lower by 14.0 mg/g in men and 17.8 mg/g in women, to morning urine Ca/Cr lower by 9.3 mg/g in men and 11.2 mg/g in women, and to first-void urine Ca/Cr lower by 7.7 mg/g in men and 9.6 mg/g in women (p < 0.001). eGFR independently related to glomerular filtered calcium load (p < 0.001) and did not relate to tubular calcium handling (p ≥ 0.35). In reduced eGFR only (<90 mL/min × 1.73 m2), low urine Ca/Cr independently related to low serum 1,25-dihydroxyvitamin D (p = 0.002) and did not relate to hyperphosphatemia, high serum parathyroid hormone, or hypocalcemia (p ≥ 0.14). Population-based data indicated consistent associations of lower kidney function with lower urine calcium due to reduction in glomerular filtered calcium. In reduced kidney function, relative hypocalciuria associated with higher prevalence of low serum 1,25-dihydroxyvitamin D.

Background: The association of hyperuricemia with various cardiovascular risk factors has often led to the debate of whether hyperuricemia is an independent risk factor for essential hypertension (HTN).  The current study was conducted to see the relationship between serum uric acid and essential HTN in Bangladeshi adults.Methods: In this cross-sectional study, conducted in a tertiary hospital of Bangladesh, 155 patients with essential hypertension (newly detected or on treatment) aged ≥18 years and 100 age-sex matched normotensive subjects were investigated. Serum uric acid, plasma glucose, serum creatinine, and lipid profile were measured in all in fasting samples.Results: The frequency of hyperuricemia was higher in the hypertensive group in comparison to the normotensive control group (29.7% vs. 6.0%, p<0.001). Serum uric acid level was higher in the hypertensive subjects than the controls (6.10±0.88 vs. 5.38±0.54 mg/dL, mean±SD, p<0.001). In the hypertensive group, subjects with stage II HTN had higher serum uric acid than those with stage I HTN (6.46±0.83 vs. 5.72±0.78 mg/dL, mean±SD, p<0.001). In the hypertensive group, uric acid level showed significant positive correlations with both systolic and diastolic blood pressure though in the control group uric acid showed such correlation with systolic BP only.Conclusion: Patients with essential hypertension had higher serum uric acid compared to normotensive controls; uric acid level showed positive correlations with systolic and diastolic BP in the hypertensive subjects.

Background: Serum uric acid is a relatively insoluble metabolite of purine metalolism which is mainly secreted by the kidneys and the rate is dependant on renal blood flow . It increases in preeclampsia and studies show it may be marker of severe preeclampsia. Methods and materials: This cross sectional study was conducted in Dhaka Medical College Hospital in eclampsia ward of gynae & Obs department during the period from January 2010 to December 2011 with the objective to determine the serum uric acid level in preeclampsia, to compare the serum uric acid level between mild and sever preeclampsia and to find out any relationship of the serum uric acid level with hypertension in preeclampsia. The study group composed of 92 diagnosed case of preeclampsia patient. Out of the 92 cases, 42 had mild preeclampsia (group-A) and 50 had severe preeclampsia (group B). Result: The results of the study showed that the mean serum uric acid level was significantly higher in severe preeclampsia (6.91+1.02) compared to mild preeclampsia (4.99 + 0.80) and there is a positive and significant relationship of serum uric acid level with severity of hypertension in preeclampsia (p<.0001). Background: Heart failure (HF) is a global pandemic and lays an added burden on public healthcare. Previous studies indicated that high and low serum uric acid levels are associated with worse outcomes in many diseases. Reduced serum uric acid may not result in a better outcome.Methods: A comparative, matched cross-sectional study design was implemented. The matching variables were age, sex, BMI, BP, and histories of CKD, CVD, diabetes mellitus, stroke, hyperlipidemia. We reviewed the electronic medical records to identify patients diagnosed with hypertension or hypertensive heart failure (HHF) admitted to Beijing Hospital's cardiology department.Results: The median age of the two groups after matching are 71. There are 55.6% males in the hypertension group and 53.8% in the heart failure group. Univariate logistic regression analysis showed that UA's quadratic term is significant (OR = 1.00, 95% CI 1.00 to 1.00; P = 0.03), which indicated a u-shaped relationship between hypertension and HHF. FBS (OR = 0.22, 95% CI 0.05 to 0.95, p = 0.07) and HDL (OR = 1.23, 95% CI 1.00 to 1.54, P = 0.05) were not significant but close.Conclusion: Our results supported that both low and high uric acid levels were predictive of hypertensive heart failure. Besides, high-density lipoprotein cholesterol and fasting blood sugar were also associated with hypertensive heart failure. Low-density lipoprotein cholesterol was not associated with hypertensive heart failure.

Background: Uric acid is one of natural antioxidants in human body. There have been several studies on the correlation between uric acid with oxidative stress and osteoporosis. However, the data are insufficient and results are controversial. In this regard, we determined the association between uric acid levels and bone mineral density (BMD) during the postmenopausal period. Methods: We analyzed data from 328 postmenopausal women (mean age, 57.3 ± 6.5 years; mean serum uric acid level, 4.6 ± 1.0 mg/dL). The participants were divided into three groups based on tertiles of the serum uric acid level. The participants receiving hormone replacement therapy (HRT), bisphosphonates, or lipid-lowering agents were included. Results: Blood urea nitrogen, serum creatinine, and serum triglyceride levels were significantly higher in the upper tertiles of uric acid levels. No significant difference was found in the mean uric acid levels between medication users and non-users. Each HRT regimen had a different mean serum uric acid level. A cross-sectional analysis showed no significant correlation between the serum uric acid levels and BMD in the spine and femoral neck (spine BMD: 1.050 ± 0.131, 1.060 ± 0.160, 1.084 ± 0.140, p = 0.22; femoral neck BMD: 0.837 ± 0.110, 0.849 ± 0.096, 0.863 ± 0.115, p = 0.28 for each tertile of uric acid). Longitudinal analysis of data from 186 women with follow-up examinations at a mean interval of 14.6 months also revealed no difference in reduction in both spine and femoral neck BMD between tertile groups of serum uric acid (the median BMD reduction for spine: −0.02, 0.01, −0.04, p = 0.95; the median BMD reduction for femoral neck: 0.008, 0.005, −0.003, p = 0.34). Conclusions: Serum uric acid level is not associated with BMD in postmenopausal women.

Tea is a common drink that consumed in Indonesia. Caffein is one of the ingredients of tea. coffee has been shown to increase urinary excretion which is also likely to increase uric acid excretion. Gout is a disease cause of high level of uric acid in blood stream.The aim of this study was to determine the tea drinkers influences serum uric acid level in the residents of Al Manshuurin Mosque Yabansai Waena Jayapura. We enrolled 45 participants in this cross sectional study. An assessment of various dietary intake amounts of substances such as tea was performed using a food frequency questionnaire. The content of tea (15 mg/cup) intake information from the past year. Descriptive and Chi square analysis were applied to identify any association of dietary intake with serum uric acid levels or the risk of gout arthritis. The results showed that the tea drinkers had a lower risk than the not tea drinkers (p= 0.01 respectively). The conclusion of this study is tea drinkers have lower uric acid levels in  blood.  Key words: Gout, Tea drinkers, uric acid
