Migraine course is influenced by female reproductive milestones, including menstruation and perimenopause; menstrual migraine (MM) represents a distinct clinical entity. Increased susceptibility to migraine during menstruation and in perimenopause is probably due to fluctuations in estrogen levels. The present review provides suggestions for the treatment of MM and perimenopausal migraine. MM is characterized by long, severe, and poorly treatable headaches, for which the use of long-acting triptans and/or combined treatment with triptans and common analgesics is advisable. Short-term prophylaxis with triptans and/or estrogen treatment is another viable option in women with regular menstrual cycles or treated with combined hormonal contraceptives; conventional prevention may also be considered depending on the attack-related disability and the presence of attacks unrelated to menstruation. In women with perimenopausal migraine, hormonal treatments should aim at avoiding estrogen fluctuations. Future research on migraine treatments will benefit from the ascertainment of the interplay between female sex hormones and the mechanisms of migraine pathogenesis, including the calcitonin gene-related peptide pathway.

Glyceryl trinitrate (GTN – nitroglycerin) is one of the oldest synthetic drugs still in widespread use. Sublingual tablets have been used for over a century for rapid, short-term prophylaxis and relief of angina pectoris. More recently three sublingual sprays have been marketed – Nitrolingual (Lipha), Coro-Nitro (Boehringer Mannheim) and Glytrin (Sanofi Winthrop). Advantages claimed for the spray formulation include greater convenience, a more rapid action and a longer storage life. How important are these properties in practice?

Orgasmic headache (headache associated with sexual activity type 2 according to the International Headache Society classification) is a sudden severe headache which occurs at orgasm. Experiences with triptan therapy are described. Two out of four patients with severe headache continuing for >2 h had a positive response to acute triptan therapy. Two out of three patients using triptans as short-term prophylaxis reported a reliable response on several occasions. Trip- tans might be a treatment option to shorten orgasmic headache attacks after the diagnosis is clear and, particularly, subarachnoid haemorrhage has been excluded. In patients who chose to predict their sexual activity, short-term prophylaxis with oral triptans 30 min before sexual activity might be a therapeutic option in those not responsive to or not tolerating indomethacin.

ABSTRACT The effect of addition of carboxymethyl cellulose, gelatine, polyphloretin phosphate and zinc hydroxide to corticotrophin was studied by means of fluorimetric plasma corticosteroid determinations in man and rats. The plasma corticosteroid level was stabilized at a low level by the administration of dexamethasone. Two days after a 3 day period of dexamethasone, 2 mg × 4 orally, the volunteers had a normal diurnal rhythm of plasma corticosteroid level indicating that short term dexamethasone treatment may be given without any serious risks. For each type of experiment, the same corticotrophin was employed to avoid dose differences. Each subject was injected with each preparation at weekly intervals. Significant differences between individuals were observed. The effect of polyphloretin phosphate exceeded that of gelatine, both substances significantly exceeding that of carboxymethyl cellulose and zinc hydroxide 8 hours after the injection and subsequently. Similar results were obtained in animal experiments.
