Purpose. To compare central corneal thickness (CCT), aqueous depth (AQD), and anterior chamber depth (ACD) measurements using the swept-source (CASIA SS-1000, Tomey, Japan) and time-domain (Visante, Carl Zeiss Meditec, USA) anterior segment optical coherence tomographers (OCT) in normal eyes. Methods. Sixty-eight eyes of 68 subjects were included. Three consecutive scans of each subject were obtained using both devices in a random order by one experienced operator. Standard deviation (Sw), coefficient of repeatability (CoR), coefficients of variation (CoV), and intraclass correlation coefficients (ICC) were used to evaluate the intraoperator repeatability. Agreement was assessed using the Bland–Altman plots and 95% limits of agreement (LoA). Results. All measurements of the swept-source OCT (SS-OCT) and time-domain OCT (TD-OCT) showed high repeatability with low CoR (CCT: 2.34 μm and 6.16 μm; AQD: 0.05 mm and 0.09 mm; ACD: 0.06 mm and 0.09 mm), low CoV (CCT: 0.16% and 0.42%; AQD: 0.61% and 0.97%; ACD: 0.53% and 0.83%), and high ICC (>0.98). The mean CCT with SS-OCT was slightly thicker than the results with TD-OCT (difference = 4.55 ± 2.62 μm, P<0.001). There was no statistically significant difference in AQD or ACD measurements between the two devices (0.01 ± 0.05 mm, P=0.111; 0.02 ± 0.05 mm, P=0.022, respectively). The 95% LoA between the SS-OCT and TD-OCT were −0.59 to 9.69 μm for CCT, −0.10 to 0.12 mm for AQD, and −0.09 to 0.12 mm for ACD. Conclusions. High levels of repeatability and agreement were found between the two devices for all three parameters, suggesting interchangeability. SS-OCT demonstrated superior repeatability compared with TD-OCT.

Abstract Purpose: To determine patient preoperative anatomical features and the parameters of implantable collamer lenses (ICLs) relevant in explaining vault variability.Setting: Ophthalmology Xi’an Aier Gucheng Eye hospital , Xi’an China.Design: Retrospective case series.Methods:This study comprised 88 eyes of 45 patients implanted with myopic or toric ICLs between May 2021 and August 2021.Pentacam imaging was used for assessing white-to-white (WTW) diameter, central keratometry, anterior chamber depth(ACD), central corneal thickness and vault. Anterior-segment optical coherence tomography(AS-OCT) was used to measure the horizontal anterior chamber angle distance (ATA). Ultrasound biomicroscopy (UBM) was used to measure horizontal and vertical sulcus to sulcus(vSTS). According to different ACD,we divide ACD into shallow group(2.8 to 3.2 mm),medium group(3.2 to 3.5 mm),and deep group(>3.5mm).Results: Linear regression analysis found significant correlation between WTW diameters and ATA diameters( y= 0.9605x+0.1491,R² = 0.9148),with a coefficient of determinant of 0.9148 (P<0.01). Linear regression analysis found significant correlation between hSTS and vSTS(y=0.9855x-0.0178,R²=0.1979),with a coefficient of determinant of 0.1979 (P<0.01). WTW diameters showed statistically significant difference in shallow and medium ACD (t=-3.28,P<0.01 ), significant difference in shallow and deep ACD (t=-4.69,P<0.01 ),there was no correlation in medium and deep ACD(t=-1.41,P>0.05 ). There was a statistically significant correlation between WTW diameters and hSTS diameters when the ACD was less than 3.5 mm (t=-0.451, P=0.000) (t=1.406, P=0.026),but weak correlation when the ACD was bigger than 3.5 mm(t=1.594, P=0.051). ATA and WTW with a mean difference close to zero(-0.66 mm). Despite the relatively high correlation (intraclass correlation co-efficient =0.689), the range of agreement is quite broad (1.33 mm).Conclusions:ATA measured with AS-OCT can not be used interchangeably with WTW obtained with Pentacam. WTW diameters and hSTS diameters have a statistically significant correlation when the ACD was less than 3.5 mm. WTW, ATA, hSTS and vSTS all should be considered during design the size of lens and regulating the vault after surgery.

Abstract Purpose To compare anterior chamber depth (ACD) and central corneal thickness (CCT) measurements by anterior segment optical coherence tomography (AS-OCT), Lenstar and ultrasound biomicroscopy (UBM). Methods A retrospective cross section study. A total of 83 eyes from 54 patients were included. Central corneal thickness (CCT) and anterior chamber depth (ACD) values obtained from three devices (AS-OCT, Lenstar and UBM) were recorded. All patients were collected from Oct. 2015 to Aug. 2016 in the Fourth Affiliated Hospital of China Medical University. Differences of these three devices (ACD and CCT) were statistically analyzed by randomized blocks analysis. Pearson correlative analysis and Bland-Altman analysis were used to compare the correlation and agreement. Results There’s no significant difference in measuring ACD by Lenstar and UBM (P>0.05), but there were significant differences in other two groups (AS-OCT and UBM, AS-OCT and Lenstar) (P<0.05). There were significant differences in the mean CCT measured by the three devices (P<0.05). There were high correlation in measuring ACD (all P<0.001, r1=0.928,r2=0.982,r3=0.932, respectively), and CCT(all P<0.001,r1=0.957,r2=0.949,r3=0.928, respectively). Conclusion AS-OCT, Lenstar and UBM yielded comparable results and are all can be used interchangeably for anterior segment measurements.

Objective. To compare fourier-domain optical coherence tomography (FD-OCT) and time-domain OCT (TD-OCT) in the determination of thinnest corneal thickness (TCT).Methods. This study included 55 keratoconus patients and 50 healthy volunteers. The RTVue-OCT (FD-OCT) and Visante-OCT (TD-OCT) were used for the measurement of the TCT. Three consecutive scans were performed. The comparison and agreement between the two modalities were analyzed by pairedt-test, the Pearson correlation, intraclass correlation coefficient (ICC), and Bland-Altman plots. intraobserver repeatability was analyzed by the intraobserver within-subject standard deviation (S(w)), coefficient of variation (CV(w)), and ICC.Results. The TCT value of normal corneas was higher by RTVue-OCT (530.4 ± 19.7 μm) than Visante-OCT (521.5 ± 18.3 μm) (p<0.001). For keratoconus eyes, the TCT was 425.0 ± 58.2 μm and 424.4 ± 55.7 μm (difference being 0.6 ± 10.2 μm,p=0.604). Strong correlation (r= 0.938∼0.985) (ICC = 0.915–0.984) was observed between the two OCTs, and each OCT exhibited excellent repeatability in determining the TCT in all subjects (ICC = 0.984–0.994).Conclusions. The values of TCT obtained from RTVue-OCT and Visante-OCT were highly correlated; however, the two values were different. Both OCT instruments exhibited good intraobserver reliability. The existence of systematic differences suggested that the two instruments cannot be used interchangeably.

Purpose. To evaluate anterior segment’s structures by Pentacam in patients with tilted disc syndrome (TDS).Methods. Group 1 included forty-six eyes of forty-six patients who have the TDS. Group 2 including forty-six eyes of forty-six cases was the control group which was equal to the study group in age, gender, and refraction. A complete ophthalmic examination was performed in both groups. All cases were evaluated by Pentacam. The axial length (AL) of eyes was measured by ultrasound. Quantitative data obtained from these measurements were compared between two groups.Results. There was no statistically significant difference for age, gender, axial length, and spherical equivalent measurements between two groups (p=0.625,p=0.830,p=0.234, andp=0.850). There was a statistically significant difference for central corneal thickness (CCT), corneal volume (CV), anterior chamber angle (ACA), and pupil size measurements between two groups (p=0.001,p=0.0001,p=0.003, andp=0.001). Also, there was no statistically significant difference for anterior chamber depth (ACD), anterior chamber volume (ACV), and lens thickness (LT) measurements between two groups (p=0.130,p=0.910, andp=0.057).Conclusion. We determined that CCT was thinner, CV was less, and ACA was narrower in patients with TDS. There are some changes in the anterior segment of the eyes with tilted disc.

Abstract Background To assess the changes of anterior chamber angle in patients with shallow anterior chamber and normal anterior chamber after phacoemulsification and intraocular lens implantation (IOL) using anterior segment swept-source optical coherence tomography (AS-SS-OCT). Methods This was a prospective case control study; 60 eyes of 60 patients who underwent cataract surgery. Based on anterior chamber depth (ACD) and gonioscopy findings, the eyes were classified into two groups, shallow anterior chamber group (30 eyes) and normal anterior chamber group (30 eyes). AS-SS-OCT was used to measure ACD and angle parameters angle opening distance (AOD), angle recess area (ARA), trabecular iris space area(TISA), and trabecular iris angle (TIA). Serial changes in each group were measured before and 3 months after phacoemulsification and IOL, and the differences between the two groups were compared. Results All angle parameters including ACD, AOD, ARA, TISA, and TIA in both groups at 3 months after cataract surgery were significantly different from the preoperative values (p<0.01). In addition, we found significant differences of all angle parameters between groups of shallow anterior chamber (SAC) and normal anterior chamber (NAC) (All P< 0.05). Besides, the TISA750 in superior quadrant and anterior chamber volume (ACV) were significantly smaller in the group of SAC than those in the group of NAC (P = 0.041 and 0.002, respectively). Conclusions The parameters of anterior chamber increased significantly in both two groups but not increase to the same extent 3 months after phacoemulsification. Iris thickening may also be one of the narrow angle factors,not just age-related lens thickening.

Abstract To determine the risk factors and unique characteristics of keratoconus (KC) progression after penetrating keratoplasty (PK), anterior segment optical coherence tomography parameters were statistically analyzed in comparison with eyes undergoing PK for other diseases as a control. Ninety-one eyes maintaining clear PK grafts for over 10 years were divided into 2 groups according to the primary indication for PK (KC vs Others groups). Corneal thinning indicators (inferior host thinnest corneal thickness/central corneal thickness [IHT/CCT], inferior graft thinnest corneal thickness/CCT [IGT/CCT]), were smaller whereas anterior chamber depth, and steepest corneal power (Ks), and maximum corneal power (Kmax) were larger in the KC group with statistical significance. Graft size, Kmax and Ks correlated with IHT/CCT and IGT/CCT in the KC group. These correlations were not detected in controls. Graft size and postoperative period were selected by multivariate regression analysis as factors for corneal ectatic changes in the KC group. In conclusion, KC eyes long after PK show inferior graft and host corneal thinning, and corneal protrusion. Corneal power parameters such as Kmax or Ks can be used to monitor KC progression after PK. A small graft might lead to KC progression after PK.

Abstract This study compared the test–retest variabilities and measurement agreement of anterior chamber angle (ACA) dimensions measured by two anterior segment swept-source optical coherence tomography (SS-OCT)—the ANTERION (Heidelberg Engineering, Heidelberg, Germany) and CASIAII (Tomey, Nagoya, Japan). Thirty-eight subjects, 18 patients with primary angle closure and 20 healthy participants with open angles, were included. The mean age was 54.7 ± 15.8 years (range: 26–75 years). One eye of each subject was randomly selected for anterior segment imaging by ANTERION and CASIAII, using the same scan pattern (6 evenly spaced radial scans across the anterior segment for three times) in the same visit. The between- and within-instrument agreement and repeatability coefficients of angle open distance (AOD500), trabecular-iris space area (TISA500), lens vault (LV), scleral spur-scleral spur distance (SSD), anterior chamber depth (ACD), and pupil diameter (PD) were measured. The anterior and posterior boundaries of the cornea, iris, and lens were automatically segmented by the SS-OCT instruments; the scleral spur was manually located by a single masked observer. There were significant differences between ANTERION and CASIAII measurements; the SSD, PD, and ACD were smaller whereas AOD500 and TISA500 were greater in ANTERION compared with CASIAII (P < 0.001). Anterior segment measurements obtained from the two SS-OCT instruments showed strong associations (R2 ranged between 0.866 and 0.998) although the between-instrument agreement was poor; the spans of 95% limits of between-instrument agreement were ≥ 1.5-folds than the within-instrument agreement for either instrument. Whereas both SS-OCT instruments showed low test–retest measurement variabilities, the repeatability coefficients of AOD500, TISA500, ACD, and PD were slightly smaller for CASIAII than ANTERION (P ≤ 0.012).

Purpose To determine the prevalence of trabecular-iris contact (TIC) and quantify this contact in healthy Caucasian individuals using Fourier-domain optical coherence tomography (FD-OCT). Methods For this cross-sectional study, 2,012 eyes of 1,006 healthy subjects were recruited among individuals undergoing a routine ophthalmologic checkup. In each participant, age, sex, intraocular pressure, and spherical refractive error were first recorded along with anterior chamber depth and volume, central corneal thickness measured with the Pentacam, and axial length with the IOLMaster. Anterior chamber angle variables and the presence of TIC in the horizontal quadrants were determined by anterior segment FD-OCT (RTVue 100®). When TIC was observed, TIC length (TICL) and its percentage in relation to the length of the trabecular meshwork (TICL percentage) were also measured. Results Trabecular-iris contact in the horizontal quadrants was observed in 34 eyes of 25 patients, representing 1.6% of the total number of eyes examined. In this subgroup of individuals, mean age was 55.8 years, 84% were women, and spherical refractive error ranged from -0.25 to 8.25 D. Eyes with TIC showed an angle width of less than 23.2 degrees and axial length shorter than 23.7 mm. Mean TICL was 239 ± 79 μm (103-495 μm) and mean TICL percentage was 46.9% ± 13.9% (17.2%-76.3%). Conclusions The prevalence of TIC was low in this population. Fourier-domain optical coherence tomography emerged as useful to assess its prevalence and quantify the extent of TIC.

AbstractThe aim of this study is to compare the scleral thickness of central serous chorioretinopathy (CSC) eyes with controls using anterior segment optical coherence tomography (AS OCT). This prospective case control study included 15 patients (15 eyes) with CSC and 15 age and gender matched healthy subjects. All subjects underwent spectral domain OCT with enhanced depth imaging and swept source AS OCT of temporal sclera. We investigated difference in scleral thickness between the two groups and relationship between choroidal and scleral thickness. Among the 15 eyes in the study group, 1 eye had acute CSC, 4 had recurrent CSC, 7 had inactive CSC, and 3 had chronic CSC. There was no significant difference in terms of age, gender, axial length and spherical equivalent between the two groups. The choroidal and scleral thickness of the study group were significantly greater than those of the control group (P < 0.001, P = 0.034). Choroidal thickness was positively correlated with scleral thickness (P = 0.031). A thick sclera along with a thick choroid were demonstrated in CSC eyes using AS OCT. Scleral characteristics might be involved in the pathogenesis of CSC by affecting outflow resistance of venous drainage in choroidal circulation.
