The knee joint, as the main lower limb motor joint, is the most vulnerable and susceptible joint. The knee injuries considerably impact the normal living ability and mental health of patients. Understanding the biomechanics of a normal and diseased knee joint is in urgent need for designing knee assistive devices and optimizing a rehabilitation exercise program. In this paper, we systematically searched electronic databases (from 2000 to November 2019) including ScienceDirect, Web of Science, PubMed, Google Scholar, and IEEE/IET Electronic Library for potentially relevant articles. After duplicates were removed and inclusion criteria applied to the titles, abstracts, and full text, 138 articles remained for review. The selected articles were divided into two groups to be analyzed. Firstly, the real movement of a normal knee joint and the normal knee biomechanics of four kinds of daily motions in the sagittal and coronal planes, which include normal walking, running, stair climbing, and sit-to-stand, were discussed and analyzed. Secondly, an overview of the current knowledge on the movement biomechanical effects of common knee musculoskeletal disorders and knee neurological disorders were provided. Finally, a discussion of the existing problems in the current studies and some recommendation for future research were presented. In general, this review reveals that there is no clear assessment about the biomechanics of normal and diseased knee joints at the current state of the art. The biomechanics properties could be significantly affected by knee musculoskeletal or neurological disorders. Deeper understanding of the biomechanics of the normal and diseased knee joint will still be an urgent need in the future.

The risk of knee injuries in freestyle skiing athletes that perform aerials is high. The internal stresses in the knee joints of these athletes cannot easily be directly measured. In order to ascertain the mechanical response of knee joints during the landing phase, and to explore the mechanism of damage to the cartilage and ligaments, a finite element model of the knee joint was established. Three successful landing conditions (neutral, backward, or forward landing) from a triple kicker were analyzed. The results demonstrate that the risk of cruciate ligament damage during a neutral landing was lowest. A forward landing carried medium risk, while backward landing was of highest risk. Backward and forward landing carried risk of injury to the anterior cruciate ligament (ACL) and posterior cruciate ligament (PCL), respectively. The magnitude of stress on the meniscus and cartilage varied for all three landing scenarios. Stress was largest during neutral landing and least in backward landing, while forward landing resulted in a medium level of stress. The results also provide the basis for training that is scientifically robust so as to reduce the risk of injury and assist in the development of a professional knee joint protector.

The knee joint is a complex biomechanical system, where the complexity of structure determines pathology variety, difficulties in treatment and rehabilitation. The conservative treatment possibilities of the ligamentous apparatus injuries of a knee are the subject of our discussion. Purpose of the study is to analyze the prevalence of knee traumatic injuries and modern ideas about the conservative treatment possibility of these injuries. Ligamentous apparatus injury of a knee joint is a common one, especially among athletes, with different mechanisms of knee injuries characteristic of different sports. Injuries of the lateral collateral ligament must be diagnosed in a timely manner in order to determine the possibilities of conservative and / or surgical treatment. Failure to recognize the injury details can lead instability and poor results to a knee after ligament reconstruction. Proponents of conservative treatment tactics provide evidence that non-operative treating of the ligaments also gives good results. The complex structure of a knee joint and a large number of variants of its damage impose a special responsibility on traumatologists-rehabilitologists, who are engaged in the restoration of its functions. The question of the choice between surgical restoration or conservative treatment of acute BCS injuries of knee joints remains open and depends on the specific situation, but in each case, rehabilitation measures are required, on the timeliness and adequacy of which the patient's quality of life will depend on.

The focus of this investigation is to study the mechanics of the human knee using a new method that integrates multi-body system and large deformation finite element algorithms. The major bones in the knee joint consisting of the femur, tibia, fibula are modeled as rigid bodies. The ligaments structures are modeled using the large deformation finite element Absolute Nodal Coordinate Formulation (ANCF) with an implementation of a Neo-Hookean constitutive model that allows for large deformations as experienced in knee flexation and rotation. The Neo-Hookean strain energy function used in this study takes into consideration the near incompressibility of the ligaments. The ANCF is used in the formulation of the algebraic equations that define the ligament/bone rigid connection. A unique feature of the ANCF is that it allows for the deformation of the ligament cross-section. At the ligament/bone connection, the ANCF is used to define a fully constrained joint. This aspect of the model reflects the actual structural mechanics of the knee. In addition, this model will reflect the fact that the geometry, placement and attachment of the two collateral ligaments (the LCL and MCL), are significantly different from what has been used in most knee models developed in previous investigations. The approach described in this paper will provide a more realistic model of the knee and thus more applicable to future research studies. The obtained preliminary results of other applications show that the ANCF can be an effective tool for modeling very flexible structures like ligaments subjected to large deformations. In the future, the ANCF models could assist in examining the mechanics of the knee to study knee injuries and possible prevention means, as well as an improved understanding of the role of each individual ligament in the diagnosis and assessment of disease states, aging and potential therapies.

Purpose: verification of reliability of MRI signs of damage to the structures of the knee joint in comparison with the data of arthroscopy knee joints.Materials and methods. 59 patients were examined (59 knee joints: 41 right (69.5%) and 18 left (30.5%)) with various knee injuries, aged 17 to 49 years (mean age 33.6 years). All patients underwent magnetic resonance imaging and medical diagnostic arthroscopy.Results. The sensitivity of the MRI to the diagnosis of meniscus ruptures was highest: 91.7% for the lateral meniscus and 95.4% for the medial meniscus. At the same time, in the diagnosis of anterior cruciate ligament ruptures, the MRI examination does not have such a high sensitivity (88.2%). In addition to the above features, MRI provides the ability to determine the damage to bone and cartilage structures. And also according to MRT it is possible to determine the plan of surgical intervention for tears of meniscuses and cruciate ligaments.Conclusion. The obtained data confirm sufficient efficiency of MRI for diagnostics of damages of knee joint structures. Further improvement of methods and increase in experience of interpretation of images will probably reduce amount of false positive and false-negative results in the future.

Background: Decalepis arayalpathra (J. Joseph and V. Chandras.) Venter is used primarily for nutrition besides its therapeutic values. Traditional preparations/formulations from its tuber are used as a vitalizer and blood purifier drink. The folklore medicinal uses cover inflammation, cough, wound healing, antipyretic, and digestive system management. A comprehensive review of the current understanding of the plant is required due to emerging concerns over its safety and efficacy. Objective: The systematic collection of the authentic information from different sources with the critical discussion is summarised in order to address various issues related to botanical identity, therapeutic medicine, nutritional usage, phytochemical, and pharmacological potentials of the D. arayalpathra. Current use of traditional systems of medicine can be used to expand future research opportunities. Materials and Methods: Available scripted information was collected manually, from peered review research papers and international databases viz. Science Direct, Google Scholar, SciFinder, Scopus, etc. The unpublished resources which were not available in database were collected through the classical books of ‘Ayurveda’ and ‘Siddha’ published in regional languages. The information from books, Ph.D. and MSc dissertations, conference papers and government reports were also collected. We thoroughly screened the scripted information of classical books, titles, abstracts, reports, and full-texts of the journals to establish the reliability of the content. Results: Tuber bearing vanilla like signature flavor is due to the presence of 2-hydroxy-4-methoxybenzaldehyde (HMB). Among five other species, Decalepis arayalpathra (DA) has come under the ‘critically endangered’ category, due to over-exploitation for traditional, therapeutic and cool drink use. The experimental studies proved that it possesses gastro-protective, anti-tumor, and antiinflammatory activities. Some efforts were also made to develop better therapeutics by logical modifications in 2-Hydroxy-4-methoxy-benzaldehyde, which is a major secondary metabolite of D. arayalpathra. ‘Amruthapala’ offers the enormous opportunity to develop herbal drink with health benefits like gastro-protective, anti-oxidant and anti-inflammatory actions. Results: The plant has the potential to generate the investigational new lead (IND) based on its major secondary metabolite i.e. 2-Hydroxy-4-methoxy-benzaldehyde. The present mini-review summarizes the current knowledge on Decalepis arayalpathra, covering its phytochemical diversity, biological potentials, strategies for its conservation, and intellectual property rights (IPR) status. Background: Osteoarthritis (OA) is a degenerative disease of joint cartilage affecting the elderly people around the world. Visualization and quantification of cartilage is very much essential for the assessment of OA and rehabilitation of the affected people. Magnetic Resonance Imaging (MRI) is the most widely used imaging modality in the treatment of knee joint diseases. But there are many challenges in proper visualization and quantification of articular cartilage using MRI. Volume rendering and 3D visualization can provide an overview of anatomy and disease condition of knee joint. In this work, cartilage is segmented from knee joint MRI, visualized in 3D using Volume of Interest (VOI) approach. Methods: Visualization of cartilage helps in the assessment of cartilage degradation in diseased knee joints. Cartilage thickness and volume were quantified using image processing techniques in OA affected knee joints. Statistical analysis is carried out on processed data set consisting of 110 of knee joints which include male (56) and female (54) of normal (22) and different stages of OA (88). The differences in thickness and volume of cartilage were observed in cartilage in groups based on age, gender and BMI in normal and progressive OA knee joints. Results: The results show that size and volume of cartilage are found to be significantly low in OA as compared to normal knee joints. The cartilage thickness and volume is significantly low for people with age 50 years and above and Body Mass Index (BMI) equal and greater than 25. Cartilage volume correlates with the progression of the disease and can be used for the evaluation of the response to therapies. Conclusion: The developed methods can be used as helping tool in the assessment of cartilage degradation in OA affected knee joint patients and treatment planning.

Conceptual frameworks for racial/ethnic health disparities are abundant, but many have received insufficient empirical attention. As a result, there are substantial gaps in scientific knowledge and a range of untested hypotheses. Particularly lacking is specificity in behavioral and biological mechanisms for such disparities and their underlying social determinants. Alongside lack of political will and public investment, insufficient clarity in mechanisms has stymied efforts to address racial health disparities. Capitalizing on emergent findings from the Midlife in the United States (MIDUS) study and other longitudinal studies of aging, this chapter evaluates research on health disparities between black and white US adults. Attention is given to candidate behavioral and biological mechanisms as precursors to group differences in morbidity and mortality and to environmental and sociocultural factors that may underlie these mechanisms. Future research topics are discussed, emphasizing those that offer promise with respect to illuminating practical solutions to racial/ethnic health disparities.

Abstract Background The knowledge about the experience of informal caregivers who provide care to people with moderate to advanced dementia in a domestic home setting is limited. A consequence of long hours of caregiving in addition to dealing with normal challenges of daily living is their experience of a poor quality of life. Some of their experiences may be described in terms of a feeling of powerlessness to make changes during care provision. This feeling may also suggest an experience of moral distress. The aim of this systematic review is to synthesise qualitative evidence relating to these experiences. Methods This review adopts a narrative synthesis approach. A search will be conducted for studies written in the English language in the bibliographic databases MEDLINE Complete, CINAHL, EMBASE, PsycINFO, Web of Science and Academic Search Complete covering periods from 1984 to present. Included studies will be qualitative or mixed-methods designs. The search terms will be related to dementia and caregivers, and the process will be focused on dementia at the moderate to the advanced stages within the domestic home setting. Reference lists of included papers will also be searched for additional relevant citations. Search terms and strategies will be checked by two independent reviewers. The identification of abstracts and full texts of studies will be done by the author, while the quality and the risk of bias will also be checked by the two independent reviewers. Discussion Psychological distress is cited as an experience reported within informal caregiving. For the caregiver, it is associated with a negative impact on general health. To date, no synthesis exists on the specific experience of informal caregiving for people with moderate to advanced dementia within the domestic home setting. This review considers that variation of accounts contributes to how the informal caregivers’ general experience is explored in future research. This may enable gaps in current knowledge to be highlighted within the wider context of caregiving in the domestic home setting. Systematic review registration This review is registered with PROSPERO (CRD42020183649).
