The aim of this study was to evaluate the effect of an oral care intervention program on the incidence of pneumonia and fever as a surrogate endpoint. In addition, we tried to determine the oral care risk factors for the incidence of fever. We provided an oral care program for the elderly at one private nursing home in July 2013. The maximum capacity of the nursing home was 60 residents. The body temperatures of all residents were measured twice a day and were summarized as the incidence of fever over a one-month period, which was used as the dependent variable. The residents’ life conditions, number of teeth, and prescribed diet were used as independent variables. The factors that affected the incidence of fever were the number of remaining teeth, a prescribed diet of sliced food, the meal care level, and the oralCandidalevels. These risk factors affected the incidence of fever independently or interactively with oral care. Some risk factors for the incidence of fever were enhanced by the oral care program. It is important to evaluate and control these factors before the implementation of an oral care program.

Background: Moving away from family can cause psychological and emotional trauma for the elderly. Accordingly, the present study aimed to evaluate the effectiveness of spiritual therapy on loneliness feeling among the elderly in a nursing home in Isfahan. Methods: The method was quasi-experimental with a pretest-posttest design and a control group. The population of this study included all of the elderly in Sadeghieh nursing home in Isfahan in 2018. The sample of this study included 30 elderly people being selected by purposive sampling and then randomly included in the experimental and control groups (15 elderly people in each group). The experimental group received the therapeutic interventions related to spiritual therapy at eight sessions of 90 minutes every week for two months. Then, the subjects in both groups were re-tested. The used instruments included Russell’s loneliness questionnaire (1996). After collecting the questionnaires and extracting the raw data, the data analysis was conducted using descriptive statistics and analysis of covariance by SPSS23 statistical software at 0.05 error level. Results: spiritual therapy had a significant effect on loneliness among the elderly in the nursing home (p < 0.001). In this regard, this therapy could reduce loneliness among the elderly in the nursing home. The effect of spiritual therapy on loneliness among the elderly in the nursing home was 53%. Conclusion: Based on the research findings, spiritual therapy can reduce loneliness among the elderly by the techniques such as taking responsibility, assigning cases to God, strengthening the intention, and deleting negative thoughts.

Összefoglaló. Bevezetés: Nagyszámú kutatás igazolta, hogy a dohányzás növeli a legjelentősebb krónikus betegségek kockázatát. Habár 2009 óta csökkenő tendenciát mutat Magyarországon a hagyományos dohányzók aránya, az e-cigarettát kipróbálóké az utóbbi években folyamatosan növekszik. Célkitűzés: A 2018-ban a felnőtt lakosság körében végzett Népegészségügyi Felmérés dohányzásra és e-cigaretta-használatra vonatkozó eredményeinek bemutatása az előző vizsgálatok tükrében. Módszer: A kérdőíves felmérésben 1586 fő került személyesen lekérdezésre. Az iteratív súlyozás a többlépcsős mintavételi designhatást és a 2016-os mikrocenzus adatait vette figyelembe. Eredmények: 2018-ban a dohányzók aránya a magyar felnőtt lakosság körében 28,7% (95% MT: 26,3–31,1%), az e-cigarettát használók aránya pedig 1,7% (95% MT: 1,1–2,5%) volt. Az iskolai végzettség a 65 év alattiak esetében a dohányzást befolyásoló tényező volt (EH: 3,32; 95% MT: 2,53–4,34), de a 65 éves és annál idősebb korcsoportban már nem (EH: 1,11; 95% MT: 0,59–2,09). Az e-cigarettát kipróbálók és használók között a leginkább említett (54,3% 95% MT: 44,0–64,5%) motivációs tényezőcsoport a dohányzásról való leszokással, az ártalomcsökkentéssel és a visszaesés megelőzésével volt kapcsolatos. A 65 éves és idősebb korcsoportban a dohányzók aránya 2015-höz képest emelkedett. 2018-ban az alapfokú iskolai végzettségűek körében volt a legmagasabb a dohányzók aránya, míg 2014-ben az érettségivel nem rendelkező középfokú végzettségűek körében. Következtetés: Bár összességében csökkent, az alacsony iskolai végzettségűek és az idősek körében emelkedett a dohányzók aránya Magyarországon. Az e-cigarettát kipróbálók és használók száma növekvő tendenciát mutat hazánkban. Orv Hetil. 2022; 163(1): 31–38. Summary. Introduction: The body of evidence suggests that smoking increases the risk of the most prevalent chronic diseases. Although the proportion of traditional smokers in Hungary has been on a declining trend since 2009, the proportion of those who tried e-cigarette has been steadily increasing in recent years. Objective: To present – in the light of previous studies – the results of the Public Health Survey among adults in 2018 on smoking and e-cigarette use. Method: 1586 persons were personally interviewed in a survey. The iterative weighting algorithm considered both the design effect of multistaged sampling and the 2016 Hungarian microcensus. Results: In 2018, the proportion of smokers in the Hungarian adult population was 28.7% (95% CI 26.3–31.1%), and the proportion of e-cigarette users was 1.7% (95% CI 1.1–2.5%). Educational level was a predictor of smoking among respondents younger than 65 years old (OR 3.32; 95% CI 2.53–4.34), but not for those aged 65 years or older (OR 1.11; 95% CI 0.59–2.09). Among e-cigarette ever or current users, the most commonly mentioned (54.3% 95% CI 44.0–64.5%) motivational factor-group to try or use e-cigarettes included motivations to quit smoking, to reduce harm, and to avoid relapsing. In the population aged 65 years old or older, the proportion of smokers increased compared to 2015. The proportion of smokers was the highest among those with primary education in 2018, while in 2014, it was the highest among those with secondary education without a graduation certificate. Conclusion: In Hungary, although overall smoking rates are declining, the smoking rate in the low educational group and among the elderly increased. The number of people trying or using e-cigarettes is showing an increasing trend in our country. Our results call for a complex public health intervention program including prevention and smoking cessation supporting subprograms with high focus on those with primary education. AbstractMethotrexate (MTX) is recommended as a first-line treatment for rheumatoid arthritis (RA). There are no strict guidelines regarding monitoring for liver damage in RA patients. This study aimed to evaluate noninvasive diagnostic procedures in assessing liver fibrosis in RA patients. Ninety-six RA patients were recruited for this study. The procollagen III N-terminal peptide (PIIINP) serum level was measured in all patients. The Enhanced Liver Fibrosis score (ELF-1) was calculated for 82 patients. Transient elastography (TE) was performed in 91 patients, those examined were divided into two groups: a study and control group, comprising patients with and without risk factors for liver fibrosis, respectively. The TE result correlated only with the body mass index—BMI (p < 0.05); there was no correlation with the cumulative MTX dose (p = 0.33). The TE result was significantly higher in those with risk factors for liver fibrosis than in those without risk factors (TE result >  = 7.1 kPa 28/42 vs 13/41, HR = 2.103, Mann–Whitney U test, approximately 0.02). There was a positive correlation between the PIIINP level and body weight (p = 0.028), cumulative MTX dose (p = 0.007), RA activity (p = 0.028) and diabetes mellitus (DM) (p = 0.001). There was a positive correlation between the ELF-1 score and age (p < 0.001), cumulative MTX dose (p = 0.007) and RA activity (p < 0.001). The PIIINP level and ELF-1 score are not organ specific, and readings may vary depending on RA activity. TE is organ specific and can be performed by a skilled ultrasonographer might be useful to assess actual liver condition.

Background: Hypertension is happened generally at someone who has age more than 40 year. The prevalence of hypertension was tend to increase along with the increasing of amount of the elderly. Changing of nutrition status and unfavorable nutrition intakes like is high fat, sodium and low micronutrients (potassium, calcium, magnesium) have an effect to incidence of essential hypertension. Retreating of biologic accompanying aging process sometimes make the elderly become resistance to get quality of intakes nutrition.Objective: The study was conducted to know relations between nutrition intakes and nutrition status with occurrence on essential hypertension of the elderly in the health center Curup and Perumnas, Rejang Lebong District.Method: The study was observational analytic with case control matched study. Subject of the study was divided into two group i.e. case and control with comparison case and control 1:1 which matched to age and sex. Total subject were 150, taken by simple random method. The cases were essential hypertension patients who had recently diagnosed by doctors. Blood pressure measurements were three times in supine position. Hypertension was defined as average of three times supine systolic/diastolic blood pressures at p >140/ 90 mmHg. Nutrition status was determined by Body Mass Index (BMI). Intake nutrients were collected using Food Frequency Questionnaire (FFQ) in the last 3 months by using food model. χ 2 McNemar were used to bivariate analytic and condition logistic regression to identify risk factors associated with essential hypertension.Result: This study showed that there were 4 variables significantly related with essential hypertension, i.e. intakes of sodium (OR: 3.43; CI 95%:1.09-10.77), potassium (OR: 0.24; CI 95%:0.07-0.84), stress (OR: 3.79; CI 95%:1.18-12.12), and obesity (OR: 4.57; CI 95%:1.49-13.95). The no significant correlation (p>0.05) identified for intakes of fat, calcium, magnesium, coffee consumption, smoking status and family history of essential hypertension.Conclusion: High intake of sodium, stress and obesity were the risk factors on occurrence of essential hypertension. In the other hand, high intake of potassium were reduce the risk on occurrence of essential hypertension (protective factor) in the health center Curup and Perumnas, Rejang Lebong District.

<p>A Hipertensão Arterial Sistêmica é uma condição clínica com múltiplas causas e é definida por níveis elevados e sustentados de pressão arterial, sendo considerada uma das principais doenças crônicas não transmissíveis que acometem a população idosa. Objetivou-se identificar os fatores de risco associados à hipertensão arterial em idosos. O presente trabalho trata-se de uma revisão integrativa, com busca realizada na Biblioteca Virtual em Saúde, por meio das bases de dados LILACS, BDENF e MEDLINE, associados ao repositório SciELO, entre os anos de 2010 e 2015, com a utilização dos descritores combinados e síntese dos dados por meio de tabulações. Foram evidenciados como fatores de risco à hipertensão arterial nos idosos: a própria senilidade, o sexo feminino, excesso de peso, pior autopercepção de saúde, não adesão ao tratamento, incapacidade funcional, depressão e a inatividade física. Os resultados mostram o papel dos fatores de riscos para o desenvolvimento e complicações da hipertensão, todavia também são sinais para a detecção precoce e para a ampliação das intervenções de medidas de prevenção e controle nas diversas esferas de atendimento em saúde.</p><p align="center"><strong><em>Risk factors associated with arterial hypertension in elderly</em></strong></p><pre><strong>Abstract: </strong>Systemic Arterial Hypertension is a clinical condition with multiple causes and is defined by elevated and sustained blood arterial pressure levels, being considered one of the main chronic non transmissible diseases that affect the elderly population. The aim of this study was to identify the risk factors associated with hypertension in the elderly. The present article deals with an integrative review, with a search carried out in the Virtual Health Library, through the LILACS, BDENF and MEDLINE databases, associated with the SciELO repository, between 2010 and 2015 years, using the descriptors combined and synthesis of the data by through tabulations. Senility, female gender, overweight, worse self-perception of health, non-adherence to treatment, functional disability, depression and physical inactivity were evidenced as risk factors for hypertension in the elderly. The results show the role of risk factors for the development and complications of hypertension, but they are also signals for the early detection and for the expansion of the interventions of prevention and control measures in the different health care spheres.</pre>

OBJECTIVE: To identify risk factors for dementia among the elderly in a rural area of Northeastern Brazil. METHOD: The subjects assessed were all 60 years old or older, and lived in a rural region of Bahia, a Northeastern State of Brazil. CAMDEX, a structured clinical evaluation protocol, was used for diagnosis, and applied at the home of the subjects. RESULTS: The risk factors identified were divided in accordance with socio-demographic characteristics, the presence of co-morbid conditions, and the use of medications. The variables with strong association with dementia were age, history of stroke, arterial hypertension, and sight impairment. CONCLUSION: Advanced age, arterial hypertension, and vascular brain injury were the main risk factors associated with dementia, which suggests that public health measures adopted to prevent and control modifiable risk factors can mitigate the prevalence of high rates of dementia.

To prevent an accident, it is necessary to notify caregivers if the elderly person is getting out of bed. We have previously developed a posture discrimination system based on body motions. Herein, we propose a discrimination method by using machine learning to improve the performance of the system. A purpose of this study is to evaluate the proposed method. Elderly people in a nursing home were chosen as subjects in this study. We analyzed the body motion data during bed rest and bed exit of the subjects using the proposed method. These results suggest that it is effective.
