Objectives. Evaluation of chondroitin sulfate (CS), as an early marker of aggrecan degradation, and chondroitin sulfate 846 epitope (CS846), as a biomarker of CS synthesis, is an attempt at answering the question whether the therapy used in juvenile idiopathic arthritis (JIA) patients contributes to the normalization of biochemical changes in aggrecan.Methods and Results. Serum levels of CS and CS846 as well as catalase (CT), superoxide dismutase (SOD), and glutathione peroxidase (GPx) activities in erythrocyte were assessed in patients before and after treatment. In the course of JIA, aggrecan metabolism is disturbed, which is reflected by a decrease (p<0.001) in CS serum level and an increase (p<0.05) in CS846 concentration. Furthermore, increased (p<0.001) activities of CT, SOD, and GPx in untreated JIA patients were recorded. The anti-inflammatory treatment resulted in the normalization of CS846 level and SOD and GPx activities. In untreated patients, we have revealed a significant correlation between serum CS and CS846, CT, CRP, ESR, MMP-3, and ADAMTS-4, respectively, as well as between CS846 and CT, GPx, CRP, ESR, and TGF-β1, respectively.Conclusion. The observed changes of CS and CS846 in JIA patients indicate a further need of the therapy continuation aimed at protecting a patient from a possible disability.

Background: Camel milk (CAM) regulates the inflammatory process, apoptotic pathways and oxidative stress. Thus, it is a therapeutic possibility for many autoimmune disorders, including autism. Vasoactive intestinal peptide (VIP) is an anti-inflammatory peptide that facilitates the immune regulatory functions by recruiting regulatory T cells to induce immune tolerance and prevent the occurrence of autoimmunity. This study aimed to investigate the effect of CAM consumption on both serum VIP levels and the severity of autism assessed by measuring the Childhood Autism Rating Scale (CARS). Mehods: Sixty-five autistic children, aged between 3-12 years, were studied. Forty-seven patients received 500 mL of CAM (either raw "24 patients" or boiled "23 patients") in their regular daily diet for two weeks. Eighteen patients received 500 mL of cow milk for two weeks. For all patients, CARS and serum VIP levels were assessed before and after milk consumption. Results: Although there was a decrease in CARS scores in patients who received raw CAM, this decrease was non-significant (P=0.070). Serum VIP levels were increased in patients who consumed CAM, either raw (P=0.076) or boiled (P=0.065), but this increase was non-significant.  Conclusions: CAM consumption for 2 weeks neither significantly decreased the severity of autism nor increased serum anti-inflammatory VIP levels. The lack of a significant response to CAM in this study may be attributable to the short duration of CAM consumption. Further studies are required to investigate the effect of CAM consumption for a longer duration on serum VIP levels and the severity of autism.

To assess the role of enzymatic antioxidants in the pathogenesis of protein energy malnutrition [PEM] and the effect of nutritional rehabilitation, we studied 30 infants with PEM [mean age 10.63 +/- 4.39 months: 10 marasmic; 8 with kwashiorkor; 12 with marasmic kwashiorkor] and 15 controls. All underwent clinical examination and laboratory investigations, including superoxide dismutase [SOD] and glutathione peroxidase [GPx] estimation before and after nutrition rehabilitation. SOD and GPx were significantly lower in all malnourished infants compared to controls, and significantly increased after nutritional rehabilitation. These significant correlations suggest that antioxidants could be introduced during PEM nutritional rehabilitation to decrease morbidity and mortality.

Background: Doxorubicin (DOX) is an effective antineoplastic drug indicated to treat many cancerous diseases but its clinical usefulness is limited by many side effects. The main and the most serious one is DOX induced cardiotoxicity. Many strategies have been tried to minimize this side effect such as addition of cardioprotective agent to DOX treatment protocols. Aims: The aim of this work was directed to investigate whether marine astaxanthin (ATX), a xanthophyll carotenoid pigment with potent antioxidant effect, could protect heart against the cardiotoxicity induced by DOX. Methodology: Forty Male Wister rats were divided into four equal groups and treated for one week as follow: Group I rats were treated with normal saline (2 ml/kg, x7, i.p.) and considered a control group. Group II rats were treated with ATX (40 mg/kg, x7, i.p.). Group III rats were treated with normal saline (2 ml/kg, x7, i.p.) and a single dose of DOX (20 mg/kg, i.p.) at day 7. Finally, group IV rats were treated with ATX (40 mg/kg, x7, i.p) and with a single dose of DOX (20 mg/kg, i.p.) at day 7. After 24 and 48 hrs of treatment, rats were anesthetized and prepared for collection of blood samples and heart isolation. The cardioprotective effect of ATX against DOX induced cardiotoxicity were evaluated by measurement of the serum level of cardiac enzymes CPK by colorimetric assay and CK-MB by Eliza. Also the levels of serum total antioxidant capacity (TAC) were measured colorimetrically. In addition, the Malondialdehyde (MDA), reduced glutathione, glutathione peroxidase (GPx) levels and superoxide dismutase (SOD) were determined in heart tissues homogenate by colorimetric method. In addition, Heart sample were taken for histopathology studies. Results: The Addition of ATX to DOX significantly (p<0.05) decreased the serum level of cardiac enzymes (CPK, CK-MB) and increased the serum total antioxidant capacity in compare with these levels in sera of rats treated with DOX only. This addition also significantly decreased the level of malondialdehyde and increased the reduced glutathione and glutathione peroxidase and superoxide dismutase significantly in the heart tissues homogenate in compare to corresponding levels in rats treated with DOX alone. Histopathological investigation of cardiac tissues confirmed the biochemical studies, where addition of ATX to DOX treatment protocol showed that the fragmentation of the muscle fiber revealed normal with central vesicular nuclei and prevented a marked disruption of normal cardiac architecture which resulted from DOX treatment. Conclusion: Marine astaxanthin provides excellent cardioprotective effect against doxorubicin induced cardiotoxicity in rats.

Objective: The production of reactive oxygen species in exercise causes oxidative stress which disturbs the balance of oxidants and antioxidants, causing destructive effects on cells. The present study aims to investigate the effect of three types of massage (Swedish, Russian, Thai) on serum levels of Malondialdehyde (MDA), Glutathione Peroxidase (GPX) and Superoxide Dismutase (SOD) following one session of exhaustive exercise. Methods: This quasi-experimental study was conducted on 48 female futsal players aged 17-22 years in Zahedan, Iran who were selected using a purposive sampling method, and randomly divided into four groups of Swedish massage (Long strokes with pressing and tapping using hands), Russian massage (Medium to high pressure), Thai massage (Pressure to certain parts of the body) and Control. The exercise program was based on Bruce protocol. Serum levels of MDA, GPX and SOD were measured by before and immediately after exercise and after massage. Data analysis was performed using repeated measures ANOVA, considering a significance level of P≤0.05. Results: In all three types of massage, there was a significant decrease in serum level of MDA (0.22±0.08), and a significant increase in GPX (1.84±0.46) and SOD (10.02±2.86) levels after exhaustive (P=0.001). No significant difference was observed in the control group. Conclusion: It seems that Russian, Thai, and Swedish types of massage can affect the serum levels of the MDA (as an oxidative stress marker) and the antioxidant enzymes of GPX and SOD during the post-exercise recovery period.

AbstractBackgroundInterleukin-33 (IL-33) and homocysteine (Hcy) were found to be up-regulated in patients with diabetic nephropathy (DN), and the present study aimed to investigate whether metformin (MT) can influence the serum levels of IL-33 and Hcy in patients with DN.MethodsSixty patients with type 2 diabetes mellitus (DM) were divided into DM group (albumin: Alb <20 mg/L), DN group (Alb >20mg/L), and DN+ MT treatment group, with 20 cases in each group. Patients in each group were treated with insulin for 3 months, and patients in DN+MT group was treated with insulin+MT for 3 months. The serum levels of IL-33, urinary microalbumin excretion rate (UAE), body mass index (BMI), total cholesterol (TC), high density lipoprotein cholesterol (HDL-C), creatinine (Cr), cystatin C (CysC) and Hcy were measured before and after medication. Twenty normal subjects were involved as control.ResultsBMI, Hcy and TC were reduced and HDL-C was increased of patients had been treated with metformin and insulin. UAE, Cr, Ccr and CysC had no differences before and after treatment. The serum level of IL-33 significantly up-regulated in patients with DN, and MT treatment significantly decreased the serum level of IL-33 in patients with DN.ConclusionMetformin could alleviate the symptom of patient with DN through decreasing the serum level of IL-33 and Hcy.

Background: Juvenile idiopathic arthritis (JIA) represents a heterogeneous group of autoimmune diseases that arises before the age of 16 years and lasts more than 6 months. During acute inflammation of the disease, serum copper concentration increases and zinc decreases, that could point to the possible pharmacological properties of these trace elements. Aim: To measure the serum level of zinc and copper in patients with juvenile idiopathic arthritis (JIA) with different subtypes and correlate the levels of zinc and copper with the disease activity. Methods: This cross-sectional study was done on 40 patients already diagnosed clinically with JIA; patients were followed-up at the Rheumatology Outpatient Clinic, Children's Hospital, Cairo University. Results: Out of forty patients, 16 were males (40%) and 24 were females (60%) with a male to female ratio (M: F) of 1:1.5. Out of the forty patients 17 were in activity and 23 were without activity. Thirty age and sex matched controls were included for comparison. Serum copper level was significantly higher in patients with JIA than those of the controls (P= 0.017) while there were no significant difference in serum level of zinc between JIA patients and that of the controls. Conclusion: Alteration of serum copper and zinc probably is a defense response against JIA; increased copper may be due to inflammation associated, these elements could serve as biomarkers for the disease activity.

Latar belakang: Inflamasi adenoid ditandai dengan ditemukannya pembesaran ukuran adenoid atau hipertrofi adenoid. Telah banyak dilakukan penelitian yang mendukung IL-6 merupakan dasar patofisiologi terjadinya hipertrofi adenoid sehingga dibutuhkan terapi yang dapat mengurangi peran IL-6. Saat ini, kortikosteroid intranasal menjadi pilihan terapi pada hipertrofi adenoid karena memiliki efek anti inflamasi. Terdapat penelitian mengenai manfaat penggunaan kortikosteroid intranasal terhadap penderita hipertrofi adenoid, dimana terjadi penurunan kadar IL-6, tetapi pengaruhnya terhadap kadar IL-6 serum tidak diteliti. Tujuan: Mengetahui pengaruh pemberian mometasone furoate semprot hidung terhadap kadar IL-6 serum pada penderita hipertrofi adenoid. Metode: One group pre and post test design ini melibatkan 16 subjek. Dilakukan pemeriksaan foto polos skull lateral soft tissue dan pengambilan darah tepi untuk menghitung kadar IL-6 serum. Diberikan terapi mometasone furoatesemprot hidung selama 6 minggu. Setelah 2 minggu dan 6 minggu dilakukan pengambilan darah tepi untuk menghitung kadar IL-6 serum, serta evaluasi foto polos skull lateral soft tissue setelah 6 minggu. Hasil:Terdapat perbedaan bermakna antara kadar IL-6 serum sebelum dan sesudah perlakuan baik, pada minggu kedua maupun minggu keenam (p=0.00). Terdapat perbedaan bermakna antara nilai foto polos skull lateral soft tissue sebelum dan sesudah perlakuan (p=0.00). Berdasarkan uji Pearson, nilai kadar IL-6 serum darah berkorelasi secara signifikan yang bersifat negatif (p<0,05) dengan foto polos skull lateral sebelum dan sesudah perlakuan dengan hasil -0,819 dan -0,692. Kesimpulan: Mometasone furoatesemprot hidung dapat menurunkan kadar IL-6 serum pada penderita hipertrofi adenoid secara bermakna, disertai dengan mengecilnya ukuran adenoid, penurunan gejala dan keluhan penderita hipertrofi adenoid. Kata kunci: Hipertrofi adenoid, Interleukin 6, Mometasone FuroateABSTRACT Background:Adenoid inflammation is characterized by adenoid hypertrophy (AH). Numerous studies had stated that Interleukin 6 (IL-6) is the pathophysiological basis for the occurrence of AH, thus, a therapy is needed to reduce the role of IL-6. Intranasal corticosteroid (IC) is the drug-of-choice for AH for its anti-inflammatory nature. Previous research had shown a decrease in IL-6 levels in adenoid tissue after IC administration, but its effect on IL-6 serum level had not yet been studied. Purpose: To find out the effect of Mometasone Furoate (MF) on IL-6 serum level in AH patients. Methods: One group pre and post test design, involving 16 subjects, underwent a soft tissue skull lateral plain x-ray, also a peripheral blood examination for IL-6 serum level. Afterwards, subjects were treated with MF nasal spray for 6 weeks. The IL-6 serum level was evaluated after 2 and 6 weeks, and the soft tissue x- ray was assessed after 6 weeks. Results: There were significant differences between IL-6 serum levels before and after treatment both in the second and sixth week (p = 0.00). There were significant differences between the value of soft tissue skull lateral plain x-ray before and after treatment (p = 0.00). Based on the Pearson test, IL-6 serum levels correlated significantly negatively (p <0.05) with soft tissue skull lateral plain x-ray before and after treatment with results -0.819 and -0.692. Conclusion: MF nasal spray significantly reduce IL-6 serum levels in AH patients, reducing adenoid size and decreasing symptoms of AH patients.
