The success of rehabilitation will not depend on just clinical procedures. A proper dental technique (ceramist) is required as well as the respect for some biomimetic principles to obtain the desired final result. This study has the purpose of describing a prosthetic rehabilitation with laminate veneers and all-ceramic crowns of a patient unsatisfied with a previous esthetic treatment because of the negligence of some biomimetic principles. A 45-year-old female patient was admitted to the dental clinic complaining about the lifelike appearance of her all-ceramic restorations. Before the fabrication of new restorations, a mock-up was conducted to verify the patient’s satisfaction. A ceramist conducted all the fabrication process so that surface characterizations could be visually verified and the lifelike appearance of natural tooth could be reproduced. After the cementation procedure, the patient reported being satisfied with the lifelike appearance of the new restorations. Based on the clinical findings of the present case report, it can be concluded that the reproduction of the lifelike esthetic appearance of natural teeth and the visualization of the final results before definitive procedures are essential to obtain the clinical success.

Digitalization has become part and parcel of contemporary prosthodontics with the probability of most of the procedures being based on the digital techniques in the near future. This digital revolution started in the latter half of the 20th century by converting analog objects/signals into digital bits and bytes. Recent developments in all-ceramic materials and systems of computer-aided designing and computer-aided manufacturing (CAD/CAM), copy milling, and so forth offer excellent esthetics and superb biocompatibility. Copy milling system for ceramics enables milling of the zirconia cores of all-ceramic restorations precisely and also if this system is properly used the procedure for fabricating all-ceramic restorations can be substantially simplified. This case report presents fabrication of all-ceramic Maryland Bridge and post-core with a copy milling system for esthetics and preservation of integrity of tooth. For both of the patients, the use of biologic, all-ceramic, copy-milled restorations resulted in clinical success and recovered function and esthetics.

ABSTRACT The use of porcelain laminate veneers to solve esthetic and/or functional problems has been shown to be a valid management option, especially in the anterior esthetic zone. In cases when patients decline orthodontic treatment, adhesively bonded porcelain veneers are a reliable treatment option to modify the appearance of tooth position and form, to close diastema or cervical embrasures, or to change the tooth shade. Porcelain veneers are one of the best corrective options available considering biological, functional, mechanical, and esthetic points of view. The present case report is about discolored anterior teeth in a healthy dentition by means of sectional porcelain veneers simply cemented onto the natural teeth and with minimal tooth preparation. Introdução: O restabelecimento de um sorriso harmônico constitui um grande desafio para a odontologia restauradora. A sincronia entre diversas especialidades se faz necessária para diagnosticar e tratar alterações estéticas, buscando a excelência do resultado e a satisfação do paciente. Um método bastante citado na literatura e aplicado por muitos clínicos é baseado na teoria da regra de proporção áurea. Quando adotada para situações complexas a proporção áurea pode ser um ponto de referência para a reabilitação. Deste modo, o trabalho tem como objetivo demonstrar uma reabilitação estética de diastemas múltiplos após tratamento ortodôntico, por meio de laminados vitrocerâmicos reforçados com dissilicato de lítio, onde o conceito de proporção áurea é aplicado. Descrição do caso clínico: Primeiramente, o plano de tratamento consistiu na distribuição uniforme dos diastemas, sendo posicionados da maneira mais harmônica e favorável para a oclusão na reabilitação com laminados vitrocerâmicos. A segunda fase consistiu na cimentação dos laminados, a fim de obter um resultado estético agradável. Discussão: A associação de ortodontia prévia a instalação dos laminados, permite uma melhor harmonia estética e oclusal, garantindo uma longevidade do tratamento. Opções restauradoras como laminados vitrocerâmicos reforçados com dissilicato de lítio permitem reabilitações extensas minimamente invasivas. BACKGROUND: Due to their high aesthetic outcome and long-term predictability, laminate veneers have become a common restorative procedure for anterior teeth. However, because of the variety in the preparation designs and the material types, the clinician faces a dilemma of which approach to use. AIM: To compare the survival rate of dental veneers according to different preparation designs and different material types. The sub-aim is to reach a favourable preparation design and material based on scientific evidence. METHODS: Comprehensive electronic search of the dental literature via PUBMED, MEDLINE and Scopus databases was performed using the following keywords: “porcelain veneers”, “composite veneers”, “all-ceramic veneers”, “success of porcelain veneers”, “preparation design”, “preparation geometry”, “patient’s satisfaction”. Additionally, references from the selected studies and reviews were searched for more information. RESULTS: Under the limitations of the available literature, the clinician preference is the decisive factor for choosing the preparation design. Nonetheless, incisal overlap preparation seems to have the most predictable outcome from all the preparation designs. CONCLUSION: Porcelain veneers show excellent aesthetic results and predictable longevity of the treatment, while composite veneers can be considered as a good conservative option, but with less durability.

ABSTRACT Scope of work The purpose of this work is the analysis and comparison of the various methods available for the realization of fixed restorations on natural teeth in the anterior sector with the presentation of some case report series. Materials and methods The methods used in relation to the clinical case, are the metal-ceramic, the galvan-ceramic, all- ceramic and porcelain veneers. Results It highlights indications and limitations of each of the methods proposed, emphasizing the prominent role of esthetics in the rehabilitation sector of the front. Conclusion The materials and techniques available, with proper clinical evaluation and to a timely therapeutic planning, are the basis of the rehabilitative ideal choice for each individual case, and they also depend on the achievement of clinical success. The demand for tooth-colored restorations has grown considerably during the last decades. Restoration of anterior teeth is a difficult task, even for an experienced operator. Color is the most important determinant of esthetics. The esthetic appearance of a restoration should match the surrounding dental tissue. This requires that the optical properties of the restorative material be similar to that of the natural teeth. Thus, for an acceptable esthetic result, favorable shade matching of the all-ceramic restoration should be achieved by controlling light absorption, reflection and transmission of the ceramic material. There are many different ceramic systems that can be used to achieve highly esthetic results. These ceramic systems include In-Ceram, Hi-Ceram, IPS-Empress, Optec, and CAD-CAM ceramics. All ceramic systems have different composition, microstructure, crystalline content and phases. Direct transmission, translucency, opacity and opalescence, all influence the optical properties of the ceramic restoration. Other factors include the thickness of porcelain, number of porcelain firing, glazing, porcelain powder/liquid ratio, surface texture and even the resin shade. This article focuses on controlling these variables to achieve the best possible esthetic result with an all ceramic system with the emphasis on CAD-CAM systems.

The increasing demand of patients looking for esthetics has resulted in the development of several techniques to restore anterior teeth. Conservative treatments should always be the first therapeutic option for the solution of aesthetic problems involving morphological changes and usually provide the result that the patient expects. In this context, ceramic laminate veneers, also known as “contact lenses,” are capable to provide an extremely faithful reproduction of natural teeth with great color stability and periodontal biocompatibility. Minimal or no preparation veneers are heavily advertised as the answer to patients’ cosmetic needs, when properly indicated by the dentist. This paper reports a clinical case where lateral incisor agenesis was aesthetically corrected using ceramic laminates.

Clinically, zirconia-supported all-ceramic restorations are failing by veneer-chipping without exposing the zirconia interface. We hypothesized that mouth motion step-stress-accelerated fatigue testing of standardized dental crowns would permit this previously unrecognized failure mode to be investigated. Using CAD software, we imported the average dimensions of a mandibular first molar crown and modeled tooth preparation. The CAD-based tooth preparation was rapid-prototyped as a die for fabrication of zirconia core porcelain-veneered crowns. Crowns were bonded to aged composite reproductions of the preparation and aged 14 days in water. Crowns were single-cycle-loaded to failure or mouth-motion step-stress- fatigue-tested. Finite element analysis indicated high stress levels below the load and at margins, in agreement with only single-cycle fracture origins. As hypothesized, the mouth motion sliding contact fatigue resulted in veneer chipping, reproducing clinical findings allowing for investigations into the underlying causes of such failures.

The use of CAD/CAM technology has allowed the fabrication of ceramic restorations efficiently and with predictable results. Lithium disilicate is a type of glass ceramic material that can be used for the elaboration of laminate veneers, being monolithic restorations which require characterization through a covering ceramic in order to achieve acceptable esthetic results. The next case report shows a predictable clinical protocol for the rehabilitation of the anterior teeth through the preparation of CAD/CAM veneers (e.max CAD, Ivoclar Vivadent, Liechtenstein) which have been characterized by a nanofluorapatite ceramic (e.max Ceram, Ivoclar Vivadent, Liechtenstein) through the layering technique.
