Purpose. To evaluate the repeatability of measurement of central corneal thickness (CCT) by spectral domain optical coherence (SD-OCT) in premature infants and compare it to CCT measurement by ultrasonic pachymetry (USP).Methods. Three CCT measurements of the left eyes of 50 premature infants were obtained by SD-OCT using the iVue system. 10 CCT measurements of each 28 left eyes of 28 infants were obtained by USP using the Pacscan 300P system. Bland-Altman plots were developed and the limit of agreement (LoA) was determined to compare the mean of the SD-OCT and USP measurements.Results. No statistically significant difference was found among the 3 CCT measurements by SD-OCT. Both USP and SD-OCT have been performed for only left eyes of 28 of the 50 babies. Those results have been compared with each other. A statistically significant difference was found between the mean CCT measurements by SD-OCT and USP (p<0.05). The LoA between the SD-OCT and USP measurements ranged from 11.4 to −64.1.Conclusions. CCT can be measured using the iVue SD-OCT system with a high level of repeatability. Although measurement of CCT by SD-OCT and USP is highly correlated, the 2 systems cannot be used interchangeably in premature infants.

Purpose. To evaluate the role of spectral-domain optical coherence tomography (SD-OCT) in early detection of Chloroquine maculopathy in rheumatoid arthritis (RA) patients.Methods. 40 left eyes of 40 female rheumatoid arthritis patients who received treatment chloroquine for more than one year were recruited in the study. All patients had no symptoms or signs of Chloroquine retinopathy. They were evaluated using SD-OCT, where the Central Foveal Thickness (CFT), parafoveal thickness and perifoveal thickness, average Retinal Nerve Fiber Layer (RNFL) thickness, and Ganglion Cell Complex (GCC) measurements were measured and compared to 40 left eyes of 40 normal females.Results. The mean CFT was found to be thinner in the Chloroquine group (238.15 µm ± 22.49) than the normal controls (248.2 µm ± 19.04), which was statistically significant (pvalue = 0.034). The mean parafoveal thickness was lesser in the Chloroquine group than the control group in all quadrants (pvalue<0.05). The perifoveal thickness in both groups showed no statistically significant difference (pvalue>0.05) in all quadrants. No significant difference was detected between the two groups regarding RNFL, GCC, or IS/OS junction.Conclusions. Preclinical Chloroquine toxicity can lead to early thinning in the central fovea as well as the parafoveal regions that is detected by SD-OCT.

Purpose: To evaluate and compare corneal hysteresis (CH), corneal resistance factor (CRF), and central corneal thickness (CCT), measurements were taken between a healthy population (controls), patients diagnosed with glaucoma (DG), and glaucoma suspect patients due to ocular hypertension (OHT), family history of glaucoma (FHG), or glaucoma-like optic discs (GLD). Additionally, Goldmann-correlated intraocular pressure (IOPg) and corneal-compensated IOP (IOPcc) were compared between the different groups of patients. Methods: In this prospective analytical-observational study, a total of 1065 patients (one eye of each) were recruited to undergo Ocular Response Analyzer (ORA) testing, ultrasound pachymetry, and clinical examination. Corneal biomechanical parameters (CH, CRF), CCT, IOPg, and IOPcc were measured in the control group (n = 574) and the other groups: DG (n = 147), FHG (n = 78), GLD (n = 90), and OHT (n = 176). We performed a variance analysis (ANOVA) for all the dependent variables according to the different diagnostic categories with multiple comparisons to identify the differences between the diagnostic categories, deeming p < 0.05 as statistically significant. Results: The mean CH in the DG group (9.69 mmHg) was significantly lower compared to controls (10.75 mmHg; mean difference 1.05, p < 0.001), FHG (10.70 mmHg; mean difference 1.00, p < 0.05), GLD (10.63 mmHg; mean difference 0.93, p < 0.05) and OHT (10.54 mmHg; mean difference 0.84, p < 0.05). No glaucoma suspects (FHG, GLD, OHT groups) presented significant differences between themselves and the control group (p = 1.00). No statistically significant differences were found in the mean CRF between DG (11.18 mmHg) and the control group (10.75 mmHg; mean difference 0.42, p = 0.40). The FHG and OHT groups showed significantly higher mean CRF values (12.32 and 12.41 mmHg, respectively) than the DG group (11.18 mmHg), with mean differences of 1.13 (p < 0.05) and 1.22 (p < 0.001), respectively. No statistically significant differences were found in CCT in the analysis between DG (562 μ) and the other groups (control = 556 μ, FHG = 576 μ, GLD = 569 μ, OHT = 570 μ). The means of IOPg and IOPcc values were higher in the DG patient and suspect groups than in the control group, with statistically significant differences in all groups (p < 0.001). Conclusion: This study presents corneal biomechanical values (CH, CRF), CCT, IOPg, and IOPcc for diagnosed glaucoma patients, three suspected glaucoma groups, and a healthy population, using the ORA. Mean CH values were markedly lower in the DG group (diagnosed with glaucoma damage) compared to the other groups. No significant difference was found in CCT between the DG and control groups. Unexpectedly, CRF showed higher values in all groups than in the control group, but the difference was only statistically significant in the suspect groups (FHG, GLD, and OHT), not in the DG group.

Purpose: Goldmann applanation tonometry (GAT) is considered the gold standard for Intraocular Pressure (IOP) measurement. It has the disadvantages of being a contact device, need for a slit-lamp, non-portability and need of a skilled examiner. Many hospitals are using a Non Contact Tonometry (NCT) as a screening device to save clinician time, however the usefulness is not proved in terms of reliability. This study was aimed to determine the usefulness of the Air-puff tonometer (TONOREF NIDEK II, NIDEK CO., LTD., JAPAN) over a GAT in a tertiary care center. Design: Cross-sectional Study Methods: This was a cross-sectional, non interventional observational study conducted on 224 eyes (right eye) from 224 patients. All patients underwent the IOP measurement with both methods and a central corneal thickness (CCT) measured. The data was analyzed using SPSS 20.0 software. Results: The mean age of the patients was 40.3±11.29 years. There was a statistically significant difference (p<0.001) between the mean NCT and GAT readings which persisted even after correction for central corneal thickness. The correlation between NCT and GAT using Pearson’s correlation coefficient was strong irrespective of the corrections for their corneal thickness (r = 0.751 and 0.718 for uncorrected and corrected values respectively). The correlation of the individual clinicians for the readings varied from moderate to strong. The ROC curve showed the best sensitivity and specificity to occur at around 13 to 14 mmHg. Conclusion: NCT seems to overestimate the IOP at low ranges as compared to the GAT and underestimate at higher ranges. The crossover of the values is seen between 12 to 13 mmHg.  The clinician should do an individualized analysis of his/her GAT measurements to the readings of the NCT machine at the clinic to obtain clinician specific nomogram.

Background: Central Corneal Thickness (CCT) is an indicator of corneal health status as well as being an essential tool in assessment and management of corneal diseases. It is an important factor in the diagnosis and management of glaucoma as it affects the measurement of intraocular pressure. However, the pattern of central corneal thickness in our population is not known. Our study aimed to describe the CCT measurements and their variation with age and sex among patients attending the eye clinic at Muhimbili National Hospital (MNH), in Dar es Salaam, Tanzania. Methodology: A hospital-based descriptive, cross-sectional study used convenient sampling to recruit adult patients aged 18 years and above, presenting to the MNH between August 2016 and January 2017.Visual acuity assessment, Goldman applanation tonometry and CCT assessment using an ultrasound pachymeter were performed. Patients with previous intraocular surgery were excluded. Results: A total of 398 patients (208 males and 190 females) were recruited. The mean CCT was 526.64±38.30 µm; being 523.99±38.20 µm for males and 529.7±38.3 µm for females. 226 (56.85%) had CCT of less than 520 µm.There was no statistically significant difference in CCT between gender. The proportion of patients with thinner CCT increased with increasing age from 25.8% in those < 30 years to 75% in those aged 71years and above. The mean CCT decreased with increasing age with P-value<0.001. Conclusion: The average CCT in patients attending Muhimbili National Hospital is 526.64 µm (SD 38.30) which is generally thin, inversely proportion to age and similar that of other Africans and African-Americans. This finding has implications for the management of glaucoma in this population.

Purpose: To determine the central corneal thickness in myopic adult patients scheduled for laser corneal refractive surgery and to explore its correlation with degree of refractive error. Materials and Methods: The case records of 130 myopic patients who underwent laser corneal refractive surgery in a military hospital over a period of two years were reviewed to determine the central corneal thickness. All patients had 6/6 vision with best correction, and did not have any other anterior segment or fundus diseases in both eyes. The central corneal thickness was measured with Visante Carl Zeiss anterior segment optical coherence tomography instrument.                                                                                                                        Results: Out of 130 patients, males were more (73, 56.2%); mean age of patients was 33.8 years (range 18-60 years) and majority were Malays (110, 84.6%).  The spherical power of myopia ranged from – 0.5 to – 10.00 D, and the cylindrical power ranged from – 0.25 to – 3.25 D. The mean central corneal thickness of both eyes was 528.2 µm (range 331- 615 µm); in the mild degree of myopia (- 0.50 to - 2.00 D) 527.9 µm, moderate degree (- 2.25 to -5.00 D) 529.4 µm, and high degree (-5.25 to -11.00 D) 523.9 µm. Conclusion: The anterior segment optical coherence tomography provides noncontact, rapid, pachymetry mapping of the corneal thickness. In Malaysian patients, the mean central corneal thickness of both eyes in myopia was 528.1 µm (range 331- 615 µm). There was no correlation between the mean central corneal thickness and degree of myopia, different genders, age groups, ethnic groups and two eyes.

Purpose. To evaluate the role of spectral-domain optical coherence tomography (SDOCT) to measure corneal thickness during accelerated corneal crosslinking (CXL).Methods. Intraoperative pachymetry was performed using SDOCT and ultrasound pachymetry (USP) in 6 eyes of 6 patients with keratoconus. Pachymetry readings were obtained at baseline, after epithelium removal and after 30 minutes of riboflavin instillation. SDOCT measurements of eyes with and without lid speculum during riboflavin instillation were compared.Results. There was no statistically significant difference in central corneal thickness (CCT) measurements between SDOCT and USP (P>0.05for all). A significant decrease in both CCT (P=0.031) and the thinnest corneal thickness (TCT) (P=0.031) was observed during CXL. There was a greater reduction in CCT (38 ± 6%) with the use of lid speculum as compared to the no-speculum eyes (18 ± 9%) (P=0.100). TCT was also reduced by a greater extent with the use of lid speculum (40 ± 5% versus 26 ± 7%;P=0.100).Conclusion. SDOCT can be successfully used to measure intraoperative corneal pachymetry during corneal CXL. SDOCT measurements demonstrated corneal thinning intraoperatively during CXL, which was further accentuated by the use of a lid speculum during the procedure.

Objective. To compare the results and repeatability of the corneal thickness (CT) and epithelial thickness (ET) maps provided by Swept-Source-Optical Coherence Tomography with those of Spectral-Domain-OCT in normal eyes. Methods. 30 normal eyes of 30 patients were assessed by 3 trained operators with SS-OCT and SD-OCT. Results. The central and minimum ET obtained with both devices were correlated: central ET, r = 0.86, p < 0.05 ; minimum ET, r = 0.72, p < 0.05 . Compared with SD-OCT, SS-OCT tended to underestimate these figures by 1.4 and 1.9 μm on average. The central and minimum CT obtained with both devices were strongly correlated: central CT, r = 0.994, p < 0.05 ; minimum CT, r = 0.995, p < 0.05 . SS-OCT tended to overestimate these figures by 11 and 14 μm on average. Repeatability was good for both devices with a mean coefficient of variation of measurements <6% for ET and <2% for CT. Interoperator variability (standard deviation and COV) was significantly higher for SS-OCT than for SD-OCT for all local epithelial thicknesses and significantly lower for the central CT and several local corneal thicknesses, whereas no significant differences between both technologies were found for the central and minimum ET and the minimum CT. Conclusion. SS-OCT and SD-OCT provide reproducible measurements of CT and ET in normal corneas with a strong correlation between both technologies. However, both technologies are not interchangeable when the main thickness parameters (i.e., central and minimum CT and minimum ET) are used for diagnosing early keratoconus or calculating the expected residual stromal bed thickness before corneal refractive surgery or anterior lamellar keratoplasty.

Objective: To study and compare the value of central corneal thickness in a healthy adult population, using different non-contact techniques from daily clinical practice. Method: This work examines and compares the pachymetric measurements of 71 eyes of 36 healthy adult patients. Corneal thickness was evaluated using three non-contact instruments: a CEM-530 specular microscope (Nidek), Pentacam HR corneal tomography (Oculus), and Cirrus HD-OCT 5000 optical coherence tomography (Carl Zeiss Meditec). Tests were performed consecutively by a single observer in the same session and under the same environmental conditions. Results: The results obtained using the three devices are interchangeable, given that no statistically significant difference was observed among them. The study did not find statistically significant difference with respect to gender either. Conclusion: These three non-contact methods for the measurement of corneal central pachymetry are a reliable alternative to ultrasonic contact pachymetry.

Abstract Purpose: To quantify the retinal thickness and volume using spectral-domain optical coherence tomography (SD-OCT) analysis in the macular region of patients with Coronavirus Disease 2019 (COVID-19).Methods: In a comparative cross-sectional, observational study, patients recovered from COVID-19 were included. All included subjects had a reverse transcription-polymerase chain reaction (RT-PCR) - confirmed diagnosis of COVID-19. Macular SD-OCT was performed at least two weeks after recovery from systemic COVID-19. Inner, outer and full retinal thicknesses and volumes were measured in COVID-19 recovered patients versus age-matched normal controls.Results: Twenty-five patients (11 male) with a mean age of 36.4 ± 11 years and 60 healthy controls (31 males) with a mean age of 39.3 ± 7.7 years were enrolled in the study. There was no statistically significant difference in the retinal thickness or volume measures between the two groups. However, the thickness in the case group was minimally more than the controls. Conclusion: Retinal thickness in COVID-19 patients may be higher than healthy subjects. Comprehensive ocular examination with special focus on posterior segment manifestations should be considered in these patients.
