The oral rehabilitation of edentulous patients with immediate loading has become a safe procedure with high predictability. The success is related to immediate fabrication of a passive fit framework to attach the implants. Based on these considerations, this case report shows an alternative technique for mandibular rehabilitation using implants immediately loaded, where the framework was fabricated using cylinders with internal reinforcement and precast pieces, electrowelding, and conventional welding providing esthetics and function to the patient in a short period of time.

The implants failure may be caused by micromotion and stress exerted on implants during the phase of bone healing. This concept is especially true in case of implants placed in atrophic ridges. So the primary stabilization and fixation of implants are an important goal that can also allow immediate loading and oral rehabilitation on the same day of surgery. This goal may be achieved thanks to the technique of welding titanium bars on implant abutments. In fact, the procedure can be performed directly in the mouth eliminating possibility of errors or distortions due to impression. This paper describes a case report and the most recent data about long-term success and high predictability of intraorally welded titanium bar in immediate loading implants.

Edentulism is a current and relevant oral-health problem which has a negative social and psychological effect on patients’ quality of life. Implant-supported fixed restoration is a well-established treatment method for edentulous patients. Current concepts and techniques in implant dentistry have developed and changed with time to provide better esthetics and functional outcomes quickly.  Immediate loading offers several advantages over conventional loading without compromising the results. An immediate fixed provisional promotes a high level of patient satisfaction with respect to esthetics, phonetics, mastication and psychological comfort, enabling patients to return to their normal routine and maintain quality of life within a short period of time. Other advantages to extraction with simultaneous replacement include the maintenance of vertical dimension, elimination of interim denture therapy, and potential improvement of soft tissue healing. The aim of the present case report was to demonstrate step wise oral rehabilitation of maxillary arch using all on 6 immediate placement and loading protocols with successful outcome after 2 years follow up.

AbstractCatatonia is a neuro-psychiatric disorder that can occur in medical, psychiatric and drug-induced conditions but is often unrecognised. A 64 year-old woman was admitted to hospital for assessment of a significant deterioration in her ability to communicate and function normally so that she had become completely dependent on others for all activities of daily living for nearly three years. Outpatient medical and psychiatric assessments failed to clarify diagnosis. On admission to a general hospital, the neurology team initially believed she had a Parkinson's-like syndrome but after further investigation and comprehensive multidisciplinary assessment, including neuro-psychiatric review, she was diagnosed with catatonia. She was subsequently admitted to a psychiatric hospital where she received electro-convulsive therapy and changes to her psychotropic medication regimen. She responded well to treatment and within a short period of time was able to function independently again.

The rehabilitation of maxillary and mandibular bone atrophy represents one of the main challenges of modern oral implantology because it requires a variety of procedures, which not only differ technically, but also differ in their results. In the face of limitations such as deficiencies in the height and thickness of the alveolar structure, prosthetic rehabilitation has sought to avoid large bone reconstruction through bone grafting; this clinical behavior has become a treatment system based on evidence from clinical scientific research. In the treatment of atrophic maxilla, the use of zygomatic implants has been safely applied as a result of extreme technical rigor and mastery of this surgical skill. For cases of posterior mandibular atrophy, short implants with a large diameter and a combination of short and long implants have been recommended to improve biomechanical resistance. These surgical alternatives have demonstrated a success rate similar to that of oral rehabilitation with the placing of conventional implants, allowing the adoption of immediate loading protocol, a decrease in morbidity, simplification and speed of the treatment, and cost reduction. This case report presents complete oral rehabilitation in a patient with bilateral bone atrophy in the posterior regions of the maxilla and mandible with the goal of developing and increasing posterior occlusal stability during immediate loading.

AbstractIntroduction. Complete edentulism is escorted by various comorbidities, which affect an individual. The management of edentulous patients was approached since the early days of dentistry.The aim of this case report was to present the implanto-prosthetic rehabilitation of a bimaxillary complete edentulous patient with implant-supported overdentures, by using Straumann dental implants.Case presentation. This case report presents the applied treatment to a bimaxillary complete edentulous patient, where the predictable osseointegration and implant stability after the placement of implants was possible, with the purpose to support the overdentures.Conclusions. The oral rehabilitation of complete bimaxillary edentulism through implant-supported overdentures is a procedure with a predictive treatment that presents beneficial aspects in the quality of life of the patients.

Dental implants are now considered as a predictable treatment modality for the oral rehabilitation of partially or fully edentulous patients. Recently emphasis has changed towards achieving a predictable esthetic success. Creating aesthetically successful implant-supported restoration in the anterior region of oral cavity depends on the presence of interimplant papilla when multiple implants are used. The present paper reports a case of interimplant papilla reconstruction in esthetic zone of maxilla during one stage early loading multiple implant procedure using demineralized freeze dried bone allograft block fixed by titanium screw.

Case report of an adult Bangladeshi male aged 26 years who complained about ineffective chewing of food and unpleasant aesthetic look. The patient was diagnosed to have a 4 mms of anterior dental open bite on skeletal Class I with normal mandibular plane and palatal plane angles. Treatment was orthodontics alone with non-extraction. The main mechanics used to close the anterior open bite, were by the conventional aligning, leveling and arch contraction by standard edgewise technique. The result showed that it was possible to correct the open bite within a short period; however, the appliance was difficult to clean. The treatment was complete after 2 years with positive overbite of 3 mms and acceptable occlusion. The patient was happy with his new appearance and function. Delivering a full arch implant restoration, which transits a patient from the frustrations of edentulism to the stability and function of a fixed prosthesis, is one of the most rewarding experiences in dentistry. With the advancement in digital technology, and with modified implant design and advance prosthetic options, what started as a two stage delayed loading procedure in the seventies, evolved to immediate implant placement in extraction sockets and even immediate loading of these implants in a growing number of indications. In this case report, the same evolution “immediate implant placement and immediate loading techniques” has been described using fast and fixed protocol from Bredent to rehabilitate patient’s failing dentition.

Head and neck cancer occupy the seventh place among the most common neoplasms in the world, with an annual incidence of approximately 640,000 new cases. Radiotherapy treatment has been highlighted, because there is greater tissue preservation, possibility of regression of the disease, decreased tumor volume, prevention of possible tumor recurrences and metastases, and even total cancer cure, but patients undergoing radiotherapy are at high risk of osteoradionecrosis if they perform surgical interventions in the region, such as dental implants and dental extraction, making prosthetic rehabilitation of partially edentulous patients after treating oral cancer something complex and often omitted; although it is a crucial factor in restoring oral function. Thus, the aim of the present study was to describe the planning, treatment and follow-up of a twelve-month prosthetic approach to rehabilitate the lower arch shape and function of a partially edentulous patient undergoing head and neck radiotherapy with a full denture mucus supported and tooth retained. After twelve months of follow-up, it was possible to evidence the functional and aesthetic clinical success, with preserved occlusal pattern, soft and bone tissue around[ and dental roots.KEYWORDSRadiotherapy; Head and Neck Neoplasms; Overdentures; Oral Rehabilitation.

There are growing prosthetic and esthetic demands for Oral Rehabilitations on osseointegratable implants, requiring precise prosthetic-surgical planning. In edentulous patients planning may be done using cephalometric analysis to determine the position of the teeth in the dental prosthesis, and consequently, those of the implants. In this clinical case, the planning and treatment of an oral implant-supported rehabilitation is described, using cephalometry to optimize prosthetic success and patient comfort. The patient presented complete mandibular and partial maxillary edentulism, with unsatisfactory esthetics and function of the anterior teeth, with accentuated vestibular inclination. In order to determine the correct position of maxillary teeth it was necessary to use a Steiner cephalometric tracing to position the maxillary central incisor in the diagnostic wax-up. Therefore, the maxillary anterior teeth were extracted, osseointegratable implants were placed (Neodent(r), Curitiba, Brazil), and an immediate temporary fixed denture was inserted. After 30 days, surgery was performed for the placement of 4 osseointegratable implants (Neodent(r), Curitiba, Brazil) in the inter-mentonian region, on which a complete, temporary, implant-supported denture was placed. After the period of osseointegration, the definitive implant-supported dental prosthesis were fabricated.
