Introduction. The association of diabetes and atherosclerosis with disorders of lipids and lipoproteins, notably high apolipoprotein B (apoB) and low apolipoprotein A1(apoA1) is well established. Because of the beginning of the atherosclerosis' process from early life, in this study, the plasma levels of apoA1 and apoB were compared in diabetic children with type I diabetes mellitus(DM), healthy children with diabetic parents (HDPs),and healthy children with nondiabetic parents (HNDPs). Methods. This case-control study was conducted among 90 children aged 9–18 years. Serum levels of apoA and apoB were compared among 30 diabetic children (DM), 30 healthy children with diabetic parents (HDPs), and 30 healthy children with nondiabetic parents (HNDP). Results. The mean serum apoA1 was higher in DM (153±69 mg/dL) followed by HNDPs (138±58 mg/dL) and HDPs (128±56 mg/dl), but the difference was not statistically significant. The mean apoB value in HNDPs was significantly lower than DM and HDPs (90±21 mg/dL versus 127±47 and 128±38 mg/dL, P<0.05, respectively). The mean apoB levels in DM (127±47 mg/dl) and HDP (128±38 mg/dL) were not statistically significantly different (P>0.05). Conclusions. Diabetic children and healthy children with diabetic parent(s) are at higher risk of dyslipidemia and atherosclerosis. Thus for primordial and primary prevention of atherosclerosis, we suggest screening these children for low plasma apoA1 and high plasma apoB levels.

Kawasaki disease (KD) is the most common cause of pediatric acquired heart disease. KD patients have spontaneously high plasma/serum levels of IL-10 during the acute phase. Therefore, two independent studies were carried out to investigate the association between genetic variants in IL-10 promoter (−1082, −819, and −592) and risk of KD. A total of 134 trios were included for the family-based association study. A significantly preferential transmission of the C allele at loci −819 T > C and −592 A > C for KD cases was observed (Ppermutation= 0.029 and Ppermutation= 0.034, respectively). There was a significant increase in the transmission of haplotype CC (p= 0.016) at the above two loci (OR, 1.632; 95% CI, 1.090–2.443; Ppermutation= 0.019). We also carried out a follow-up case-control study that included 146 KD cases and 315 unrelated healthy children. {The haplotype CC (−819, −592) showed an increased risk of KD (but statistically non-significant; OR, 1.332; 95% CI, 0.987–1.797;p= 0.061). In diplotype analysis, a trend was found between number of CC haplotype and risk of KD (but non-significant,p= 0.061). In conclusion, CC genotype and CC/CC diplotype at IL-10-819T > C and −592A > C were significantly associated with risk of KD in case-parent trio study, which were replicated partially in our follow-up case-control study.

Background: The emergence of chronic physiologic diseases such as diabetes damages the children's psychological, cognitive, communicational, social and emotional processes. Therefore, the present study was aimed to compare executive functions in normal and diabetic children. Methods: The research method was cross-sectional. The statistical population included healthy and sick children in 2019 in Isfahan. The statistical population of the study included diabetic children and 150 healthy children who were selected through convenient sampling method. The applied questionnaires were the executive functions. The data from the study were analyzed through t method. Results: There is a significant difference between diabetic children and healthy ones in the variable of executive functions (p < 0.001). In a way that diabetic children got lower mean scores in executive functions. The mean scores of executive functions of children with diabetes were 233.63 and the mean scores of non-children with diabetes were 192.64. Conclusion: According to the findings of the present study it can be concluded that diabetes causes the decrease of executive functions due to being chronic and this process emphasizes the necessity of applying child-oriented psychological interventions for these people.

Hematuria of unknown origin occurs in 30% of patients with diabetic nephropathy. In nondiabetic persons, hematuria may be caused by hypercalciuria with or without nephrolithiasis. Eight children with type I diabetes mellitus, hematuria, and hypercalciuria were observed in our clinic during a 1-year period. Two of these also had evidence of renal papillary necrosis. To assess the importance of hypercalciuria in the pathogenesis of hematuria in children with diabetes mellitus, we measured urinary calcium excretion in a large population of such patients. The calcium to creatinine ratio in the urine of diabetic children (0.21 ± 0.01) was greater than that of nondiabetic children (0.12 ± 0.01). A calcium to creatinine ratio of 0.28 was established as the upper limit of normal in our nondiabetic population, and 27% of the diabetic children were hypercalciuric on this basis. The diabetic children with hypercalciuria also had hyperphosphaturia and a urinary CaHPO4.2H2O molar ion product three times that found in the nondiabetic control population. These data suggest that many children with diabetes are at risk for renal damage due to hypercalciuria. Because hypercalciuria is more common in diabetic than nondiabetic children, it may play a previously unrecognized role in the renal disease associated with diabetes mellitus.

Background Individuals residing at higher altitude may have oxygen saturation of hemoglobin different to those living at lower altitude. # Objectives To find out the baseline value of SpO2 in healthy Nepali children (2-14 years) living permanently at high altitude using pulse oximeter and also to study the relation of SpO2 with age, sex and ethnicity. Methods A descriptive observational study was conducted at 4 different altitudes ranging from 2700 to 3800 m in Mustang district. The mean pulse oximery values at different altitudes were calculated and compared. Results One hundred six children were enrolled with the median age of 10 years. The mean SpO2 value of children permanently residing at altitude 2700m was 95.18%, at 2800m was 94.82%, at 3550m was 94.1% and 3800m was 93.1%.The difference in the SpO2 values at different altitude was statistically significant. No sex or age wise differences were noted on the mean SpO2 values in the study group. Conclusions The mean SpO2 values were higher than several other studies done in the altitude above 2500 meters. Enrollment of older children and the different ethnic background could be the contributing factors for the differences. Abstract Introduction The clinical complications of congenital hypothyroidism such as brain disorders are very subtle and are not recognizable in infancy period. They are recognizable when it is too late for treatment or prevention. General screening of newborns is effective in diagnosing congenital hypothyroidism and initiating initial treatment. The aim of this study is to compare the physical and mental growth pattern of children with congenital hypothyroidism with healthy ones. Methods This case–control study was performed on 34 patients and 68 healthy children who were matched in terms of inclusion and exclusion criteria. Children development screening test (ASQ), children development assessment test (Bayley), preschool Wechsler intelligence scale (WPPSI) and age and steps questionnaire of emotional social development (ASQ-SE) were completed by trained questioners. Data were analyzed using STATA software. Results The results indicated that there was no significant difference between the mean of verbal (P = 0.77), non-verbal (P = 0.81) and general (P = 0.66) IQ in permanent and transient patients and healthy individuals. Also, there was no significant difference between the mean of different ranges of ASQ test (including communication, delicate and large movements, problem solving and social) at 12 months and 42 months (P < 0.05). According to Bayley test, there was no significant difference between the cases (permanent and transient) and controls in the cognitive (P = 0.42) and expressive (P = 0.38) categories. The difference was significant in the perceptual (P = 0.011), large (P = 0.03) and delicate (P = 0.04) movements categories. Conclusion This study emphasized on the high effectiveness of neonate hypothyroidism screening program, so that the difference between 3.5 years old children with and without this disease has decreased significantly. Early diagnosis of the patients, while creating beneficial effects for patients and increasing quality of life, cause reduction in the long-term costs of the health system.

Aim of the study was to correlate fibronectin (Fn) and von Villebrand factor (vWf) levels measured in plasma and in platelets with the progression of diabetic retinopathy. Patients were classified in five groups reflecting the progression of this microvascular complication, on the basis of fluorangiographic findings (0 = no microangiopathy; 1= simple microangiopathy; 2= oedematous retinopathy; 3= ischaemic retinopathy; 4= ischaemic proliferative retinopathy). 43 patients were studied, 22 suffering from type I diabetes and 21 from type II diabetes, according to the classification of National Diabetes Data Group. Fn and vWf were measured in plasma and in platelet samples using an original double-sandwich microELISA method and expressed as micrograms/ml or as micrograms/10* platelets. Platelets were counted and solubilized with 0.5% Triton × 100. Bleeding time and platelet adhesion to glass beads were also evaluated on every patient. Intraplatelet Fn levels were reduced in retinopathies and correlate with the severity of the microvascular alteration, being the difference significant between the two extreme groups (p<0.05). vWf intraplatelet levels were also significantly lower in patients with severe microvascular complications (p<0.05). No significant differences were detected for plasma Fn and vWf levels in the 5 groups. Intraplatelet Fn and vWf levels may therefore be considered as markers of the severity of diabetic retinopathy. The leakage of Fn and vWf from activated platelet and the incorporation of these glycoproteins in the subendothelial matrix may be responsible for the worsening of this microvascular complication.

Abstract We assessed the effect of particular apolipoprotein (apo) E phenotypes, lipoprotein(a) [Lp(a)], and other lipoproteins on the development of dyslipoproteinemia in 450 patients with type I diabetes, ages 13-14 years. The control group consisted of 450 healthy school children of both sexes, ages 13-14 years. Both groups were found to be normolipidemic, but the concentration of Lp(a) was significantly (P &lt; 0.05) higher in the diabetic children than in the control group. Apo E 3/2 and apo E 4/4 phenotypes were more frequent in the group of diabetics. Diabetics with the apo E 3/3 phenotype had higher concentrations of very-low-density lipoprotein (VLDL) and Lp(a), and lower concentrations of low-density lipoprotein (LDL) than the apo E 3/3 nondiabetics. For apo E 3/2 phenotypes, total cholesterol, LDL cholesterol, LDL, apo A-I, and Lp(a) concentrations were higher in the diabetic children than in the control group; for apo E 4/3 phenotypes, this was true for triglycerides and VLDL cholesterol. The distribution of Lp(a) lipoprotein concentrations between 0.01 and &gt; 0.5 g/L indicated a more frequent occurrence of higher Lp(a) values in diabetic children than in the control group. Results of this study indicate that an increased concentration of Lp(a) lipoprotein and apo E 3/2 and apo E 4/3 phenotypes contribute to the expression of dyslipoproteinemia in type I diabetes in childhood.

Abstract. Luteinizing hormone releasing hormone (LRH) and human chorionic gonadotrophin (hCG) tests were performed in 18 prepubertal and 29 pubertal boys with hypospadias to examine pituitary and gonadal function. Thirty-one normal boys were also examined as controls. In the prepubertal group, the mean level of serum peak LH after LRH was significantly higher and the mean level of serum peak testosterone after hCG was significantly lower in boys with hypospadias than in normal boys, though about half of the patients had peak LH levels within ± 2 sd of the normal mean. There was little or no response to hCG in 3 out of 18 boys with hypospadias. In the pubertal group, serum levels of LH, FSH and testosterone in all cases were clearly higher than those in the prepubertal group and distinct or moderate responses to LRH and hCG were found in all boys including patients examined. Although serum basal FSH and testosterone levels were similar in the two groups, the mean levels of serum basal LH, peak LH and FSH in LRH test were significantly higher and the mean level of serum peak testosterone after hCG was significantly lower in boys with hypospadias than in normal boys. However, more than half of the patients had these levels within ± 2 sd of the normal means. The difference was more marked in LH than in FSH levels. An elevated mean serum FSH suggests hypofunction of not only the Leydig cells but also of the seminiferous tubules at least in some boys with hypospadias.

ABSTRACTObjective: The aim of this study is to analyze the association of serum lipids and lipoproteins in patients with angina pectoris.Methods: A total of 110 patients (60 males and 50 females) with angina pectoris aged (55±5) years and 80 healthy controls (43 males and37 females) aged (45±4) years were enrolled in this case-control study from the clinic of Al Yarmouk Hospital. Serum lipids, lipoprotein(a) [Lp(a)],apolipoprotein-A1, and apolipoprotein-B levels were measured and studied.Results: The results of this study showed that increased odds of angina pectoris were associated with increased serum levels of Lp(a) more thanserum levels of apolipoprotein-B.Conclusions: Analysis of Lp(a) may be an important determinant of cardiovascular disease diagnosis.Keywords: Lipoprotein(a), Apolipoprotein-A1, Apo lipoprotein-B, Angina pectoris.
