A novel class of naturally-occurring glycosidase inhibitors, having sulfonium sulfate structures, has been isolated as bioactive components from Indian plants, belonging to the Salacia genus in the family Celastraceae, and used in Ayurvedic medicine for the treatment of type-2 diabetes. Thus far, five such sulfonium salts, namely, salacinol, kotalanol, salaprinol, ponkoranol and de-O-sulfonated kotalanol, have been isolated from this plant species. These structurally unique zwitterionic glycosidase inhibitors have received much attention due to their therapeutic potential in the treatment of type-2 diabetes. We recently reported a review article which focused mainly on salacinol and related analogues. The present review presents an update on the remaining four compounds from this class of glycosidase inhibitors, with respect to their isolation, glucosidase inhibitory activities, and synthesis. In addition, progress towards the stereochemical structure elucidation of kotalanol, through synthesis of analogues, is described. Review with 42 references.

The work discusses the two biomedical problems: family diabetes (bearing in mind the presence of cases of type 2 diabetes mellitus in the family, including its different generations) and the features of relationship of family diabetes with major non-communicable human diseases (NCDs). The paper is timed to the anniversary of the famous - in our country and abroad - expert in the field of gerontology and endocrinology, Professor V. M. Dilman. The widely recognized works of V. M. Dilman, based on original ideas and giving rise to important practical consequences (including the use of antidiabetic biguanides in areas not studied before him, the need to eliminate metabolic immunodepression, to take into account the changes with age at the level of the hypothalamic threshold in various homeostatic systems and a whole number of other essential proposals), which for a long time, as it seems, will stimulate the further scientific search of his followers and specialists, who have yet to get acquainted with the area that attracted Prof. Dilman and interested him for many years.

Glucagon is used for the treatment of hypoglycemia, and glucagon receptor antagonists are under development for the treatment of type 2 diabetes. Moreover, glucagon-like peptide (GLP)-1 and GLP-2 receptor agonists appear to be promising therapies for the treatment of type 2 diabetes and intestinal disorders, respectively. This review discusses the physiological, pharmacological, and therapeutic actions of the proglucagon-derived peptides, with an emphasis on clinical relevance of the peptides for the treatment of human disease.

1. Glucagon-like peptide-1 (7-36) amide (GLP-1) is a gut hormone released postprandially that stimulates insulin secretion, suppresses glucagon secretion and delays gastric emptying. The insulinotropic action of GLP-1 is more potent under hyperglycaemic conditions. Several published studies have indicated the therapeutic potential of subcutaneous GLP-1 in non-insulin-dependent (Type 2) diabetes mellitus. 2. We investigated whether subcutaneous GLP-1, at a dose shown to improve glycaemic control in early Type 2 diabetes, is insulinotropic at normal fasting glucose concentrations. A double-blind, randomized, crossover study of 10 healthy subjects injected with GLP-1 or saline subcutaneously after a 16 h fast was performed. The effect on cardiovascular parameters was also examined. 3. GLP-1 caused a near 5-fold rise in plasma insulin concentration. After treatment with GLP-1, circulating plasma glucose concentrations fell below the normal range in all subjects. One subject had symptoms of hypoglycaemia after GLP-1. A rise in pulse rate was found which correlated with the fall in plasma glucose concentration. An increase in blood pressure occurred with GLP-1 injection which was seen at the same time as the rise in plasma GLP-1 concentrations. 4. This study indicates that subcutaneous GLP-1 can override the normal homoeostatic mechanism maintaining fasting plasma glucose in man, and is also associated with an increase in blood pressure.

The review considers recent experimental studies of biological activity and mechanisms of therapeutic action of rosmarinic acid, luteolin and its sulfated derivatives in diseases associated with disorders of carbohydrate and lipid metabolism. Particular attention is focused on the results of studies showing a high therapeutic potential of these phenolic compounds in their prophylactic and therapeutic use at experimental modeling of type 2 diabetes and hyperlipidemia. Based on the analysis of our results and the literature data putative mechanisms of therapeutic action of rosmarinic acid, luteolin and its sulfated derivatives have been proposed.

Abstract Introduction: The high rate of diabetes mellitus index (DM), along with the increase in cardiovascular compromise that DM favors, and the scarcity of epidemiological data regarding the prevalence of peripheral arterial disease (PAD) in this population, make it important to study risk factors associated with the development of PAD in the population with type 2 diabetes mellitus (DM2). Objective: To estimate the prevalence of PAD together with the associated factors in a sample of patients with DM2, treated in the Family Health Strategies (FHS) program, in the municipality of Pindamonhangaba, SP. Methods: Quantitative research in a cross-sectional study of 38 individuals who were diagnosed with DM2, between 40 and 77 years old, selected by convenience sampling and treated in the family health program in two different districts of the municipality. The method consisted of the evaluation of personal and anthropometric data, anamnesis and physical examination including the ankle-brachial index (ABI). Results: PAD was present in 21.1% (95%CI: 16.9 to 25.8) of the investigated population. Risk factors observed were age range of 51 to 69 years (75%), overweight (50%), systemic arterial hypertension (SAH) (100%), smoking (62.5%) and physical inactivity (87.5%). Conclusion: The prevalence of PAD was more than a fifth of those diagnosed with DM2, and the most prevalent associated risk factors were SAH, physical inactivity, smoking and overweight with and without PAD.

AbstractDiabetes mellitus is a chronic disease that cannot be cured, but can be prevented by using the 5 pillars of diabetes mellitus management, one of which is controlling blood sugar levels. Patients with diabetes mellitus need support from the family because the family can influence a person's behavior and lifestyle so that it has an impact on the quality of life of patients with type 2 diabetes mellitus. To determine the relationship between family support and blood sugar levels in patients with type 2 diabetes mellitus through a literature review. The data collection technique uses a literature review method of 5 articles sourced from an online database with electronic searches on Google Scholar, Garba Garuda, and ProQuest published in 2017-2021. The critical analysis instrument used is using a strobe. The results of a literature review on the family support variable were 2 articles with supporting categories as many as 128 respondents, 2 articles with less categories as many as 63 respondents, and 1 article with good categories as many as 22 respondents. Variable blood sugar levels 2 articles in the controlled category as many as 98 respondents, 1 article in the poor category as many as 45 respondents, 1 article in the controlled category as many as 17 respondents, and 1 article in the diabetes category as many as 29 respondents. There is a relationship between family support and blood sugar levels in patients with type 2 diabetes mellitus. Insulin secretion declines progressively before and during the course of type 2 diabetes. Evidence indicates that this process is, in part, secondary to increased requirement for insulin secretion that is brought about by insulin resistance and by hyperglycemia. The effects of over-secretion extend far beyond a mere reduction of available insulin stores and may cause not only functional but also structural damage. The time is ripe for clinical studies, which explore the therapeutic potential of reducing over-secretion.

We studied 98 native Greek patients with Type 2 diabetes mellitus. Their degree of glycemic control, evaluated with glycated hemoglobin A1c levels, was correlated with perceived family support, assessed with the Family Support Scale. The different social context of Greece—and its corresponding perception of family support—influence glycemic control in a way that is at variance from data of some studies of Anglo-Saxon families. In conclusion, family support should be taken into consideration in the management of native Greek patients with Type 2 diabetes mellitus.
