Nonlinear FEA Simulation of Thorax Considering Transient CPR Forces and Sternotomy

The median sternotomy has become the desired incision in the modern era of cardiac surgery. The objective of this study is to investigate the sternum loading due to daily forces after sternotomy and during healing. Two models of thorax were built. The first is to simulate the healthy thorax and the second is to simulate the thorax after sternotomy and got closure using stainless steel stitches. In this paper, ANSYS was used to build the throax model. The results that have been collected after solving the model were analyzed. The analysis was promising and proved that the model was working properly and its ability to simulation what happens in the real life.

The article analyses the final lines of V. Soloviev’s work “The General Sense of Art,” correlating them with the art tendencies of the modern civilization. The author addresses Soloviev’s idea about the transforming role of art in relation to the reality, the real life. It is stated that, in the modern era, within the commercial mass culture domination, the entertaining function of art comes to the forefront. At the same time, an opposing tendency can be observed: a movement towards the all-encompassing unity, understanding of the universal, the priority of common values (the ideas of ecological ethics, common religion, non-violence). The author indicates that the dialectic negation of the negation law is manifested in the history of art, in the realization of its varied functions.

This paper examines the mental revolution in applying the Islamic economics system which is expected to open the horizon of humans’ thought, especially Muslims, so as they are always careful in carrying out all economics activities. The significance embodied in the idea of the mental revolution is the transformation of the ethos, namely the fundamental change in the mentality, the way of thinking, the way of feeling, and the way of believing that is proven in daily behavior and actions. The mistake which occurs in the economics system of this modern era vastly needs a mental revolution to restore the consciousness of economics actors that the world is only an intermediary towards the real life in hereafter so that the economics activities will constantly be performed with good and correct actions dealing with Al-Qur’an and As-Sunnah.

Abstract Background Injury to vital structures posterior to the sternum is a complication associated with redo sternotomy in congenital cardiac surgery. The goal of our study was a novel evaluation of real-time cine cardiovascular magnetic resonance (CMR) to predict the presence of significant retrosternal adhesions of cardiac and vascular structures prior to redo sternotomy in patients with congenital heart disease. Methods Twenty-three patients who had prior congenital heart surgery via median sternotomy had comprehensive CMR studies prior to redo sternotomy. The real time cine (RTC) sequence that was used is an ungated balanced steady-state free precession (bSSFP) sequence using SENSitivity Encoding for acceleration with real-time reconstruction. Spontaneously breathing patients were instructed to take deep breaths during the acquisition whilst increased tidal volumes were delivered to mechanically ventilated patients. All patients underwent redo cardiac surgery subsequently and the presence and severity of retrosternal adhesions were noted at the time of the redo sternotomies. Results Median age at the time of CMR and operation were 5.5 years (range, 0.2–18.4y) and 6.1 years (range, 0.3–18.8y) respectively. There were 15 males and 8 females in the study group. Preoperative retrosternal adhesions were identified on RTC in 13 patients and confirmed in 11 (85%) at the time of surgery. In only 2 patients, no adhesions were identified on CMR but were found to have significant retrosternal adhesions at surgery; false positive rate 15% (CI 0.4–29.6%), false negative rate 20% (CI 3.7–36.4%). The total classification error of the real time cine sequence was 17% (CI 1.7–32.4%) with an overall accuracy of 83% (CI 67.7–98.4%). Standard breath-hold cine images correlated poorly with surgical findings and did not increase the diagnostic yield. Conclusions RTC imaging can predict the presence of significant retrosternal adhesions and thus help in risk assessment prior to redo sternotomy. These findings complement the surgical planning and potentially reduce surgical complications .

In Italy, at the end of the 1970s, methadone hydrochloride was introduced for the treatment of opioid use disorder, in the form of a racemic mixture consisting of levomethadone and dextromethadone.In 2015 Levometadone was introduced, a new formulation marketed in Italy for the treatment of opioid use disorder in 2015.The article aims to bring the experience of an Italian Addiction Centre back to the use of this new formulation in the "real life" analyzing the efficacy, the trend of adverse events and pharmacological iterations in a context in which the treated population often uses besides the opiates, cocaine and alcohol, are burdened by a relevant physical and psychic comorbidity and frequently have a prescribed polypharmacy.
