Abstract: The Report of the National Broadband Task Force (2001) raised the prospect of extending broadband capability to virtually all Canadian communities within the next few years. The prototype for Canada as a broadband-connected nation is already under way in the province of Alberta, where, within the next two years, 95% of communities will be provided with broadband connectivity. It is expected that the Alberta SuperNet will provide these communities with higher levels of both government information and social services. It is unclear, however, how the network will impact such things as higher education, economic development, and cultural identity. This paper reports on a three-year federally funded research project studying the broad social impact of the SuperNet experiment. Résumé : Le Rapport du Groupe de travail national sur les services à large bande (2001) soulève la possibilité d'étendre la capacité à large bande à presque toutes les communautés canadiennes dans les prochaines années. Le prototype pour un Canada qui soit une nation reliée par connexions à large bande progresse déjà dans la province de l'Alberta, où, dans les deux prochaines années, 95% des communautés auront une connectivité à large bande. On s'attend à ce que l'Alberta SuperNet soit en mesure d'accorder à ces communautés des niveaux accrus d'information gouvernementale et de services sociaux. Il n'est pas clair, cependant, quel effet ce réseau aura sur des secteurs comme l'enseignement supérieur, le développement économique et l'identité culturelle. Cet article rend compte d'un projet de recherche de trois ans de durée subventionné par le gouvernement fédéral qui étudie l'impact social général de l'expérience SuperNet.

The frequent use of antimicrobials in pediatric patients has led to a significant increase in multidrug-resistant bacterial infections among children. Antimicrobial stewardship programs have been created in many hospitals in an effort to curtail and optimize the use of antibiotics. Pediatric-focused programs are necessary because of the differences in antimicrobial need and use among this patient population, unique considerations and dosing, vulnerability for resistance due to a lifetime of antibiotic exposure, and the increased risk of adverse events. This paper serves as a position statement of the Pediatric Pharmacy Advocacy Group (PPAG) who supports the implementation of antimicrobial stewardship programs for all pediatric patients. PPAG also believes that a pediatric pharmacy specialist should be included as part of that program and that services be covered by managed care organizations and government insurance entities. PPAG also recommends that states create legislation similar to that in existence in California and Missouri and that a federal Task Force for Combating Antibiotic-Resistant Bacteria be permanently established. PPAG also supports post-doctoral pharmacy training programs in antibiotic stewardship.

Background The Task Force classification (TFC) identifies weaning from mechanical ventilation (MV) in adults as: simple (SW), difficult (DW) and prolonged (PW). The usefulness of this classification is unknown in pediatric patients. Methods Prospective, descriptive, multicenter study in 29 pediatric intensive care units from Argentina during a three-month period including all admitted patients who required MV and met weaning criteria. The evolution of these patients was analyzed according to TFC. Results 474 patients were included, 454 (95.8 %) of whom were successfully extubated. Eighty percent (n=370) were extubated after the first attempt and classified as SW, 84 (18,2%) were DW and only 7 (1,5%) faced a PW. Successful extubation on the first trial was 77.9% (n=356) and for subsequent trials, 56.7%, 64.5% and 66.7%. The duration of MV (in days) by group was [median (interquartile range)]: 5.4 (2.8-9.7) (SW), 10.6 (7.7-18.8) (DW) and 27.4 (15.1-28.4) (PW). Fourteen patients underwent tracheostomy without any weaning attempt, 3 patients were on MV at the end of the study and 3 patients died without any attempt. The only variable associated with PW and DW (as a single group, with SW as reference) in multivariable analysis was ventilator-associated pneumonia (OR 2.58, 95%CI 1.01-6.11). Conclusions We observed a low prevalence of PW. Patients with PW and DW showed no significant differences from patients with SW, albeit a higher incidence of VAP.

ABSTRACT Soon after the Exxon Valdez spill, Chevron assembled an Oil Spill Response Task Force to coordinate activities among its operating companies and to enhance its prevention, preparedness, and response capabilities. This paper explains several of the programs developed and actions taken.
