This Chapter focuses in the electrophysiological bases to support Trans Palpebral Electrical Stimulation TPES as a new alternative to control Intraocular Pressure IOP. Primary open Angle Glaucoma POAG is described in our approach as a dysfunction of the membrane potential of TM cells due to the dysfunction of the Maxi potassium depended Calcium Channels BKCa2+ of the Trabecular Mesh TM. We review through the paper the main contributions about Trabecular mesh dysfunction related with Voltage dependent ionic channels. We also present in this paper new results in controlling intra ocular pressure IOP during one year of trans palpebral Electric stimulation in patients with Primary open-angle glaucoma (POAG).

Purpose: To compare between the results of rectangular and triangular shaped scleral flaps in trabeculectomy for primary open angle glaucoma. Study design: Quasi experimental study Place and Duration of Study: Khalid eye clinic, Karachi, from July 2018 to June 2019. Methods: This study included 24 patients of either gender and age from 40 to 65 years. Patients with a diagnosis of primary open angle glaucoma refractory to medical treatment were included by convenient sampling technique. They were divided into two groups. Group A underwent trabeculectomy with rectangular shaped scleral flap whereas Group B underwent trabeculectomy with triangular shaped scleral flap. The main outcome measures were intraocular pressure reduction, anterior chamber depth and post-operative complications. All patients were followed up for a period of two months. Results: Mean age was 52.1± 6.72 years. Mean pre-operative intraocular pressure in group A was 24.4±2.13 mmHg and 23.5±1.64 mmHg in group B. Mean post-operative IOP was 11.6±1.51 mmHg in group A and 13.4±1.67 mmHg in group B with p-value of 0.11. Pre-operative anterior chamber depth (ACD) was grade 4 both groups. It remained same in all patients of group A but two patients of group B changed to grade 3 ACD. Two patients of group B developed hypotony on the first post-operative day. They did not require revision suturing and were managed conservatively. Conclusion: Both types of scleral flap are effective in reducing IOP but rectangular scleral flap reduced IOP more as compared to triangular scleral flap but the difference was not statistically significant. Background: Raised intraocular pressure (IOP) is a strong risk factor for development of glaucoma. If the condition is detected early enough it is possible to arrest the development or slow the progress with medication that lowers IOP. Travoprost is a newer prostaglandin analogue lowers IOP by facilitating outflow of aqueous humor.Methods: This was a prospective, randomized, open labelled, parallel group study, in which sixty cases of newly diagnosed primary open angle glaucoma (POAG) were included. After baseline clinical evaluation 30 subjects of glaucoma were treated with timolol 0.5% eye drop while another 30 subjects were given travoprost 0.004% eye drop and followed for 1 month to see IOP lowering efficacy of the two drugs. Statistical analysis was done using Open Epi 2.3 version for paired t test and unpaired t test.Results: In subjects treated with timolol eye drop mean reduction of IOP from baseline to week 4 was by 6.6mmHg while that of travoprost treated subjects was by 8.07mmHg. There was significant reduction in mean IOP of study subjects in travoprost group as compared to timolol group (P<0.05).Conclusions: Both timolol 0.5% and travoprost 0.004% eye drops are effective in reducing intra ocular pressure in POAG. On comparison travoprost was found to be statistically superior to timolol in lowering IOP in patients with POAG.

Deterioration of vision in elderly people is a major health issue. Currently, the rates of eye disease such as cataract, macular degeneration, diabetic retinopathy and glaucoma are highest in older group. Primary open angle glaucoma is an Insidious and chronic vision - threatening eye aliment due to neuro- retino -optic nerve degeneration, which may be due to raised intra ocular pressure (IOP) or older age or due to independent factors. Classic treatment can delay progression of glaucomatous optic neuropathy by lowering intra ocular pressure (IOP) medically or surgically, but it has its own limitations and interdisciplinary research will be necessary to advance understanding, diagnosis and treatment of this neurodegenerative disease. Main aim of this study is to evaluate the neuro-protective effect of Ayurvedic line of management in primary open angle glaucoma. A multidisciplinary approach from Ayurvedic line of medicine was employed for a diagnosed case of Primary open angle glaucoma (POAG) for 2months. It showed marked improvement in symptoms. This clinical study concludes that Ayurvedic treatment protocol along with Anti glaucoma eye drops are more effective in reducing the intra ocular pressure and control the progression. Hence early diagnosis & proper management can prevent, arrest, OR delay the progression of Primary open angle glaucoma (POAG).

Background: Primary Open-Angle Glaucoma (POAG) the most common form of glaucoma is a neurodegenerative disease, which is the third most common cause of blindness worldwide. It is estimated that 60 million people in the world are affected by this disease and 8.4 million are bilaterally blind. Among the various factors that have been implicated in the pathophysiology of this disease is infection with Helicobacter pylori (HP), a Gram-negative bacterium that is commonly found in stomach and present in approximately one-half of the world’s population. Establishment of such a causal correlation will probably have important practical applications as the eradication of H. pylori might lead to developments in the treatment of glaucoma. Objectives: To investigate the association between Primary Open-Angle Glaucoma (POAG) and Helicobacter Pylori infection and to observe fluctuations in intra ocular pressure after Helicobacter Pylori infection eradication. Design: Duration based, prospective observational study. Participants: 50 patients with documented Primary Open-Angle Glaucoma (POAG) as case group and 50 non-glaucoma participants as control group. Methods: Upper gastrointestinal endoscopy was performed to evaluate macroscopic abnormalities, and gastric mucosal biopsy specimens were obtained for the presence of H. pylori infection tested by Rapid Urease Test (RUT). All subjects underwent detailed ocular examinations including visual acuity, slit-lamp examination, fundoscopy, intra-ocular pressure recording, gonioscopy, GHT to assess visual fields and OCT of optic nerve head. Results: In 90% of POAG patients of case group and in 68% of non-glaucoma participants of control group Helicobacter pylori infection was detected by RUT (P-Value=0.007). Conclusion: H. pylori infection is more frequent in glaucoma patients, perhaps more so in those of Indian ethnicity. It may play a role as a secondary aggravating factor or even may be the primary cause. The establishment of such a causal relationship will probably have important practical applications as the eradication of H. pylori might lead to developments in the treatment of glaucoma.
