Prognostic significance of CD9 expression differs between tumour cells and stromal immune cells, and depends on the molecular subtype of the invasive breast carcinoma

PURPOSE: To assess the frequency of expression and the prognostic significance of a hypoxia-regulated marker, carbonic anhydrase IX (CA IX), in a cohort of patients with invasive breast cancer. PATIENTS AND METHODS: CA IX expression was evaluated by immunohistochemistry with a murine monoclonal antibody, M75, in a series of 103 women treated surgically for invasive breast cancer. The majority of patients were treated with adjuvant hormonal or chemotherapy. The frequency of CA IX expression, its association with recognized prognostic factors, and the relationship with outcome was evaluated by univariate and multivariate statistical analyses. RESULTS: CA IX expression was present in 49 (48%) of 103 cases. The level of CA IX expression was found to be significantly associated with tumor necrosis (P < .001), higher grade (P = .02), and negative estrogen receptor status (P < .001). Furthermore, CA IX expression was associated with a higher relapse rate (P = .004) and a worse overall survival (P = .001). By multivariate analysis, CA IX was also shown to be an independent predictive factor for overall survival (hazard ratio, 2.61; 95% confidence interval, 1.01 to 6.75, P = .05). CONCLUSION: CA IX expression was associated with worse relapse-free survival and overall survival in an unselected cohort of patients with invasive breast carcinoma. The potential role of CA IX as a marker of hypoxia within breast carcinomas was also indicated by a significant association with necrosis. Further work assessing its prognostic significance in breast cancer is warranted, particularly interactions with radiotherapy and chemotherapy resistance.

Background: Breast cancer is a leading cause of cancer death  in women worldwide. Breast carcinoma is currently managed by assessing clinicopathological features. Elucidation of molecular mechanisms of pathogenesis of breast carcinoma  may lead to the development of new targeted therapies, particularly in triple negative cancers. Literature shows a few studies on the expression of calretinin  in breast carcinoma particularly in basal like type and its prognostic significance.  In this study, authors are trying  to assess the expression of  a new marker calretinin in different molecular subtypes of invasive carcinoma breast.Methods: This study was done in  107 cases of invasive carcinoma breast specimens received in  Department of Pathology, Government Medical college, Kottayam from December  2017 to May 2019.Results: Among the molecular subtypes, Basal like tumours showed 68.4% of cases with high level and 31.6% of cases with low level calretinin expression which is comparable with the study by Farrag et al. All the other molecular subgroups showed predominantly low level of calretinin expression.Conclusions: Different molecular subtypes of invasive carcinoma breast showed varied calretinin expression. High level calretinin expression was significantly associated with grade 3 (p value = 0.002), ER negativity (p = 0.004), PR negativity (p = 0.018)  and Basal like molecular subtype (p : <0.001). This suggests that calretinin might play a role in pathogenesis of basal like breast carcinomas.

INTRODUCTION: Triple negative breast carcinoma (TNBC) molecular subtype and its variant; basal-like triple negative (BLTN) carcinoma is an established poor prognostic indicator. The aim of this study is to analyse the survivin expression in TNBC and BLTN, and relating the results with clinicopathological parameters. MATERIALS AND METHODS: We conducted a cross sectional study using 94 archived formalin-fixed paraffin embedded tissue blocks of invasive breast carcinoma, no special type (NST). Estrogen receptor (ER), progesterone receptor (PR) and human epidermal growth receptor 2 (HER2) were used as surrogate markers to classify the cases into molecular subtypes. Samples with triple negative phenotype (ER, PR and HER2 negative) were stained with CK 5/6 to identify the BLTN subtype. All the samples were also immunostained for survivin. RESULT: Out of 94 cases, 41.5% (39 cases) were TNBC. Among the TNBC cases, only 41.0% (16 cases) were BLTN subtype when they found to be positive for CK 5/6. Among 94 cases of invasive breast carcinoma, 28.7% (27 cases) were survivin positive with (53.8%) 21 cases were TNBC and (11%) 6 cases were non-TNBC (p< 0.001). Among 16 cases of BLTN subtype, only 8 cases were survivin positive (p = 0.752). Survivin expression was also statistically significant with tubule formation (p=0.029), nuclear pleomorphism (p=0.008), tumour grade (p=0.010), ER status (p< 0.001) and PR status (p=0.001). CONCLUSION: Survivin expression was statistically significant in invasive breast carcinoma. Even though the expression was significantly high in TNBC, it is not related to whether it is a basal-like or non-basal-like variant.
