Endoscopic resection, particularly endoscopic submucosal dissection (ESD), for colorectal cancers enables a precise pathological diagnosis and safe R0 resection. The recurrence rate after ESD is generally extremely low, with annual surveillance colonoscopy recommended. However, surveillance may not be considered for super-elderly patients owing to their condition. This is a case report of an 85-year-old man in whom curative resection was achieved for an intramucosal adenocarcinoma with ESD. The patient presented with a hypoechoic mass located in his lower right abdomen, diagnosed via surveillance abdominal ultrasound. He had undergone curative ESD for intramucosal cecal cancer 2 years prior. Colonoscopy revealed a type 2 epithelial tumor at the proximal aspect of the ESD scar. Ileocolic resection with lymph node dissection was performed. An epithelial tumor and well-differentiated adenocarcinoma but not a submucosal tumor was detected in the mucosal layer. The lesion was diagnosed not as a local recurrence after ESD but as a newly emerged original advanced cancer. After ESD for colorectal cancer, a newly developed advanced cancer may occur at the site of the ESD scar in a shorter term than usual. Surveillance colonoscopy after ESD is necessary even for super-elderly patients.

Abstract Background Globally, different age groups in the elderly population have experienced major shifts over time. Human life expectancy doubled from the 19th to the twentieth century and has increased to 80 years in the twenty-first century. These conditions imply economic challenges and the increasing prevalence of certain health conditions. Old age is associated with increased care needs in various aspects of daily life. This study assessed the health care needs of elderly patients with lung, liver, and colorectal cancer in Taiwan and analyzed the factors underlying their needs. Methods This cross-sectional descriptive survey assessed 234 elderly patients with diagnosis of lung, liver, and colorectal cancer in Taiwan. We investigated their health care needs and daily living functions by using the Supportive Care Needs Survey and Karnofsky Performance Status, respectively. Results Patients required the most assistance in physical functioning and daily living. Patients aged ≥85 years required more care than those aged 65–74 years in terms of information access and sexuality needs. Patients with poor functional status required more care than those capable of undertaking normal activities. Patients diagnosed as having liver cancer required more care than those with lung or colorectal cancer. Patients with advanced cancer required more physical and daily care than those with early-stage cancer. Conclusions Patients’ health care needs differed with age, primary cancer site, and functional status. Patients aged ≥85 years and those with poor function, primary liver cancer, and advanced cancer had higher care needs.

AbstractRecently, new approaches to wildlife management are being developed, such as coexistence management and convivial conservation. These approaches aim to shift management practices from mitigating human–wildlife conflicts towards cohabitation and explore mutual benefits. To align empirical research to these new approaches, we argue for the relevance of positive psychology theory to inspire and structure research into the benefits of human–wildlife interactions. Positive psychology suggests three pathways through which human–wildlife interactions may lead to happiness and well-being: pleasure, engagement, and meaning. Applying these pathways to human–wildlife research may (i) structure existing research into the benefits of human–wildlife interactions, (ii) disclose unidentified benefits of human–wildlife interactions, and (iii) unravel mechanisms which make experiencing and protecting wildlife worthwhile and rewarding. Also, we suggest a potential feedback loop between wildlife experiences, happiness and well-being, and pro-environmental behaviours. More in-depth research into these mechanisms may improve our understanding of attitudes towards conservation of wildlife and its habitat and may suggest strategies to strengthen stewardship actions and public support for conservation strategies. Together, these strands of research could initiate research into what could be called a “Positive Ecology”.
