Acne fulminans (AF), also known as acne maligna, is a rare painful ulcerative form of acne with an abrupt onset and systemic symptoms. Its incidence appears to be decreasing, possibly because of earlier and better treatment of acne. This report highlights a case on a necrotizing facial wound due to AF that was successfully treated with oral prednisolone and antimicrobial medication.

Acne fulminans (AF) is a rare, acute, and severe form of acne vulgaris often associated with systemic symptoms. Its treatment is challenging and controversial. We report a case of isotretinoin-induced AF (IIAF) in a 12-year-old boy resistant to traditional therapies (oral steroids and isotretinoin). The patient was successfully treated with adalimumab that can be considered an effective off-label option in the treatment of resistant IIAF in children.

Rubella, a common viral disease of childhood is characterized by maculopapular rash, lymphadenopathy, and mild respiratory and systemic symptoms. Although the association between rubella and acute appendicitis is uncommon, one should be aware of their simultaneous occurrence. During a rubella epidemic in the Tel Aviv area, two children were operated on because of an attack of acute appendicitis. R.S. a 10-year-old boy had rubella of two days duration and abdominal pain and vomiting which had started one day prior to admission.

Isotretinoin-induced acne fulminans without systemic symptoms (IIAF-WOSS) is an uncommon clinical variant of acne, not exhibiting systemic symptoms but with potentially severe skin lesions. Some authors believe that its occurrence is dose-dependent. Herein, we present the case of a sixteen-year-old boy with IIAF-WOSS, which developed two weeks after starting treatment with isotretinoin 0.6 mg/kg/day. The patient was successfully treated with a systemic steroid. IIAF-WOSS may cause significant disfiguring scarring, thus the physician needs to be aware of this condition, even early with low doses of isotretinoin.
