ABSTRACT Spindle Cell Lipoma (SCL) is an uncommon benign tumor of adipose tissue that is usually super􀃶cially located in the posterior neck, back and shoulder region. We report a rare case of SCL in a 39 years old male presenting as neck swelling with c/o stridor and dysphagia. The contrast enhanced computed tomographic scan of neck showed an external and internal part of the swelling traversing through the thyrohyoid membrane suggestive of Mixed Pyolaryngocele. The tumor was excised by a trans-cervical approach but the post operative histopathology was found to be spindle cell Lipoma. We report this case due to its atypical presentation and location. The author describes a benign soft tissue tumor that could be easily mistaken for sarcoma. It represents a combined feature of solitary fibrous tumor and spindle cell lipoma. The clinical presentation, diagnosis, and management of this lesion are discussed. It highlights the importance of proper diagnosis to prevent unnecessary and ineffective treatment by clinicians as the complete excision of this lesion is the treatment of choice and recurrence is very unlikely. It is believed that this case is the first reported case of dendritic myxofibrolipoma occurring in the lower lip mucosa in an Omani patient.
