Annually a large amount of goods has been transported over the world and the most of the transportation is carried by seaborne. Although the seaborne trade is considered to be the most efficient – economically and environmentally friendly way of transportation an enormous amount of fossil fuels is used to fulfil the operation of the merchant fleet. Although we are living in times where efforts are directed to improvement of the state of the environment and reduction of the negative impact of the Greenhouse Gases, the fossil fuels still remain the main energy source used to run the internal combustion engines widely used in the ships nowadays. A well-known fact is that the fuel burning leads to significant harmful emissions generation causing a serious negative effect on the surrounding us environment. The introduced by IMO and MEPC requirements regarding the Greenhouse Gases (GHG) imposes for development and application of new criteria when selecting the ship’s propulsion plant. In the current article will be observed three ship groups consisting of real container vessels. The purpose is to analyse the impact caused by the fuel type used in the ship’s engines on the GHG generation and the economic indicators related with fuel costs

We are proposing a parametric model to rate insurance for vehicles belonging to a fleet. The tables of premiums presented take into account past vehicle accidents, observable characteristics of the vehicles and fleets, and violations of the road-safety code committed by drivers and carriers. The premiums are also adjusted according to accidents accumulated by the fleets over time. The proposed model accounts directly for explicit changes in the various components of the probability of accidents. It represents an extension of bonus malus-type automobile insurance models for individual premiums (Lemaire, 1985; Dionne and Vanasse, 1989 and 1992; Pinquet, 1997 and 1998; Frangos and Vrontos, 2001; Purcaru and Denuit, 2003). The extension adds a fleet effect to the vehicle effect so as to account for the impact that the unobservable characteristics or actions of carriers can have on truck accident rates. This form of rating makes it possible to visualize what impact the behaviors of owners and drivers can have on the predicted rate of accidents and, consequently, on premiums. The results are compared to those of the semiparametric approach.

Objective: Motor vehicle accidents (MVA) may result in intractable disability. This paper investigates posttraumatic stress disorder (PTSD) as a potential cause. Method: The literature was reviewed for recent studies on prevalence, symptom profile, and outcome of PTSD. Results: PTSD is prevalent in roughly 10% of survivors of MVAs during the first year. Comorbid depression and pain are common. Medical complications, psychophysiological reactivity, and possibly litigation may slow remission. Phobic symptoms can persist for years. Mood disturbance may augment the impact of pain on daily living and on self-perceived disability. Conclusion: Recently developed screening instruments, structured interviews, and behavioural approach tests yield quantitative and reliable assessments of symptom severity. Cognitive–behavioural intervention and antidepressants may improve coping, ease fear, and reduce the impact of pain.

Objective. To study the impact of helmet use on outcomes after recreational vehicle accidents.Methods. This is an observational cohort of adult and pediatric patients who sustained a TBI while riding a recreational vehicle. Recreational vehicles included bicycles, motorcycles, and all-terrain vehicles (ATVs), as well as a category for other vehicles such as skateboards and scooters.Results. Lack of helmet use was significantly associated with having a more severe traumatic brain injury and being admitted to the hospital. Similarly, 25% of those who did wearing a helmet were admitted to the ICU versus 36% of those who did not (P=0.0489). The hospital length of stay was significantly greater for patients who did not use helmets.Conclusion. Lack of helmet use is significantly correlated with abnormal neuroimaging and admission to the hospital and ICU; these data support a call for action to implement more widespread injury prevention and helmet safety education and advocacy.

Background The Coronavirus Disease 2019 (COVID-19) pandemic has had a dramatic impact on individual and societal behaviors, as well as on health care systems. It confers a unique opportunity to examine the relationship among disease, policies, and patterns of activity, as well as their impacts on surgical unit functionality. This study aims to compare the distribution and patterns of injury at a tertiary hand surgery trauma center before and during the COVID-19 pandemic. Methods A retrospective analysis of all patients presenting to the Royal North Shore Hospital hand surgery service in the 5-week period from March 16 to April 21 in 2019 and 2020 was undertaken, forming 2 cohorts for comparison. Demographic, injury, and operative data were collected and compared descriptively using comparative statistics. Results There were 114 primary operative presentations during the 5-week period in 2020, representing a 27.4% decrease from the 157 presentations during the equivalent period in 2019. There was an increase in the proportion of emergency presentations from 73.9% in 2019 to 85.1% in 2020 ( P = .03), with a corresponding decrease in elective presentations during 2020. The incidence of sporting injuries and motor vehicle accidents decreased in 2020, whereas falls and accidents involving knives and tools remained relatively constant. Operating times decreased in 2020, whereas the length of hospital stay remained constant. Conclusions The COVID-19 pandemic and consequent restrictions of activity have had substantial impacts on the patterns of hand trauma and its management. These insights have implications for staff and resource management during times of social disruption in the future.

This paper examines the need for a systematic way of detecting individuals likely toencounter moderate to severe psychological sequelae after being injured in a motorvehicle accident. It presents the findings of assessments of consecutive adult admissionsduring a six-month period to the Accident and Emergency Department of RoyalMelbourne Hospital, a major acute hospital in Melbourne, within two days of beinginjured in a motor vehicle accident. Seventy of the 250 subjects surveyed completedthe General Health Questionnaire 12 and the Impact of Event Scale questionnaire,with 77- per cent found likely to have had a psychiatric disorder. Seventy-four per centhad at least moderate levels of intrusion or avoidance phenomena. Forty-four per centhad severe levels of one or the other. Prevalence of these psychological sequelae wasgreater than that found in several recent studies on the effects of motor vehicleaccidents.A small number (11?15- per cent) sought or received professional counselling withinthe first three months. This group did not differ, according to levels of intrusion,avoidance, or scores on the GHQ 12, to subjects who did not seek or receive suchassistance. Results identify the necessity of investigating and implementing methodsof detecting and treating the many who apparently sustain significant psychologicalsequelae following injury in a motor vehicle accident.
